In applying the tenets of system theory to ensure effective change, all the elements of the system must both be considered and consulted. In this way, the negative impact of the change on each piece of the puzzle is minimized while improvements are capitalized on to their fullest extent. In Epidemic of Care, authors George J. Isham, MD, and George Halvorson present a thought-provoking, comprehensive look at the concerns and contributions of various healthcare segments and how they affect the current expense crises in the American healthcare system. Halvorson is CEO and Chairman of Kaiser Permanente, the nation's largest nonprofit healthcare delivery system. The Partnering for Patient Empowerment through Community Awareness, or PPECA project, brought tog... more The Partnering for Patient Empowerment through Community Awareness, or PPECA project, brought together members of the community to develop a model to improve communication among key players regarding patient safety information--the consumer, the healthcare professional and the librarian--so that the best quality care is provided to patients; and to improve consumer awareness about patient safety and healthcare resources so that patients can better understand the importance of information and feel empowered to accept an active role in health care. We hope that the model described in this document will encourage and support the development of community-based patient safety partnerships between consumer groups, public libraries and healthcare institutions. Information dissemination, awareness building and consumer education on patient safety all begin at the community level. The model presented will illustrate the viability of the VA National Center for Patient Safety’s ... more The model presented will illustrate the viability of the VA National Center for Patient Safety’s Healthcare Failure Mode Effects Analysis (HFMEA) as a tool to identify likely failure points present in secondary information and knowledge identification activity currently installed in the clinical environment (1). This human factors viewpoint will inform the discussion of the process and help identify the potential pitfalls and repercussions of the failures inherent in current practice. Research from the field will also be utilized to augment further examples of possible process breakdowns to be considered (2). In addition, practitioner expertise will provide illustrative solutions for decreasing the likelihood of common and/or dangerous errors and gaps in the information delivery and knowledge transfer process.

In today's healthcare environment, a broad team of professionals should engage in the quest to improve patient safety through identifying and discussing the successes and failures that lead to organizational learning. Often it is stated that management engages the entire organization in this work, but rarely is the role of the librarian explicitly noted. It has been observed that multidisciplinary teams are wiser when they allow for diverse input (Surowiecki, 2004). Broadening the base of experts in the clinical environment to include librarians and information professionals can help to accelerate progress by improving access to what the organization knows and learning from the experience of others (ISMP, 2008; Conway, 2008).
