First described in the anesthesia literature in the late 1980s, medical simulation has become an ... more First described in the anesthesia literature in the late 1980s, medical simulation has become an essential part of training in medical education at the undergraduate and graduate level, as well as in nursing and pharmacy education. Traditionally, simulation occurs in dedicated simulation centers. This center-based simulation offers the benefit of being separate from patients and focusing on learning for the individual and the team. Dedicated time is scheduled to participate in this educational activity, and debriefing of the simulation can take part without competing demands of the clinical work environment. In situ simulation differs from a center-based simulation in that it occurs in the actual clinical environment, often using providers and staff who are currently on shift. Because it occurs in the real working environment, it provides opportunities for learning on the unit and organizational level as well. In situ simulation has been used to improve teamwork and communication, as well as helping to identify systems issues and latent patient safety threats. In situ simulations, such as mock codes, may ultimately improve patient outcomes.

Intravenous drug use (IVDU) can lead to numerous complications from skin abscesses to bacteremia ... more Intravenous drug use (IVDU) can lead to numerous complications from skin abscesses to bacteremia to endocarditis. Here, we present a rare case of acute suppurative thyroiditis as a complication of IVDU, in which a 26-year-old female with a past medical history of IVDU presented to the emergency department for evaluation of a large right sided neck mass. On exam, she had signs of sepsis and thyrotoxicosis. Fine needle aspiration confirmed suppurative thyroiditis. Blood cultures and culture from fine needle aspiration grew Staphylococcus aureus. Patient was treated with 2 weeks of intravenous antibiotics with good resolution of her symptoms.

Robotic assisted laparoscopic surgery is becoming more widely available, but despite its multiple... more Robotic assisted laparoscopic surgery is becoming more widely available, but despite its multiple benefits, it is not without risk. This case is of a 62-year-old female who presented to the emergency department for dyspnea two days after robotic assisted laparoscopic hysterectomy. Physical exam revealed diffuse facial, neck, upper extremity, torso, and lower extremity crepitus, which was diagnosed as diffuse subcutaneous air on computed tomography (CT). Imaging also revealed right apical pneumothorax and pneumomediastinum. The patient progressively improved over one month, with resolution of symptoms.

Pelvic pain is a common presentation to the emergency department (ED). For female patients, endom... more Pelvic pain is a common presentation to the emergency department (ED). For female patients, endometriosis can be difficult to diagnose and can have life-threatening complications if missed. In this case report, we present a case of a patient initially presenting to the ED with a few days of crampy lower abdominal pain. After initial imaging, she was found to have a large pelvic hematoma with concern for active extravasation and a large hemothorax. After further evaluation, she was suspected of having endometriosis leading to thoracic endometriosis and a catamenial pneumothorax. Although endometriosis is not typically an emergent diagnosis, the complications of significant endometrial tissue spread can cause lifethreatening impacts. Clinicians should consider complications of endometriosis in females of menstruating age.
