Multicenter studies are widely used to meet accrual targets in clinical trials. Clinical data mon... more Multicenter studies are widely used to meet accrual targets in clinical trials. Clinical data monitoring is required to ensure the quality and validity of the data gathered across centers. One approach to this end is central statistical monitoring, which aims at detecting atypical patterns in the data by means of statistical methods. In this context, we consider the simple case of a continuous variable, and we propose a detection procedure based on a linear mixed-effects model to detect location differences between each center and all other centers. We describe the performance of the procedure as a function of contamination rate and signal-to-noise ratio. We investigate the effect of center size and variance structure and illustrate the use of the procedure using data from two multicenter clinical trials.

CDE (cyclophosphamide, doxorubicin, etoposide) is one of the standard chemotherapy regimens in th... more CDE (cyclophosphamide, doxorubicin, etoposide) is one of the standard chemotherapy regimens in the treatment of small-cell lung cancer (SCLC), with myelosuppression as dose-limiting toxicity. In this trial the impact of prophylactic antibiotics on incidence of febrile leucopenia (FL) during chemotherapy for SCLC was evaluated. Patients with chemo-naïve SCLC were randomized to standard-dose CDE (C 1,000 mg/m2 day 1, D 45 mg/m2 day 1, E 100 mg/m2 days 1-3. i.v., q 3 weeks, x5) or to intensified CDE chemotherapy (125% dose, q 2 weeks, x4, with filgrastim 5 microg/kg/day days 4-13) to assess the impact on survival (n = 240 patients). Patients were also randomized to prophylactic antibiotics (ciprofloxacin 750 mg plus roxithromycin 150 mg, bid. days 4-13) or to placebo in a 2 x 2 factorial design (first 163 patients). The aim of this study was to evaluate the prevalence and noise exposures associated with the severity of hearing loss (HL) in a Belgian military population. A cross-sectional study was carried out at the Centre for Medical Expertise (CME) and in four Units of Occupational Medicine (UOM). Hearing thresholds were determined by audiometry. The examination included a questionnaire on hearing-related medical history, and noise exposure in military and leisure time activity. A multinomial logistic regression model was used to assess the association of the severity of HL with tinnitus, with the military occupation, and with noise exposures. Of the 2055 subjects aged 18-55 years, 661 (32.2%) had a slight HL (25-40 dB), 280 (13.6%) had a moderate HL (45-60 dB) and 206 (10.0%) had a severe HL (>60 dB) of 4 and 6 kHz for both ears. The prevalence of slight, moderate and severe HL increased significantly with age and was higher for subjects from Paracommando and infantry units. Fighting in Built-Up Area (FIBUA) training, shooting with large caliber weapons, and participation in military exercises were the best determinants of HL in this population. These results suggest that subjects from infantry and Paracommando units run the highest risk of HL because they are exposed to very loud noises in their professional life, like large caliber shooting and FIBUA training.
