Osteoradionecrosis is a severe and devastating late complication of radiotherapy in patients with... more Osteoradionecrosis is a severe and devastating late complication of radiotherapy in patients with head and neck cancer. The diagnosis of Osteoradionecrosis is established by clinical and radiographic evidence of bone necrosis after irradiation. The current article reports a case of osteoradionecrosis in an irradiated patient (male; 62-year-old), which evolved into pathological fracture of the jaw, 2 years after the end of radiotherapy for oral cancer treatment. The patient was rehabilitated in other service, with reconstruction plates, without adjuvant hyperbaric oxygen therapy, progressing to a fistula formation and bone exposure. The patient was referred to our service, with an extensive bone and plate exposure, with signs of severe osteoradionecrosis. After the adequacy of the oral environment, the patient underwent surgery for necrotic bone and reconstruction plate resection, with a primary closure of the skin and intraoral mucosa. The post-operative clinical and radiographic examinations performed after 12 month showed no signal of recurrence. Pathologic fracture in conjunction with osteoradionecrosis has a relatively high treatment complication rate. The incorrect diagnosis of mandibular fracture associated with osteoradionecrosis leads to inadequate treatment, with irreversible consequences to the patient, as described in the related case.

The rehabilitation of speech and swallowing are critical for patients submitted to tumor resectio... more The rehabilitation of speech and swallowing are critical for patients submitted to tumor resection on the tongue and, in cases of extensive lesions, with even greater impairment, serious damage to the quality of life of individuals are expected. When excision of the tumor followed by a primary closure is the defined treatment, the patient may experience difficulty in phonation by reducing the tongue mobility, which may be associated with dysphagia and even impossibility of adapting conventional complete dentures. Thus, the aim of this paper is to present a surgical technique for cancer release in cancer patients, specifically those patients with tongue and floor of the mouth tumors on I and II who underwent surgical resection. The technique is easy and quick, possible to be performed under local anesthesia and with low complications.

Existem três variantes clínicas, denominadas folicular, extrafolicular e periférica. Relatamos um caso de paciente do gênero feminino, com 35 anos, com TOA variante extrafolicular, localizado em maxila. Exames diagnósticos de imagem revelaram tumor localmente agressivo, com destruição óssea, sem invasão de tecidos adjacentes. A paciente foi tratada com enucleação cirúrgica conservadora. Encontra-se sem evidência de doença, após seguimento de cinco meses. Descritores: Tumores Odontogênicos. Neoplasias Maxilares. Tumor Adenomatóide. Maxila. The adenomatoid odontogenic tumor (AOT) is a benign, noninvasive and low growth tumor. There are three clinical variants: follicular, extrafollicular and periferical. We report a case of a woman, 35 years-old with extrafollicular AOT, located in maxilla. Diagnostic image exams revealed locally aggressive tumor with bone destruction, without adjacent tissues invasion. The patient was treated with surgical enucleation. Nowadays, she has been with no evidence of disease after five-month follow-up.

Introduction Medication-related osteonecrosis of the jaws (MRONJ) is a complication that develops... more Introduction Medication-related osteonecrosis of the jaws (MRONJ) is a complication that develops in patients who use or have used antiresorptive or antiangiogenic medications for the treatment of bone metabolic disease and bone metastases. Clinically, MRONJ is characterized by the appearance of an inflammation in soft tissues and exposure of necrotic bone tissue in mandible or maxilla, for a period of 8 weeks, in patients with no history of head and neck radiotherapy that were being or are being treated with antiresorptive and/or antiangiogenic agents. The fibrin-rich platelets and leukocytes (L-PRF) membrane has been used as an alternative for MRONJ prevention. The aim of this study was to evaluate the use of L-PRF in prevention and treatment of bone necrosis. Material and Methods The patients included had MRONJ diagnosis confirmed after clinical and radiographic examination and patients whose only therapeutic option was dental extraction. Results Twenty patients were included in the study and were divided in three groups. Two patients were removed from the study due to previous history of pentoxifylline and tocopherol use. The result of surgical treatment was successful in 57% in group 1 (control/MRONJ prevention), 100% in group 2 (MRONJ prevention), and 80% in group 3 (MRONJ treatment). Conclusion L-PRF is an autologous biomaterial that allows the release of growth factors for a prolonged time, resulting in a better healing, reducing the risk contamination, edema, and postoperative pain, being a great ally in the prevention and treatment of MRONJ because it returns to these patients, mainly quality of life, reducing pain, and recurrent infections commonly seen in the processes of bone necrosis of the jaws.

Bisphosphonates (BPs), antiresorptive and antiangiogenic drugs are used to prevent metastatic bon... more Bisphosphonates (BPs), antiresorptive and antiangiogenic drugs are used to prevent metastatic bone cancers in prostate cancer, breast cancer and multiple myeloma and to treat osteoporosis and Paget's disease. Recently, in 2003 the first case of osteonecrosis of the jaws was induced, hitherto by bisphosphonates, but a few years later it was shown that other medications were also responsible for the development of this type of necrosis. Thus, in 2014 there was a change in the name for medication-related osteonecrosis of the jaws (MRONJ). Since then, the treatment for this type of necrosis is quite controversial in the world literature, and there is still no protocol for established treatment, be it clinical or surgical. The objective of this work is to demonstrate the efficacy of platelet and leukocyterich fibrin membranes (L-PRF) after curettage of necrotic bone tissue in the management of drug-related jaw osteonecrosis, since they have innumerable biological benefits such as large amount of growth factors and cytokines, hemostatic capacity, angiogenesis capacity, and has been shown to accelerate and improve results in hard and soft tissue wound healing. The patient presented MRONJ and have been treated with surgical necrotic bone debridement, placement of L-PRF in the affected site and primary closure. Patient were followed up clinically and radiographically until total mucosal coverage of the necrotic bone was achieved.

Introduction: Langerhans Cell Histiocytosis (LCH) is a disorder characterized by abnormal prolife... more Introduction: Langerhans Cell Histiocytosis (LCH) is a disorder characterized by abnormal proliferation of CD1a+/S-100/CD207+ Langerhans cells. It has a peak of incidence between one and threeyear-old, with a prevalence in males than females. It may affect several anatomical locations. Objective: to report two cases of oral manifestations of HCL, which exemplify the broad age range affected by the disease and its different clinical presentations. Material and methods: This is a retrospective, descriptive, observational study in the format of a technical note, represented by two clinical cases that demonstrates the different Oral Manifestations of Langerhans Cell's Histiocytosis. Case report: a 47-year-old man presenting ulcerated lesions in hard palate and gingival region and a three-year-old boy with a diffuse swelling at the posterior mandible were diagnosed with HCL at the same institution. Conclusion: the HCL can manifest itself as a single lesion or as a multisystemic and potentially fatal disease. The clinician should be attentive to diagnose it at an early stage, to avoid worse complications and sequelae to the patient.

There has been no evidence of recurrance two years after the first operation. This treatment technique provides the optimal cosmetic and functional rehabilitation for patients who have sustained disabling deformity. We examined the pathological study of malignant ameloblastoma with occlusal reconstruction using osseointegrated implants and autogenous iliac bone graft.

Lesions that affect the oral cavity resulting from chemotherapy can lead to systemic impairment, ... more Lesions that affect the oral cavity resulting from chemotherapy can lead to systemic impairment, increasing the length of hospital stay, impairing the patient's quality of life. Through a integrative review, this work aims to show the conduct and importance of dentists in promoting oral health and preventing an oral infectious focus. The search was performed in the Pubmed, Cochrone, SciElo and Virtual Health Library (VHL) databases, using the descriptors in English, oral care and Chemotherapy and Cancer treatment and dentistry, filtering only articles in English and published between 2006 and 2021. The search located six hundred and twenty articles, but only seven met the search criteria. One author used a mouthwash containing propolis where 65% of patients were cured on the seventh day after the onset of oral mucositis (OM) lesions. One author used cryotherapy with chamomile infusion, patients who underwent this approach did not pass grade I, being effective in reducing the occurrence of OM, and the other two authors proposed a protocol for oral cavity care. All authors obtained satisfactory results.

Introduction: The central giant cell lesion (LCCG) consists of a non-neoplastic proliferative pro... more Introduction: The central giant cell lesion (LCCG) consists of a non-neoplastic proliferative process but may exhibit aggressive behavior, similar to a neoplasm. It is observed more frequently in young adults and children, with a predilection for females, and its prevalence is higher in the jaw, it is usually asymptomatic and their clinical behavior can be aggressive or not. Such lesions present obscure etiology and different clinical presentations. Case report: A female patient, 26 years old, presented to the surgical service and Maxillofacial the University Hospital João de Barros Barreto, an increase of volume in the left mandibular body. The extraoral examination there were no changes. On intraoral examination it was found left side of the swelling of the jaw, fixed and hard consistency with an increase of about one year. the tests were ordered, panoramic radiography of the jaws, where we observed a defect radiolucent unilocular well-defined, with jagged edges and without cortical bone disruption and incisional biopsy that confirmed the hypothesis diagnostic LCCG. According to the histopathological finding noted the presence of multinucleated giant cells with uniformly distributed eosinophilic staining and predominantly cell stroma. Discussion: This was surgery to remove the lesion under general anesthesia, with a safety margin of 1 mm, peripheral osteotomy and fixation with reinforcement 2.4 titanium plate. Conclusion: The treatment approach performed in this case Brazilian Journal of health Review Braz. J. Hea. Rev., Curitiba, v. 2, n. 6, p. 5382-5395 nov./dec. 2019. ISSN 2595-6825 5384 proved to be effective, given the good adaptation of the plate, and which patient follows up, no evidence of relapse.

The aim of the present study was to characterize mandibular fracture patients and their treatment, as performed in the Department of Maxillofacial Surgery, Faculty of Dentistry, Plovdiv. The available information was obtained retrospectively from the records of 612 patients with 911 fractures of the mandible, treated for a period of five years-from 1998 to 2002. 81.2% of the patients were male, and 18.8%-female (Sp = 1.58%). Mean age was 33.5 years (Sx = 15.219).
