Critically ill patients are generally considered more vulnerable to harm from drug-related problems due to frequent medication-related events and complicated clinical courses. However, drug-related problems identified by on-ward clinical pharmacists in medical intensive care units in Thailand are not well reported. This study reports clinically relevant data with the description of identified problems, common causes of drug-related problems, and pharmacists’ interventions performed in real world, so that it may serve as an educational material for pharmacists who implement a pharmaceutical care and participate in medical intensive care units. The Ministry of Public Health in Thailand has encouraged the hospital in community to use the self-health records which were found to enhance healthy behaviors. Objective: To compare the knowledge and attitudes on fall prevention among elderly patients and their caregivers before and after the education program that employed using self-health records and receiving the counseling from nurses and pharmacists. Methods: This embedded mixed methods study was designed by using the structured questionnaires including demographic data and measuring the knowledge and attitude on fall prevention for quantitative arm (143 elderly patients and 42 caregivers). Objective: To determine the prevalence and profile of adverse drug reactions (ADRs) of voriconazo... more Objective: To determine the prevalence and profile of adverse drug reactions (ADRs) of voriconazole in Thai patients. Material and Method: A retrospective, descriptive study was conducted in in-patients who received voriconazole at Ramathibodi Hospital during the period of 1st July 2005 to 31st March 2008. Patients’ profiles were reviewed for adverse drug events (ADEs) of voriconazole. The causality assessment of ADRs was performed by a clinical pharmacist and a physician using Naranjo’s algorithm. ADR data were classified based on system-organ classification arranged by WHO Collaborating Centre for International Drug Monitoring. Prevalence, onset, duration and severity as well as outcome of ADRs were determined. Results: One hundred and twenty-eight patients’ profiles (62 males, 66 females) were assessed with 162 voriconazole use episodes. In total, 94 patients had ADRs from 114 voriconazole use episodes.
