Gereç ve Yöntem: Hastalar›n Brunnstrom evrelemesi, Barthel indeksi (BI) ve Ashworth skalas›na ait skorlar› kaydedildi. Short form-36 (SF-36) her hasta taraf›ndan tamamland›. ICF verileri, hastalarla yap›lan görüflmelerden ve t›bbi kay›tlardan elde edildi. ‹çsel geçerlilik, her ICF kategorisi için hastalar taraf›ndan rapor edilen problemlerin frekans ve yüzde hesaplamalar› ile de¤erlendirildi. Yap›sal geçerlili¤in de¤erlendirilmesi için ise, spearman korelasyon düzeyi hesapland›. After the initial treatment, breast cancer patients require a dynamic follow-up period to check for tumor recurrence, metastasis, as radiation therapy side effects as well as side effects of chemotherapy, and physiatric evaluations and rehabilitative interventions must be a part of this process. Functional problems which need rehabilitation and have unfavourable effect on quality of life, emerges after breast cancer therapy. For example, chemotherapy may induce neuropathy, impair concentration and memory, and cause fatigue, and when combined with radiotherapy after axillary lymph node dissection (ALND), it has been associated with chronic pain in the ipsilateral arm. Objectives: Pathophysiology and etiology of anterior interosseous nerve (AIN) syndrome are still ... more Objectives: Pathophysiology and etiology of anterior interosseous nerve (AIN) syndrome are still controversial. This anatomical dissection study aimed to understand the anatomy of AIN. Methods: From a random sample of upper extremities of whole-body human cadavers (n=10), 20 upper extremities were included in the study. Two of the cadavers were females and 8 were males (age range 34-62 years). Specimens were dissected with the elbow in extension, wrist in neutral position and forearm in pronation. After superficial dissection, the pronator teres muscle was released, and the branching pattern of the AIN and the separation of the nerve from the interepicondylar line were recorded. The branches to the pronator teres, flexor pollicis longus, flexor digitorum profundus and flexor digitorum superficialis were recorded according to their distance from the interepicondylar line. Results: The AIN branched from the main trunk 5.1 to 47.89 mm (mean 37.58±11.25 mm) distal to the interepicondylar line. AIN gave off 1-4 branches to the pronator teres. The first branch left the AIN 10.05-83.84 mm proximal and entered the muscle 23.49-43.72 mm distal to the interepicondylar line. AIN gave 1-4 branches to the flexor pollicis longus, flexor digitorum profundus and flexor digitorum superficialis at varying distances. The origin of the branches of AIN, as well as the innervation by one or multiple branches for a muscle, was variable. Conclusion: This study provides a detailed map of the anterior interosseous nerve innervating flexor pollicis longus, flexor digitorum profundus and flexor digitorum superficialis muscles, to serve as a guide for location of AIN block in patients with upper extremity spasticity and AIN syndrome.

Objetivo: Investigar a eficácia do kinesio taping e do taping placebo sobre a dor, limiar de dor ... more Objetivo: Investigar a eficácia do kinesio taping e do taping placebo sobre a dor, limiar de dor à pressão, amplitude de movimento cervical e incapacidade em pacientes com síndrome dolorosa miofascial (SDM) cervical. Métodos: Ensaio clínico randomizado duplo-cego controlado por placebo. Foram alocados em dois grupos, aleatoriamente, 61 pacientes com SDM. O grupo 1 (n = 31) foi tratado com kinesio taping e o grupo 2 (n = 30) foi tratado com taping placebo cinco vezes em intervalos de três dias, durante 15 dias. Além disso, todos os pacientes foram submetidos a um programa de exercícios para o pescoço. Os pacientes foram avaliados em relação à dor, ao limiar de dor à pressão, à amplitude de movimento cervical e à incapacidade. A dor foi avaliada com a escala visual analógica, o limiar de dor à pressão foi medido com um algômetro e a amplitude de movimento cervical ativa foi mensurada com a goniometria. A incapacidade foi avaliada com o Neck Pain Disability Scale. As mensuraç ões foram feitas antes e depois do tratamento. Resultados: No fim do tratamento, houve melhoria estatisticamente significativa na dor, no limiar de dor à pressão, na amplitude de movimento cervical e na incapacidade (p < 0,05) em ambos os grupos. Também houve uma diferença estatisticamente significativa entre os grupos em relação à dor, ao limiar de dor à pressão e à flexão-extensão cervical (p < 0,05); não houve diferença na rotação cervical, flexão lateral cervical e incapacidade (p > 0,05). Conclusão: O kinesio taping leva à melhoria na dor, no limiar de dor à pressão e na amplitude de movimento cervical, mas não na incapacidade em um curto período. Portanto, o kinesio taping pode ser usado como um método de terapia opcional para o tratamento de pacientes com SDM.

Objective: To determine the prevalance of lymphedema after breast cancer treatment with level 3 a... more Objective: To determine the prevalance of lymphedema after breast cancer treatment with level 3 axillary lymph node dissection (ALND) and to evaluate the risk factors which underlie this condition. Material and Methods: A total of 190 women >18-years-old who underwent breast cancer treatment with level 3 ALND >6 months ago were included in this cross-sectional study. The sociodemographic and clinical characteristics of all of the patients were recorded, and all patients were evaluated for lymphedema of the upper extremity by a circumferential measurement method. Results: On examination, 79 (41.5%) women had lymphedema with a mean development time of 12.7±26.62 months. After univariate analysis, the patients' age, body mass index (BMI), and number of metastatic lymph nodes (LNs) were found to increase the development of lymphedema. In addition, chemotherapy, breast or chest wall radiotherapy, and axillary radiotherapy also played a role. In the multivariate model, BMI (OR=5.491; 95% CI: 1.382-21.82), metastatic LNs (OR=0.314; 95% CI: 0.118-0.839), axillary radiotherapy (OR=15.34; 95% CI: 5.526-42.581), chemotherapy (OR=5.325; 95% CI: 1.48-19.153), and age (OR=1.044; 95% CI: 1.007-1.083) were significantly associated with an increased risk of lymphedema. Conclusion: This study demonstrated that there was a higher lymphedema prevalence ratio of 41.5% in breast cancer patients who underwent level 3 ALND and found that the risk factors for lymphedema development were axillary radiotherapy, chemotherapy, number of metastatic LNs, age, and BMI.

Background The discordance between the structural damage and the severity of the pain in knee ost... more Background The discordance between the structural damage and the severity of the pain in knee osteoarthritis (OA) suggests the effect of central and peripheral sensitization mechanisms on pain perception. This comes into the question of whether the characteristics of neuropathic pain (NP) exist in OA. Objectives To investigate the existency of NP features in osteoarthritis patients. Methods A total of 103 patients with knee pain lasting at least 3 months and diagnosed as knee OA based on the criteria of American College of Rheumatology were included in this study. The patients having any disease that may cause NP and having NP medication were excluded. The patients' knee radiographies were graded according to Kellgren Lawrence criteria. The patients were allocated in two groups; as grade I-II (Group 1) and grade III-IV (Group2). The frequency of NP were evaluated by The Leeds Assessment of Neuropathic Symptoms and Signs (LANSS) Pain Scale and Neuropathic Pain Diagnostic Questionnaire (DN4). Additionally the pain at rest and the activity were measured by Visual Analogue Scale (VAS), functional level was assessed by by Knee Outcome Survey-Activities for Daily Living Scale (KOS-ADSL) and quality of life was evaluated by Nottingham Health Profile (NHP). Results The mean ages of group 1 and group two were 59.74 and 67.30 respectively. The ratio of the results of LANNS and DN4 in group 1 was 10% and 79.2% in goup 2. There were statistically significant differences in group 2 in LANNS, DN-4, VAS, KOS-ADSL, NHP scores (p<0.001). A positive correlation between NP scores (LANNS, DN-4) and the grades of disease was determined (p<0.0001, r=0,80 ve 0,81 respectively). Also there was a positive correlation between NP (LANNS and DN-4) scores and KOS-ADSL (p<0.001, r=0,84 and 0.83). However LANNS and DN-4 were negatively correlated with NHP (p<0.001, r=-0.88 ve -0.87). Conclusions The coexistence of NP increases with the progression of osteoarthritis. NP seems to have negative effects on functional level and quality of life. While planning a treatment program for progressive OA, NP should be kept in mind program of. Background Central nervous system dysfunction in fibromiyalgia syndrome (FMS) may affect other se... more Background Central nervous system dysfunction in fibromiyalgia syndrome (FMS) may affect other sensorimotor functions of the patients, besides widespread pain. Balance disorder in FMS was stated in a few studies. Objectives To determine the effectiveness of the balance training exercise program in FMS and to find out the factors that associate with balance disorders in FMS. Methods A total of 68 patients, aged 18-65, diagnosed with FMS, joined in 6 weeks exercise program. The patients were randomly assigned into two groups. Group 1 (n=35) recieved flexibility exercises (15 min./day), balance exercises recommended by American Sport College (20 min./day) for 5 days a week and static (5min./day) and dynamic (5min./day) balance training with Kinesthetic Ability Trainer (KAT) 4000 device for 3 days a week (each one 5 min./day). Group 2 (n=33) received only flexibility program for 5 days a week. Patients were evaluated by means of balance, fall risk,functional level, quality of life and depression. Functional balance was measured by Berg Balance Scale (BBS), dynamic and static balance were assessed by Sport Kinesthetic Ability Trainer (KAT) 4000 device. Fall risk of the patients was specified by Hendrich II Fall Risk Model. Fibromiyalgia Impact Questionnaire (FIQ), Nottingham Health Profile were used to determine functional level and quality of life and Beck Depression Inventory was applied to evaluate depression level. The assesments were performed at baseline and after six week at end of the exercise program. Results Exercise program was accomplished by 57 patients and there were no significant differences between group 1 (n=28, mean age=48.11±13.42) and group 2 (n=29, mean age=48.17±12.68) according to sociodemographic characteristics. In balance exercise group statistical significant improvements were observed in all parameters (p<0.05), however there was no improvement in flexibility exercise group (p>0.05) at sixth weeks. In the comparison of two groups, there were significant differences in balance group for KAT 4000 static balance (p=0.017) and FIQ measurements (p=0.005). Additionally negative correlation was shown between depression levels and BBS (r= -0.434, p<0.01), where as depression level was also correlated with fall risk (r=0.357, p<0.01). Conclusions A 6-week balance training program has found to have beneficial effect on static balance and functional level in patients with FMS, however it has no effect on dynamic and functional balance, and quality of life. Although diabetic lumbosacral radiculoplexus neuropathy (LSRPN) is a well-known disease, clinical... more Although diabetic lumbosacral radiculoplexus neuropathy (LSRPN) is a well-known disease, clinical knowledge about non-diabetic idiopathic LSRPN is quite inadequate. LSRPN is rarely seen and does not require surgical treatment. If the accurate diagnosis is not considered, surgical treatment of radiculopathy or discopathy is inevitable, with sudden onset of low back and leg pain and loss of lower extremity muscle strength. Despite the common diagnosis of isolated femoral nerve neuropathy cases in LSRPN, rare cases of idiopathic sciatic nerve mononeuropathy were also observed. Especially, involvement of the isolated sciatic nerve or the lower trunk of the lumbosacral plexus may mostly interfere with the diagnosis of radiculopathy. In this article, a case of isolated idiopathic sciatic neuropathy is presented, and the etiology, diagnosis, and treatment modalities of LSRPN are discussed.

Thoracic outlet syndrome (TOS) is a heterogeneous group of disorders which occures with the compr... more Thoracic outlet syndrome (TOS) is a heterogeneous group of disorders which occures with the compression of one or more neurovascular elements at the cervicobrachial region. It is also a cause of neck and shoulder pain due to complex mechanisms. Diagnosing TOS can be difficult due to a great variety of symptoms may be seen in this disorder. These symptoms can simulate different diseases such as neck, shoulder and elbow pathologies and peripheric neuropathies or TOS may associate with other peripheric neurpathies. Thus once a peripheric neuropathy diagnosed, the possibility of an additional TOS should be investigated. Despite all these challenges, a clinical practice guideline for differential diagnosis of TOS is not determined yet. A careful history and thorough clinical examination are the most important components of the diagnosis. This article aims to discuss the clinical features and physical examination methods of the other related upper extremity and cervical pathologies in order to make an accurate differential diagnosis for TOS.

We conducted lupus clinic audit using NICE and Renal association guidelines as standards. Methods: We conducted a snapshot audit of last clinic episode of our Lupus clinic together with some additional retrospective data over 3 months. 40 patient's notes were reviewed and questionnaire filled. Results: Our cohort consisted of 38 females usually aged between 35-55 years. 25% of patients had disease duration of 3-4 years. Weight was checked only in 25% and BP in about 30% of whom majority were over 130/80. 75% of patient's creatinine was between 60-90 and eGFR over 60%. Ds DNA and C3 and C4 levels were checked in 80% and the split showed that 50% had high ds dna levels and 25% had low C3/C4 levels. ESR was high in 50% with low CRP levels. Urine dipstick was checked in only 25% although previous recordings showed more than 80% had urine dipstick checked at least once. Urine dipstick showed protein in 15 patients of whom only 8 patients had albumin creatinine ratio calculated. 5 new renal referrals were done out of 9 positive urine dipstick irrespective of creatinine levels. Conclusions: There was good monitoring of lupus activity by blood tests but was poor with basic observations such as weight, BP, urine dipstick. However our results show that limited urine dipstick testing can identify lupus patients with subclinical nephritis needing renal referral. This audit has led to improvement in training and education of the whole team to improve recording observations thus enhancing early detection of lupus nephritis.

To evaluate the clinical effectiveness of low-frequency pulsed electromagnetic field (PEMF) thera... more To evaluate the clinical effectiveness of low-frequency pulsed electromagnetic field (PEMF) therapy for women with fibromyalgia (FM). Fifty-six women with FM, aged 18 to 60 years, were randomly assigned to either PEMF or sham therapy. Both the PEMF group (n=28) and the sham group (n=28) participated in therapy, 30 minutes per session, twice a day for 3 weeks. Treatment outcomes were assessed by the fibromyalgia Impact questionnaire (FIQ), visual analog scale (VAS), patient global assessment of response to therapy, Beck Depression Inventory (BDI), and Short-Form 36 health survey (SF-36), after treatment (at 4 wk) and follow-up (at 12 wk). The PEMF group showed significant improvements in FIQ, VAS pain, BDI score, and SF-36 scale in all domains at the end of therapy. These improvements in FIQ, VAS pain, and SF-36 pain score during follow-up. The sham group also showed improvement were maintained on all outcome measures except total FIQ scores after treatment. At 12 weeks follow-up, only improvements in the BDI and SF-36 scores were present in the sham group. Low-frequency PEMF therapy might improve function, pain, fatigue, and global status in FM patients.

The literature includes a few cases of gabapentin-induced incontinence, and most of them involved patients with epilepsy who were between the ages of 12 and 43 years. Herein, we present three patients with neuropathic pain due to different diagnoses, and, to our knowledge, these are the oldest reported cases of urinary incontinence caused by gabapentin therapy. A 56-year-old female patient who underwent hip arthroplasty developed a sciatic nerve injury and neuropathic pain postoperatively. Ten days after she began taking gabapentin to relieve her pain, she experienced daily urinary incontinence. This anatomical dissection study aimed to understand the anatomy of AIN. Methods: From a random sample of upper extremities of whole-body human cadavers (n=10), 20 upper extremities were included in the study. Two of the cadavers were females and 8 were males (age range 34-62 years). Specimens were dissected with the elbow in extension, wrist in neutral position and forearm in pronation. After superficial dissection, the pronator teres muscle was released, and the branching pattern of the AIN and the separation of the nerve from the interepicondylar line were recorded. The branches to the pronator teres, flexor pollicis longus, flexor digitorum profundus and flexor digitorum superficialis were recorded according to their distance from the interepicondylar line. Results: The AIN branched from the main trunk 5.1 to 47.89 mm (mean 37.58±11.25 mm) distal to the interepicondylar line. AIN gave off 1-4 branches to the pronator teres. The first branch left the AIN 10.05-83.84 mm proximal and entered the muscle 23.49-43.72 mm distal to the interepicondylar line. AIN gave 1-4 branches to the flexor pollicis longus, flexor digitorum profundus and flexor digitorum superficialis at varying distances. The origin of the branches of AIN, as well as the innervation by one or multiple branches for a muscle, was variable. Conclusion: This study provides a detailed map of the anterior interosseous nerve innervating flexor pollicis longus, flexor digitorum profundus and flexor digitorum superficialis muscles, to serve as a guide for location of AIN block in patients with upper extremity spasticity and AIN syndrome.

Métodos: Ensaio clínico randomizado duplo-cego controlado por placebo. Foram alocados em dois grupos, aleatoriamente, 61 pacientes com SDM. O grupo 1 (n = 31) foi tratado com kinesio taping e o grupo 2 (n = 30) foi tratado com taping placebo cinco vezes em intervalos de três dias, durante 15 dias. Além disso, todos os pacientes foram submetidos a um programa de exercícios para o pescoço. Os pacientes foram avaliados em relação à dor, ao limiar de dor à pressão, à amplitude de movimento cervical e à incapacidade. A dor foi avaliada com a escala visual analógica, o limiar de dor à pressão foi medido com um algômetro e a amplitude de movimento cervical ativa foi mensurada com a goniometria. A incapacidade foi avaliada com o Neck Pain Disability Scale. As mensuraç ões foram feitas antes e depois do tratamento. Resultados: No fim do tratamento, houve melhoria estatisticamente significativa na dor, no limiar de dor à pressão, na amplitude de movimento cervical e na incapacidade (p < 0,05) em ambos os grupos. Também houve uma diferença estatisticamente significativa entre os grupos em relação à dor, ao limiar de dor à pressão e à flexão-extensão cervical (p < 0,05); não houve diferença na rotação cervical, flexão lateral cervical e incapacidade (p > 0,05). Conclusão: O kinesio taping leva à melhoria na dor, no limiar de dor à pressão e na amplitude de movimento cervical, mas não na incapacidade em um curto período. Portanto, o kinesio taping pode ser usado como um método de terapia opcional para o tratamento de pacientes com SDM.

Objective: To determine the prevalance of lymphedema after breast cancer treatment with level 3 a... more Objective: To determine the prevalance of lymphedema after breast cancer treatment with level 3 axillary lymph node dissection (ALND) and to evaluate the risk factors which underlie this condition. Material and Methods: A total of 190 women >18-years-old who underwent breast cancer treatment with level 3 ALND >6 months ago were included in this cross-sectional study. The sociodemographic and clinical characteristics of all of the patients were recorded, and all patients were evaluated for lymphedema of the upper extremity by a circumferential measurement method. Results: On examination, 79 (41.5%) women had lymphedema with a mean development time of 12.7±26.62 months. After univariate analysis, the patients' age, body mass index (BMI), and number of metastatic lymph nodes (LNs) were found to increase the development of lymphedema. In addition, chemotherapy, breast or chest wall radiotherapy, and axillary radiotherapy also played a role. In the multivariate model, BMI (OR=5.491; 95% CI: 1.382-21.82), metastatic LNs (OR=0.314; 95% CI: 0.118-0.839), axillary radiotherapy (OR=15.34; 95% CI: 5.526-42.581), chemotherapy (OR=5.325; 95% CI: 1.48-19.153), and age (OR=1.044; 95% CI: 1.007-1.083) were significantly associated with an increased risk of lymphedema. Conclusion: This study demonstrated that there was a higher lymphedema prevalence ratio of 41.5% in breast cancer patients who underwent level 3 ALND and found that the risk factors for lymphedema development were axillary radiotherapy, chemotherapy, number of metastatic LNs, age, and BMI.

Background The discordance between the structural damage and the severity of the pain in knee ost... more Background The discordance between the structural damage and the severity of the pain in knee osteoarthritis (OA) suggests the effect of central and peripheral sensitization mechanisms on pain perception. This comes into the question of whether the characteristics of neuropathic pain (NP) exist in OA. Objectives To investigate the existency of NP features in osteoarthritis patients. Methods A total of 103 patients with knee pain lasting at least 3 months and diagnosed as knee OA based on the criteria of American College of Rheumatology were included in this study. The patients having any disease that may cause NP and having NP medication were excluded. The patients' knee radiographies were graded according to Kellgren Lawrence criteria. The patients were allocated in two groups; as grade I-II (Group 1) and grade III-IV (Group2). The frequency of NP were evaluated by The Leeds Assessment of Neuropathic Symptoms and Signs (LANSS) Pain Scale and Neuropathic Pain Diagnostic Questionnaire (DN4). Additionally the pain at rest and the activity were measured by Visual Analogue Scale (VAS), functional level was assessed by by Knee Outcome Survey-Activities for Daily Living Scale (KOS-ADSL) and quality of life was evaluated by Nottingham Health Profile (NHP). Results The mean ages of group 1 and group two were 59.74 and 67.30 respectively. The ratio of the results of LANNS and DN4 in group 1 was 10% and 79.2% in goup 2. There were statistically significant differences in group 2 in LANNS, DN-4, VAS, KOS-ADSL, NHP scores (p<0.001). A positive correlation between NP scores (LANNS, DN-4) and the grades of disease was determined (p<0.0001, r=0,80 ve 0,81 respectively). Also there was a positive correlation between NP (LANNS and DN-4) scores and KOS-ADSL (p<0.001, r=0,84 and 0.83). However LANNS and DN-4 were negatively correlated with NHP (p<0.001, r=-0.88 ve -0.87). Conclusions The coexistence of NP increases with the progression of osteoarthritis. NP seems to have negative effects on functional level and quality of life. While planning a treatment program for progressive OA, NP should be kept in mind program of. Balance disorder in FMS was stated in a few studies. Objectives To determine the effectiveness of the balance training exercise program in FMS and to find out the factors that associate with balance disorders in FMS. Methods A total of 68 patients, aged 18-65, diagnosed with FMS, joined in 6 weeks exercise program. The patients were randomly assigned into two groups. Group 1 (n=35) recieved flexibility exercises (15 min./day), balance exercises recommended by American Sport College (20 min./day) for 5 days a week and static (5min./day) and dynamic (5min./day) balance training with Kinesthetic Ability Trainer (KAT) 4000 device for 3 days a week (each one 5 min./day). Group 2 (n=33) received only flexibility program for 5 days a week. Patients were evaluated by means of balance, fall risk,functional level, quality of life and depression. Functional balance was measured by Berg Balance Scale (BBS), dynamic and static balance were assessed by Sport Kinesthetic Ability Trainer (KAT) 4000 device. Fall risk of the patients was specified by Hendrich II Fall Risk Model. Fibromiyalgia Impact Questionnaire (FIQ), Nottingham Health Profile were used to determine functional level and quality of life and Beck Depression Inventory was applied to evaluate depression level. The assesments were performed at baseline and after six week at end of the exercise program. Results Exercise program was accomplished by 57 patients and there were no significant differences between group 1 (n=28, mean age=48.11±13.42) and group 2 (n=29, mean age=48.17±12.68) according to sociodemographic characteristics. In balance exercise group statistical significant improvements were observed in all parameters (p<0.05), however there was no improvement in flexibility exercise group (p>0.05) at sixth weeks. In the comparison of two groups, there were significant differences in balance group for KAT 4000 static balance (p=0.017) and FIQ measurements (p=0.005). Additionally negative correlation was shown between depression levels and BBS (r= -0.434, p<0.01), where as depression level was also correlated with fall risk (r=0.357, p<0.01). Conclusions A 6-week balance training program has found to have beneficial effect on static balance and functional level in patients with FMS, however it has no effect on dynamic and functional balance, and quality of life. LSRPN is rarely seen and does not require surgical treatment. If the accurate diagnosis is not considered, surgical treatment of radiculopathy or discopathy is inevitable, with sudden onset of low back and leg pain and loss of lower extremity muscle strength. Despite the common diagnosis of isolated femoral nerve neuropathy cases in LSRPN, rare cases of idiopathic sciatic nerve mononeuropathy were also observed. Especially, involvement of the isolated sciatic nerve or the lower trunk of the lumbosacral plexus may mostly interfere with the diagnosis of radiculopathy. In this article, a case of isolated idiopathic sciatic neuropathy is presented, and the etiology, diagnosis, and treatment modalities of LSRPN are discussed.

It is also a cause of neck and shoulder pain due to complex mechanisms. Diagnosing TOS can be difficult due to a great variety of symptoms may be seen in this disorder. These symptoms can simulate different diseases such as neck, shoulder and elbow pathologies and peripheric neuropathies or TOS may associate with other peripheric neurpathies. Thus once a peripheric neuropathy diagnosed, the possibility of an additional TOS should be investigated. Despite all these challenges, a clinical practice guideline for differential diagnosis of TOS is not determined yet. A careful history and thorough clinical examination are the most important components of the diagnosis. This article aims to discuss the clinical features and physical examination methods of the other related upper extremity and cervical pathologies in order to make an accurate differential diagnosis for TOS.

We conducted lupus clinic audit using NICE and Renal association guidelines as standards. Methods: We conducted a snapshot audit of last clinic episode of our Lupus clinic together with some additional retrospective data over 3 months. 40 patient's notes were reviewed and questionnaire filled. Results: Our cohort consisted of 38 females usually aged between 35-55 years. 25% of patients had disease duration of 3-4 years. Weight was checked only in 25% and BP in about 30% of whom majority were over 130/80. 75% of patient's creatinine was between 60-90 and eGFR over 60%. Ds DNA and C3 and C4 levels were checked in 80% and the split showed that 50% had high ds dna levels and 25% had low C3/C4 levels. ESR was high in 50% with low CRP levels. Urine dipstick was checked in only 25% although previous recordings showed more than 80% had urine dipstick checked at least once. Urine dipstick showed protein in 15 patients of whom only 8 patients had albumin creatinine ratio calculated. 5 new renal referrals were done out of 9 positive urine dipstick irrespective of creatinine levels. Conclusions: There was good monitoring of lupus activity by blood tests but was poor with basic observations such as weight, BP, urine dipstick. However our results show that limited urine dipstick testing can identify lupus patients with subclinical nephritis needing renal referral. This audit has led to improvement in training and education of the whole team to improve recording observations thus enhancing early detection of lupus nephritis.

Fifty-six women with FM, aged 18 to 60 years, were randomly assigned to either PEMF or sham therapy. Both the PEMF group (n=28) and the sham group (n=28) participated in therapy, 30 minutes per session, twice a day for 3 weeks. Treatment outcomes were assessed by the fibromyalgia Impact questionnaire (FIQ), visual analog scale (VAS), patient global assessment of response to therapy, Beck Depression Inventory (BDI), and Short-Form 36 health survey (SF-36), after treatment (at 4 wk) and follow-up (at 12 wk). The PEMF group showed significant improvements in FIQ, VAS pain, BDI score, and SF-36 scale in all domains at the end of therapy. These improvements in FIQ, VAS pain, and SF-36 pain score during follow-up. The sham group also showed improvement were maintained on all outcome measures except total FIQ scores after treatment. At 12 weeks follow-up, only improvements in the BDI and SF-36 scores were present in the sham group. Low-frequency PEMF therapy might improve function, pain, fatigue, and global status in FM patients.

Herein, we present three patients with neuropathic pain due to different diagnoses, and, to our knowledge, these are the oldest reported cases of urinary incontinence caused by gabapentin therapy. A 56-year-old female patient who underwent hip arthroplasty developed a sciatic nerve injury and neuropathic pain postoperatively. Ten days after she began taking gabapentin to relieve her pain, she experienced daily urinary incontinence.
