Purpose: To determine if surface treatment and cement selection for traditional 3 mol% yttria par... more Purpose: To determine if surface treatment and cement selection for traditional 3 mol% yttria partially stabilized zirconia (3Y-PSZ), "translucent" 5 mol% yttria-stabilized zirconia (5Y-Z), or lithium disilicate crowns affected their fracture load. Materials and Methods: Crowns with 0.8 mm uniform thickness (96, n = 8/group) were milled of 3Y-PSZ (Lava Plus), 5Y-Z (Lava Esthetic), or lithium disilicate (e.max CAD) and sintered/ crystallized. Half the crowns were either particle-abraded with 30 μm alumina (zirconias) or etched with 5% hydrofluoric acid (lithium disilicate), and the other half received no surface treatment. Half the crowns from each group were luted with resin-modified glass ionomer (RMGI, RelyX Luting Plus) and half were luted with a resin cement (RelyX Unicem 2) to resin composite dies. Crowns were load cycled (100,000 cycles, 100 N force, 24°C water) and then loaded with a steel indenter until failure. A 3-way ANOVA examined the effect of material, cement, and surface treatment. Post-hoc comparisons were performed with Tukey-Kramer tests. Results: Fracture load was signficiantly different for materials and cements (P < 0.001) but not surface treatments (P = 0.770). A Comparison of the Edentulous and Dentate Prosthodontic Patient for the Occurrence of Obstructiv... more A Comparison of the Edentulous and Dentate Prosthodontic Patient for the Occurrence of Obstructive Sleep Apnea The purpose of this study was to determine if there were any differences in the occurrence of obstructive sleep apnea in patients with teeth and without teeth. Forty middle-aged subjects were screened, 20 were wearers of maxillary and mandibular complete dentures, and 20 were dentate patients. All subjects were screened via five screening tools: the Epworth Sleepiness Scale survey, measurement of neck size, calculated body mass index, and an intraoral exam visualizing the throat and determining the presence or absence of snoring. The study found no statistical difference between the two groups for the prevalence of obstructive sleep apnea. However, the study did find a direct correlation between the five screening tools. In conclusion, while there is no difference between edentulous and dentate patients for the occurrence of obstructive sleep apnea, the screening tools are effective in determining the presence of obstructive sleep apnea and should be used during routine patient exams.

METHOD AND MATERIALS Written records for 250 active-care patients were randomly selected and analyzed for associations between CDH, and its resolution following occlusal equilibration. Patients in group A (treatment) and B (delayed treatment) received occlusal equilibration following the detection of verified CDH using the air indexing method. Group C patients were not detected with verified CDH during the study period of 17 years. The resolution of CDH was measured by the loss of a positive patient threshold response to air stimulation. RESULTS Significant associations existed between CDH, hyperfunction, parafunction, and abfractive lesions. A detailed description of the methods and dynamic clinical simulation of mandibular movements of a patient are demonstrated. Functional jaw movements on 3 days were recorded by JMT tracking system. The simulation was performed by merging CBCT and JMT data in the software SICAT Function suite. The condylar position simulated by SICAT Function suite was compared with real condyle position showed by a CBCT of the patient. The incisor ranges of functional movements were displayed by JMT tracking system. The visualization of patient-specific mandibular movement including the translation of the condyles was displayed after data merge. The etiology (chemical, friction, abfraction) of noncarious cervical lesion (NCCL) progression is... more The etiology (chemical, friction, abfraction) of noncarious cervical lesion (NCCL) progression is poorly understood. The purpose of this 5-year prospective clinical trial was to measure the relationship between NCCLs and various etiologic factors. After review board approval, 29 participants with NCCLs were enrolled. Polyvinyl siloxane impressions were made of each NCCL, and casts were poured at baseline, 1, 2, and 5 years. The casts were scanned with a noncontact profilometer, and 1-, 2-, and 5-year scans were superimposed over baseline scans to measure volumetric change in NCCLs. T-scan and Fujifilm Prescale films were used to record relative and absolute occlusal forces on teeth with NCCLs at the 5-year recall. Participant diet, medical condition, toothbrushing, and adverse oral habit questionnaires were given at the 5-year recall. The purpose of this article is to introduce an objective method for quantifying cervical dentin h... more The purpose of this article is to introduce an objective method for quantifying cervical dentin hypersensitivity. Air emissions from a standard air-water syringe or a syringe with a Fluid Control Block are directed toward the cervices of teeth at a 45-degree angle to the long axis of test teeth from a distance of 0.5 cm for 0.5 to 1.0 second. An air indexing method has been developed to quantify threshold patient response values for individual teeth to this defined air stimulus. The air indexing method, using the Fluid Control Block, offers the clinician objective information to compare cervical dentin hypersensitivity before and after treatment for this common, painful condition.

The size of the diastema, size and proportion of the clinical crowns, tooth preparation, and dental occlusion require special attention; an excessive frenum attachment requires additional care and coordination of the treatment. This article describes the placement of six resin-bonded porcelain veneer restorations in a patient with a large diastema between the maxillary central incisors and associated frenum and soft tissue involvement. The learning objective of this article is to demonstrate the importance of pretreatment planning and problem solving in achieving predictable final restorations.

The prosthodontic treatment of patients with a history of temporomandibular disorders presents un... more The prosthodontic treatment of patients with a history of temporomandibular disorders presents unique risk-management problems because of the varied nature, uncertain prognosis, and frequent recurrence of these disorders. Patients with temporomandibular disorders frequently require managed treatment over extended periods of time and may complicate the delivery and outcome of prosthodontic treatment. Informed consent is needed to protect both the patient and the prosthodontist because it facilitates proper communication, goal setting, and cooperation. The purpose of this article is to discuss the essential elements of informed consent and to present an informed consent program useful in the prosthodontic treatment of patients with temporomandibular disorders.

This study measured the amount of positional change in the terminal transverse horizontal axis of... more This study measured the amount of positional change in the terminal transverse horizontal axis of the mandible that results from altering undesirable neuromuscular influence. Centric relation records were made on 19 healthy subjects before and after deprogramming. The axis was located with a computerized axiograph, and the centric relation records were compared in vivo at the axis location. The positional change of the axis was quantified in three dimensions at an accuracy of +/- 0.05 mm. The net amount of positional change was compared to scores from the temporomandibular joint scale to test for correlation with the potential for developing a temporomandibular disorder. The findings showed that the amount of positional change in the terminal transverse horizontal axis in 18 of 19 healthy patients after 12 hours of deprogramming was less than 0.5 mm and did not correlate with scores from the TMJ Scale.

When remaining occlusal contacts of a dental posterior quadrant have been prepared to receive com... more When remaining occlusal contacts of a dental posterior quadrant have been prepared to receive complete or partial coverage restorations, it is necessary to use an interocclusal recording material to approximate the casts for laboratory mounting procedures. Inaccuracy in working cast approximation leads to occlusal discrepancies in the fabricated restorations that require correction by the dentist at the time of insertion. Traditionally waxes, impression compounds, plasters, and zinc oxide-eugenol pastes have been used intraorally to record maxillomandibular relationships. Elastomeric materials have also been advocated. Manufacturers of thermoplastic resins have recently suggested that these materials may be used with ease and accuracy for interocclusal records. This investigation measured and compared the accuracies of a thermoplastic resin, an acrylic resin, and a vinyl polysiloxane interocclusal recording material. A measuring method was developed in which a computerized axiograph was used to record positional errors in three planes, next to the posterior teeth, with the interocclusal recording materials listed.

Other TMD screening forms and processes provide equally valid classification and prediction. Screening systems like the TMJ Scale have been validated by extensive epidemiological application and can be used if greater reliability is desired. Regardless of the screening process used, additional history review and clinical examination are indicated for all potential TMD patients before diagnosis. For therapy to be efficacious, the number and variety of disorders require various treatment modalities. A screening process can be incorporated easily into dental practice and is a safeguard against overlooking these dysfunctional problems while other oral health needs are met.
