I often hear from colleagues who are struggling to decide whether to go back to school for a doct... more I often hear from colleagues who are struggling to decide whether to go back to school for a doctor of nursing practice (DNP) or a doctor of philosophy (PhD) degree. Before the DNP came into being, not all that long ago, the choices were more or less limited to the PhD and doctorate of nursing science, with a couple of other less common options. Since the advent of the DNP, many nurses have opted for that choice, not fully understanding why they were getting it or how they might use it to further their careers. Not insignificant in the consideration was the decision by the American Association of Colleges of Nursing (AACN) that "call[ed] for educating advanced practice registered nurses (APRNs) and other nurses seeking top clinical positions in Doctor of Nursing Practice (DNP) programs." 1 I remember thinking that this was unlikely to happen for 2 main reasons: employers, especially physician employers, would not know the difference between a DNP-prepared nurse practitioner (NP) and one without a DNP, and there was unlikely to be a salary differential, so what was the incentive? Since then, the number of nurses, not just NPs, working toward and earning the DNP degree has grown tremendously. While employers and patients probably still don't know what a DNP degree entails or how it translates to patient care, some employers do give a pay differential if you have a DNP. Meanwhile, the PhD is languishing, so much so that a lot of discussion focused on how to get more students into PhD programs at a recent AACN deans' meeting in Washington, DC. We all know NPs who enrolled in NP programs with little knowledge regarding what an NP actually does. I can't count how many times a prospective NP student has told me the role of the NP is to write prescriptions and give orders! I always encourage prospective students to shadow an NP and learn as much as they can before investing time, energy, and money in a career they might not enjoy. The same applies to choosing a doctoral program. PhD programs are research focused. They prepare the nurse to conduct and critique research. Research, herein defined, is qualitative or quantitative; it might include randomized control trials or be quasi-experimental. This type of research is quite different from the scholarship in which DNP-prepared nurses typically engage. It is a myth that PhD programs prepare you better than do DNP programs to teach. For a long time, it was assumed that, if you had a PhD, you could teach. This applies to other disciplines as well as nursing. However, more schools of nursing are offering doctoral or postdoctoral courses in education in recognition that teaching is a specialty in itself and to teach well, one must be trained appropriately. PhD-prepared researchers are expected to conduct and lead research studies. They are expected to disseminate their work via peer-reviewed publications and presentations. Most schools of nursing also expect them to obtain grant funding. This funding supports a portion of the researcher's salary. PhD-prepared nurses also work in health care agencies and hospitals, but the expectations are largely the same as in academic settings. PhD-prepared nurses

This is a change from the past, when nurses working in home healthcare were believed to be the most highly satisfied group of nurses (Simmons et al., 2001). The source of nurses' dissatisfaction has been attributed to the changing work environment in home care (Anthony & Milone-Nuzzo, 2005; Ellenbecker & Byleckie, 2005b) brought about by recent federal and state regulations. Requirements for the Outcome Assessment and Information Set (OASIS) and a capitated reimbursement system with incentives to limit the amount of contact nurses have with patients have increased nurses' work demands (Anthony & Milone-Nuzzo; Smith-Stoner, 2004b). Academic and clinical settings must carefully consider from whom to raise the money and for what it will be used. In-depth analysis and multiple perspectives are necessary to reach the best outcome. This chapter describes a case in which a school of nursing must make an important decision that will require fundraising. There are several possibilities and the decision will have long-term consequences. Red Teaming methods assisted in the final resolution.

It is critical to understand that one is not likely to use Red Team tools as smoothly as one woul... more It is critical to understand that one is not likely to use Red Team tools as smoothly as one would like initially. Learning Red Teaming is like studying a new language regarding how to plan and make better decisions. Like any new language, it is awkward and halting at first. Similarly, the more one uses the language and fights through the early missteps, the more fluent one will become. People describe fluency in a new language as the ability to tell jokes in that language because jokes require a deep understanding of culture that is greater than simply the mechanics of communicating information. In Red Teaming, achieving that fluency of thought is evident when one can begin creating one’s own Red Teaming tools that reflect the underlying principles of Red Teaming but are specifically tailored to the needs of the unique problem.
