Posterior decompression plus posterior reconstruction (laminoplasty) is a useful surgical method ... more Posterior decompression plus posterior reconstruction (laminoplasty) is a useful surgical method for treatment of cervical compressive myelopathy. There are many laminoplasty procedures. This paper describes the Z-shaped laminoplasty developed by Hattori in 1971, and presents a clinical follow-up of the authors' experience with 130 patients. The procedure involves grinding the laminae down with an air drill and making a Z-shaped cut into the thinned laminae without excising the laminae. This technique enlarges the spinal canal. The purpose of this technique is to decompress the spinal cord and at the same time maintain clinical stability. Postoperative results were satisfactory without any major complication. Follow-up study was conducted in 78 cases with a minimal follow-up period of more than 2 years. Satisfactory clinical results were maintained for long periods postoperatively, and the enlargement of the spinal canal was well maintained as demonstrated on follow up X-ray study.

T... more Hand reconstruction after wide resection of digital malignant tumors is still very challenging. The technique of adjacent digital ray transposition has been used for digital defects arising from trauma, but few papers have focused on reconstruction following resection of malignant tumors of the finger. The indications, complications and functional outcomes with this approach are discussed. Four patients underwent primary reconstruction with adjacent digital ray transposition after tumor resection. Two were men and two were women and the median age was 62 years. The malignant tumors were low-grade chondrosarcomas of the metacarpal bone in two cases, epithelioid sarcoma at the fingertip in one case and synovial sarcoma at the base of the thumb in the fourth case. Metacarpal osteotomies were rigidly stabilized with a plate and screws and with an intramedullary bone peg graft. Digit length and rotation were satisfactory and there were no non-unions. Local recurrence was not observed at the final follow-up. The mean musculoskeletal tumor society (MTS) score was 60% (range 48-80%). The results of the disabilities of the arm, shoulder and hand score were similar to the MTS score, with a mean score of 35 points (range 22.5-63.5). The worst result was for index-to-thumb transposition and the best was for index-to-middle. All patients experienced emotional difficulty with acceptance of a three-finger hand. Primary reconstruction with digital ray transposition produces acceptable functional outcomes after resection of malignant tumor. This procedure is best indicated for central single ray amputation but sometimes is associated with esthetical problems.

In a 4-year period, 17 consecutive patients with posttraumatic chronic regional pain syndrome wer... more In a 4-year period, 17 consecutive patients with posttraumatic chronic regional pain syndrome were treated with a new technique, Movelat manipulation therapy. At average follow-up of 8 months, satisfactory results were achieved in 15 patients (88%), but 2 patients, 1 with digital nerve injury and 1 with ulnar nerve injury, did not respond to the therapy. Factors associated with good clinical response include chronic regional pain syndrome type I, i.e., dystrophy produced by a trauma to the hand but not involving a specific nerve injury, early-stage disease (within 3 months after trauma), and involvement of the upper limbs. Complications were rare and mild (pain over the tourniquet site in 3%, temporary dizziness in 1%). This therapy is simple and safe and recommended for early treatment of chronic regional pain syndrome.

The nonverbal stimuli consisted of 5 pure tones, differing sequentially in sound frequency, which made up a characteristic form. This form had to be recognized by visual multiple choice. Tile duration of the sets of digits and of tones was 1.5 sec. In part 1, digits were presented to the right or left ear, simultaneously with a tonal sequence to the other ear. Parts 2 and 3 were control tasks with only digits or only tonal sequences to both ears. shows the average number of correct responses taken from 38 righthanded students. Significantly more digits are reported from the right ear than from the left in parts 1 and 2. Tonal sequences together with digits are equally well reported from both ears in part 1, whereas tonal sequences to both ears yield a significant left ear superiority in part 3.

The immunosuppressive effect of combined therapy using FK-506 and 15-deoxyspergualin was investig... more The immunosuppressive effect of combined therapy using FK-506 and 15-deoxyspergualin was investigated in rat limb allotransplantation. The right hindlimb of an inbred Dark Agouti rat (RT1a) was transplanted to a Lewis rat (RT1l) and observed for 120 days. Eighty-eight transplantations were performed. Rejection of the grafts was evaluated histologically as well as macroscopically. A pathologic grading system was used to rate the severity of rejection. Limb survival time in Lewis rats receiving FK-506 therapy for days was significantly prolonged by combined therapy with 15-deoxyspergualin. Histologic study showed practically no rejection in all the graft-composing tissues except the skin and bone marrow, in which there was rejection. These results suggest that tissues like bone, cartilage, and muscle may survive transplantation with short-term combination FK-506 and 15-deoxyspergualin therapy even after withdrawal of both agents.

In the management of upper type of brachial plexus injury, reconstruction to restore shoulder fun... more In the management of upper type of brachial plexus injury, reconstruction to restore shoulder function is accomplished by multiple nerve transfers. We used the accessory nerve to neurotize the suprascapular nerve in 12 patients (11 men, 1 woman) from 1989 to 2003. The average age at the time of operation was 28.1 years (range 16 to 53). The mean preoperative time was 3.6 months. The type of paralysis was C5-C6 type in four cases, C5-C7 type in five cases, and C5-C8 type in three cases. The average time of follow-up was 28.5 months. All the patients showed reinnervation of the supraspinatus and infraspinatus muscles that was confirmed by electromyogram. At the time of final followup, the average shoulder flexion was 70.4 degrees and abduction was 77.1 degrees. However, average shoulder external rotation was only 16.7 degrees. We compared the shoulder flexion and abduction in patients with or without paralysis of the serratus anterior muscle and found significantly better functional outcome in the latter group of patients. We, therefore, conclude that repair of long thoracic nerve is mandatory for achieving optimum shoulder function.

Ten patients with an amputated thumb had reconstruction by a free wrap-around flap taken from the... more Ten patients with an amputated thumb had reconstruction by a free wrap-around flap taken from the big toe, including the nail, and an iliac-bone graft as the core of the reconstruction. The long-term results showed that a better replica of the thumb was achieved. The results were quite satisfactory both cosmetically and functionally, but there were some new problems: resorption of the bone graft, bulging of the pulp, skin atrophy at the base of the thumb, and deformity of the nail in a few patients. Modifications of the flap design and of the shape of the bone graft eliminated these problems nearly entirely.

The clinical effects of cefoxitin (CFX) were evaluated in the prophylaxis of postoperative infect... more The clinical effects of cefoxitin (CFX) were evaluated in the prophylaxis of postoperative infections in the field of orthopaedics. The clinical response was good in 46 out of 50 patients; an efficacy rate of 92%. Four patients (8%) who did not respond to CFX were suffering from infections due to Mycobacterium tuberculosis (1), suspected Pseudomonas aeruginosa (1), and infection of unknown organism (2). A review was also made of recent trends among clinically isolated bacterial strains and their susceptibility to antibiotics in the field of orthopaedics. CFX is recommended as an antibiotic of first choice for the prophylaxis of postoperative infections in the field of orthopaedics.
