Dynamic X-ray imaging systems are used for interventional cardiac procedures to treat coronary he... more Dynamic X-ray imaging systems are used for interventional cardiac procedures to treat coronary heart disease. X-ray settings are controlled automatically by specially-designed X-ray dose control mechanisms whose role is to ensure an adequate level of image quality is maintained with an acceptable radiation dose to the patient. Current commonplace dose control designs quantify image quality by performing a simple technical measurement directly from the image. However, the utility of cardiac X-ray images is in their interpretation by a cardiologist during an interventional procedure, rather than in a technical measurement. With the long term goal of devising a clinically-relevant image quality metric for an intelligent dose control system, we aim to investigate the relationship of image noise with clinical professionals' perception of dynamic image sequences. Computer-generated noise was added, in incremental amounts, to angiograms of five different patients selected to represent the range of adult cardiac patient sizes. A two alternative forced choice staircase experiment was used to determine the amount of noise which can be added to a patient image sequences without changing image quality as perceived by clinical professionals. Twenty-five viewing sessions (five for each patient) were completed by thirteen observers. Results demonstrated scope to increase the noise of cardiac X-ray images by up to 21% ± 8% before it is noticeable by clinical professionals. This indicates a potential for 21% radiation dose reduction since X-ray image noise and radiation dose are directly related; this would be beneficial to both patients and personnel.

Two software tools were developed; one simulated x-ray production for a simple two dimensional radiographic system geometry comprising an x-ray source, beam filter, test object and detector. The other simulated the acquisition and display of two dimensional radiographic images of complex three dimensional objects using a ray casting algorithm through three dimensional mesh objects. Both tools were intended to be simple to use, produce results accurate enough to be useful for educational purposes, and have an acceptable simulation time on modest computer hardware. The radiographic factors and acquisition geometry could be altered in both tools via their graphical user interfaces. Dual-energy X-ray absorptiometry (DXA) body composition measurements are widely performed in both... more Dual-energy X-ray absorptiometry (DXA) body composition measurements are widely performed in both clinical and research settings, and enable the rapid and noninvasive estimation of total and regional fat and lean mass tissues. DXA upgrading can occur during longitudinal monitoring or study; therefore, cross calibration of old and new absorptiometers is required. We compared soft tissue estimations from the GE Prodigy (GE Healthcare, Madison, WI) with the more recent iDXA (GE Healthcare) and developed translational equations to enable Prodigy values to be converted to iDXA values. Eighty-three males and females aged 20.1-63.3 yr and with a body mass index range of 17.0-34.4 kg/m(2) were recruited for the study. The increasing use of dual energy X-ray absorptiometry (DXA) in children has led to the need for ... more The increasing use of dual energy X-ray absorptiometry (DXA) in children has led to the need for robust reference data for interpretation of scans in daily clinical practice. Such data need to be representative of the population being studied and be 'future-proofed' to software and hardware upgrades. The aim was to combine all available paediatric DXA reference data from seven UK centres to create reference curves adjusted for age, sex, ethnicity and body size to enable clinical application, using in-vivo cross calibration and making data back- and forward- compatible. Seven UK sites collected data on GE-Lunar or Hologic Scanners between 1996 and 2012. Males and females aged 4 to 20 years were recruited (n = 3598). The split by ethnic group was: White Caucasian 2887; South Asian 385; Black Afro-Caribbean 286; mixed heritage 40. Scans of the total body and lumbar spine (L1-L4) were obtained. The European Spine Phantom was used to cross-calibrate the 7 centres and 11 scanners....

The assessment of image quality in medical imaging often requires observers to rate images for so... more The assessment of image quality in medical imaging often requires observers to rate images for some metric or detectability task. These subjective results are used in optimisation, radiation dose reduction or system comparison studies and may be compared to objective measures from a computer vision algorithm performing the same task. One popular scoring approach is to use a Likert scale, then assign consecutive numbers to the categories. The mean of these response values is then taken and used for comparison with the objective or second subjective response. Agreement is often assessed using correlation coefficients. We highlight a number of weaknesses in this common approach, including inappropriate analyses of ordinal data, and the inability to properly account for correlations caused by repeated images or observers. We suggest alternative data collection and analysis techniques such as amendments to the scale and multilevel proportional odds models. We detail the suitability of ea...
