PROBLEM CONSIDERED Obstructive sleep apnea (OSA) is underdiagnosed during pregnancy, but there is... more PROBLEM CONSIDERED Obstructive sleep apnea (OSA) is underdiagnosed during pregnancy, but there is strong theoretical and some empiric evidence that treatment may improve obstetric outcomes. Barriers to screening, testing, and treatment are common during pregnancy. The goal of this described intervention was to reduce these barriers and improve detection of OSA in pregnancy. METHODS Representatives from sleep medicine and perinatology established a cross-disciplinary, collaborative Sleep Pregnancy Clinic offering a streamlined referral process for multimodal screening, testing, and treatment of OSA during pregnancy. This is a retrospective analysis of data from the clinic's first 19 months. RESULTS Between June 2017 and December 2018, 134 pregnant women were referred for OSA testing. Sleep apnea is emerging as an important and underdiagnosed comorbidity in pregnancy. Screening, d... more Sleep apnea is emerging as an important and underdiagnosed comorbidity in pregnancy. Screening, diagnosis, and initiation of therapy are all time-sensitive processes during the dynamic progression of gestation. Completion of referral and testing for sleep apnea during pregnancy requires a significant commitment of time and effort on the part of the pregnant patient. We evaluated for predictors of non-completion of sleep apnea testing within our obstetric-sleep referral pipeline, in an effort to inform and optimize future referrals. We performed a retrospective chart-review of 405 pregnant patient referrals for sleep apnea evaluation at the University of Wisconsin-Madison/UnityPoint sleep apnea pregnancy clinic. We used logistic regression analysis to determine predictors of lack of completion of sleep apnea testing. Based on data obtained in their cadaveric studies they considered that this minimally invasive interbody fusion could be safely implemented clinically. The authors describe their initial experience with a microendoscopic transforaminal lumbar interbody fusion (METLIF) technique, with regard to safety in the placement of percutaneous instrumentation, perioperative morbidity, and early postoperative results. Methods The METLIF procedure was performed unilaterally in 20 patients with single-level lumbar spondylolisthesis or pure mechanical back pain with endoscopic assistance, hemilaminectomy, unilateral facetectomy, and microdiscectomy. Recent evidence suggests that sepsis induces a significant state of immune suppression, as indicated by a reduction in Th1 cell responses and apoptosis of T, B, and dendritic cells (DCs) [2, 3]. Reversal of apoptosis and immune suppression in animal models correlates with improved outcomes from sepsis [4]. On the basis of these observations, positive immunomodulators may represent beneficial therapeutic agents for sepsis. One such immunomodulator is the Toll-like receptor (TLR) 9 ligand, an unmethylated dinucleotide CpG sequence motif (reviewed in [5]). The specific activity of the CpG ligand is determined by both the number of CpG motifs and the flanking nucleotide sequences [6]. Prader-Willi syndrome (PWS) is a rare neurodevelopmental genetic disorder associated with a chara... more Prader-Willi syndrome (PWS) is a rare neurodevelopmental genetic disorder associated with a characteristic behavioral phenotype that includes severe hyperphagia and a variety of other behavioral challenges such as temper outbursts and anxiety. These behaviors have a significant and dramatic impact on the daily functioning and quality of life for the person with PWS and their families. To date, effective therapies addressing these behavioral challenges have proven elusive, but several potential treatments are on the horizon. However, a limiting factor for treatment studies in PWS is the lack of consensus in the field regarding how to best define and measure the complex and interrelated behavioral features of this syndrome. The International PWS Clinical Trials Consortium (PWS-CTC, www.pwsctc.org) includes expert PWS scientists, clinicians, and patient advocacy organization representatives focused on facilitating clinical trials in this rare disease. Red tree corals (Primnoa pacifica) are abundant in the eastern Gulf of Alaska, from the glacial f... more Red tree corals (Primnoa pacifica) are abundant in the eastern Gulf of Alaska, from the glacial fjords of Southeast Alaska where they emerge to as shallow as 6 m, to the continental shelf edge and seamounts where they are more commonly found at depths greater than 150-500 m. This keystone species forms large thickets, creating habitat for many associated species, including economically valuable fishes and crabs, and so are important benthic suspension feeders in this region. Though the reproductive periodicity of this species was reported in 2014 from a shallow fjord (Tracy Arm), this study examined reproductive ecologies from 8 sites-two within Glacier Bay National Park and Preserve, three on the continental shelf edge, one within Endicott Arm (Holkham Bay) and two time points from the Tracy Arm (Holkham Bay) study. Male reproductive traits were similar at all sites but there were distinct differences in oogenesis. Though per polyp fecundity mostly showed no significant difference between sites, there was a non-significant trend of increasing number of oocytes with depth. In addition, the average oocyte size from Tracy Arm (the shallowest site) was 105 µm, whereas from Shutter Ridge (one of the deepest sites) the average size was 309 µm. Moreover, the maximum oocyte size at Endicott Arm was 221 µm and at Tracy Arm was 802 µm (both shallow sites), whereas at Dixon Entrance (a deep site) it was 2120 µm, a difference not usually observed within a single species. We propose two theories to explain the observed differences, (a) this species shows great phenotypic plasticity in reproductive ecology, adjusting to different environmental variables based on energetic need and potentially demonstrating micro-evolution; or (b) the fjord sites are at a reproductive dead end, with the stress of shallow-water conditions effectively preventing gametogenesis reaching full potential and likely limiting successful reproductive events from occurring, at least on a regular basis.

T... more Recent findings suggest increased use of fringe habitats by the endangered West Indian manatee. To begin collecting fundamental data on population dynamics and ecology of fringe manatees in the northern Gulf of Mexico, we established the Mobile Manatees Sighting Network (MMSN), the first formal network to receive and track manatee sightings in Alabama waters. Comparison of MMSN data with compiled historical data indicated that West Indian manatees are regular seasonal visitors to Alabama waters. Historical (1985-2006) and newly collected data shared consistent distributions, seasonal patterns of abundance in sighting number, and group size frequencies. These data indicate that MMSN was as effective at capturing data as two decades of historical sighting reports and suggest consistent long-term patterns in habitat preference and seasonal movements of manatees in Alabama waters. A nearly fourfold increase in number of manatee sightings, with inception of the MMSN, made evident the success of community outreach efforts but also betrayed the potential influence of observers on data quality. The MMSN maintains a 24-hr toll-free phone line, website with interactive online sighting form, and E-mail address to receive citizen sighting reports and provide supporting educational materials. Our data demonstrate that this type of monitoring for manatees in fringe habitat is feasible, effective, and essential to guide local management and recovery efforts of this endangered species.
