Na reabilitacao, a efetividade da avaliacao terapeutica e importante para que sejam tracadas meta... more Na reabilitacao, a efetividade da avaliacao terapeutica e importante para que sejam tracadas metas, objetivos, prognostico e ajuste da terapia. Devido a ausencia de uma escala especifica de avaliacao da funcionalidade de pacientes com lesao medular (LM) na cultura e linguagem brasileira, faz-se necessario traduzir e adaptar o Spinal Cord Independence Measure III (SCIM III), instrumento utilizado em outros paises. Este estudo teve como objetivos adaptar transculturalmente e validar o questionario SCIM III no Brasil. Os resultados obtidos demonstraram a confiabilidade, reprodutividade e a validade do SCIM III e indicam que e uma medida eficiente para avaliacao funcional em LM.

Multiple sclerosis (MS) is a demyelinating, inflammatory illness, that attack the white matter of... more Multiple sclerosis (MS) is a demyelinating, inflammatory illness, that attack the white matter of the central nervous system, and abnormal vestibular sensations (vertigo, disequilibrium) are frequent. The vestibular rehabilitation (VR) is determined by mechanisms of adaptations, neural substitutions and compensations. This study evaluated the improvement of the central or peripheral vertigo in patients with relapsing-remitting MS submitted to the VR (exercises of Cawthorne-Cooksey), through the scale of Berg and Dizziness Handicap Inventory (DHI). In this sample of 4 cases the VR, carried through in a period of 2 months, demonstrated the improvement in 3 patients according to the Berg scale and in 2 patients considering that of the DHI.

Fatigue is one of the most common disabling symptoms in multiple sclerosis (MS). The aim of this ... more Fatigue is one of the most common disabling symptoms in multiple sclerosis (MS). The aim of this study was to evaluate the fatigability on patients with MS by the application of hand grip isotonic and isometric exercises with dynamometer. As results the fatigability, the isometric strength and time were statistically similar in the control group and in MS. We conclude that although fatigue is a frequent subjective complaint on MS, the fatigability and the recover after exercises seems to be normal.

Assim, ha a necessidade de se conhecer esta populacao e suas incapacidades para permitir o tratamento mais especifico e global. Objetivo: Este estudo deseja determinar a presenca de incapacidades motoras, cognitivas, dolorosas e mistas entre a populacao de internados no Hospital Central da Santa Casa de Misericordia de Sao Paulo. Metodos: O trabalho consiste em estudo observacional transversal de busca ativa, no qual foram estudados pacientes internados no Hospital Central da Santa Casa de Sao Paulo. Resultados: Foram estudados 198 pacientes no total. Incapacidades foram encontradas em 70% dos pacientes estudados. OBJECTIVE The aim of this study is a cross-cultural adaptation and validation of the modified fat... more OBJECTIVE The aim of this study is a cross-cultural adaptation and validation of the modified fatigue impact scale for Portuguese (MFIS-BR). METHOD The MFIS was translated to Portuguese and re-translated to English. Two pilot studies had been carried through to obtain the MFIS-BR, which was applied to 57 consecutive stable MS patients and 45 healthy controls. The retest was applied 30 days later. RESULTS In analyzing the statistics the internal consistency reability was similar to the original scale (0.74-0.86). The MFIS-BR showed ability to identify the different groups. Concerning the reliability the interclass correlation coefficient showed an excellent agreement (0.264-1.0). CONCLUSION The MFIS-BR fills the criteria of applicability, sensibility which are similar to the original version.

To evaluate the frequency and characteristics of misalignments (MAs) in the retinal nerve fiber l... more To evaluate the frequency and characteristics of misalignments (MAs) in the retinal nerve fiber layer (RNFL) analysis protocol of spectral-domain optical coherence tomography (Cirrus) and determine factors associated with MAs. Three hundred eyes (162 normal and 138 glaucomatous eyes) were included in this cross-sectional study. The MAs were considered limited when they affected only part of the scan line, and complete (CMA) when they were observed in the entire scan line. A subgroup (153 cases) with repeated scans was analyzed to compare the RNFL thicknesses in the scans with and without CMAs. Two hundred ninety-nine limited MAs were found in 140 eyes (46.7%) and 151 CMAs were found in 91 eyes (30.3%). The frequency and number of CMAs were significantly related to age (P<0.05). Seventy-two CMAs were in the measurement ring in 48 eyes, more frequently in the 3 and 9-o'clock positions (P=0.001) and the horizontal quadrants (P=0.001). Among the repeated scans, the number of cases with CMAs was similar to the first scan (P=0.32). No significant differences were found in global or quadrant RNFL thickness between scans with and without CMAs. CMAs were present in the first or second scans in about 30% of cases and were related to older age. CMAs were more frequently in horizontal meridians and quadrants. No differences in RNFL thickness were found between scans with and without CMAs in the same patients. Scans with CMAs in the measurement ring can be considered in the RNFL evaluation.

The purpose of the study was to evaluate the influence of the examiner&am... more The purpose of the study was to evaluate the influence of the examiner's experience on the reproducibility of retinal nerve fiber layer (RNFL) measurements obtained with Cirrus optical coherence tomography (OCT) and Stratus. Fifty-one normal and glaucomatous eyes of 51 participants were included. Two examiners (1 novice, 1 experienced) obtained 2 scans using both OCTs. For quantitative measurements, Bland and Altman limits of agreement were evaluated. For qualitative classifications, κ coefficients were calculated. Signal strength was higher with Cirrus than with Stratus (P<0.05). Signal strength was higher in scans performed by the experienced examiner than in those carried out by the inexperienced examiner in Stratus but not in Cirrus. RNFL measurement differences between Cirrus and Stratus were influenced by the examiner for the inferior (P=0.02), superior (P<0.001), and temporal quadrants (P=0.009). The RNFL quantitative agreement of examiners was higher in Cirrus than in Stratus. The qualitative agreement (κ coefficients) of both examiners in the RNFL classification were almost perfect with Cirrus (in the average, superior, and inferior quadrants), and moderate with Stratus (only in average and inferior quadrant). The signal strength is independent of the examiner's experience in Cirrus but not in Stratus. RNFL measurements obtained by both examiners were more reproducible with Cirrus than with Stratus. The differences in RNFL measurements between both OCTs were related to the examiner's experience in all 3 quadrants. Agreement between operators in the RNFL classification was higher with Cirrus than Stratus.

A esclerose múltipla (EM) é doença desmielinizante, inflamatória, que acomete a substância branca... more A esclerose múltipla (EM) é doença desmielinizante, inflamatória, que acomete a substância branca do sistema nervoso central, e sensações vestibulares anormais (vertigem, desequilíbrio) são freqüentes. A reabilitação vestibular (RV) é determinada por mecanismos de adaptações, substituições e compensações neurais. Este estudo avaliou a melhora da vertigem central ou periférica em pacientes com EM remitente-recorrente submetidos à RV (exercícios de Cawthorne-Cooksey), através da escala de Berg e Dizziness Handicap Inventory (DHI). Nesta amostra de 4 casos a RV, realizada em um período de 2 meses, demonstrou a melhora em 3 pacientes avaliados pela escala de Berg e em 2 pacientes quando avaliados pela DHI.
