Ça­lış­ma­ pla­nı:­ Mart 2004-Haziran 2006 tarihleri arasında PE nedeniyle modifiye Ravitch tekniği uygulanan 24 hasta (18 erkek, 6 kadın; ort. yaş 11.4; dağılım 5-24 yıl) ve Haziran 2006-Kasım 2008 tarihleri arasında Nuss tekniği uygulanan 26 hasta (17 erkek, 9 kadın; ort. yaş 13.3; dağılım 2.5-36 yıl) geriye yönelik olarak değerlendirildi. Ortalama ameliyat süresi Nuss grubunda 40 dakika (28-56 dakika), Ravitch grubunda ise 165 dakika (125-240 dakika) idi. Bul gu lar: Ameliyat sonrası hasta memnuniyeti değerlendirildi. Nuss tekniği uygulanan 24 hastada (%92.3) sonuç mükemmel ve iki hastada (%7.7) yeterliydi. Nuss grubunda iki hastada (%7.7) pnömotoraks görüldü. Lipomas, one of the most common benign soft tissue lesions, may mimic malignant masses with their clinical and radiological manifestations. We aimed to present a case of giant lipoma with the complaint of severe pain with the review of the literature.

Acil torakotomi yapıldı ve sağ hemitorakstan dört Kirschner teli dikkatlice çıkarıldı. Hasta ameliyat sonrası altıncı günde sorunsuz olarak taburcu edildi. Ortopedik cerrahlar evde yeterli bakımı yapılamayan hastalarda ucu dışarıda kalan bu tür malzemelerin kullanılmasından kaçınmalıdır. Hastalar sosyal çevreleri ile birlikte bir bütün olarak değerlendirilerek sadece hastalık için değil hasta için de en uygun tedavi yöntemi seçilmelidir. In the present report, we aimed to highlight ecstasy as a rare cause of spontaneous pneumomediastinum and its potential lifethreatening effects. A 16-year-old female with dyspnea and chest pain presented to the emergency department. Chest computed tomography demonstrated pneumomediastinum at the level of the thoracic inlet and upper and posterior mediastinum. The patient was admitted to the thoracic surgery department with a preliminary diagnosis of spontaneous pneumomediastinum. She refused to answer any questions during the first visit, an a psychiatric consultation was requested. The most important finding of psychiatric consultation was ecstasy abuse, which could not be identified in the emergency department evaluation. Four days later, the symptoms resolved completely and control chest X-rays showed no complications; therefore, the patient was discharged. While investigating the etiology of spontaneous mediastinum, particularly in a young, healthy patient, ecstasy abuse should always be considered. Because the fatal complications that may develop due to ecstasy ingestion may be overlooked.

Compared with children, foreign body aspiration is rare in adults and sometimes could be fatal. The most common symptom is severe coughing that appears after choking. Foreign body aspiration should be considered in the diagnosis of patients with wheezing, chronic cough, voice alteration, hoarseness, or recurrent pulmonary infections. A 78-year-old male patient with tracheostomy due to laryngeal cancer was admitted to the emergency department on account of aspiration of tweezers after coughing when he was cleaning the tracheostomy cannula. The patient complained of shortness of breath and coughing. Physical examination revealed decreased breath sounds on the right side of the lung, and he had dyspnea and central cyanosis. There was a foreign body extending from the carina to the right main bronchus on the posteroanterior radiograph of the lung. In the operating room, rigid bronchoscopy was performed from the tracheostomy stoma and the foreign body was removed using forceps. This case shows that patients with tracheostomy and their relatives must be well educated in the care of stoma. Rigid bronchoscopy by an experienced specialist for foreign body aspiration is a safe and effective method for diagnosis and treatment.

However, in recent years, the aspiration of pins which are used for securing turbans (headscarves) is seen frequently in young women. The aim of this article was to review 105 patients who were admitted to our hospital for turban pin aspiration. Chest X-rays were used for diagnosis. Various methods of treatment were performed: laryngoscopy in 6 patients, flexible fiberoptic bronchoscopy in 16, rigid bronchoscopy in 93, and thoracotomy in one patient, while in the other patient the turban pin was spontaneously expectorated. Localization of the pin in the right bronchial system was common (52%). In 6 patients, turban pins located in the larynx were extracted successfully by direct laryngoscopy. Turban pins were successfully removed with a flexible fiberoptic bronchoscope in 4 patients of the 16 (25%) and by rigid bronchoscope in 93 patients of 94 (99%). The average time until discharge was 18 hours and there was no mortality. Turban pin aspiration is common in Islamic populations and treatment usually requires bronchoscopic procedures. In order to minimize turban pin aspiration frequency, we recommend that turbans should be secured by traditional fastening methods or with an apparatus which cannot be aspirated.

Surgical removal is currently the generally accepted choice of treatment for lung hydatidosis. However, operating on bilateral widespread lung hydatidosis is still controversial. The aim of this retrospective study was to evaluate the results of surgical treatment in bilateral multiple hydatid disease of the lung. Methods: In this study, we reviewed our experience in the surgical treatment of 17 (3.7%) patients with bilateral, and at least three, lung hydatid cysts. These 17 patients (8 male, 9 female), with an average age of 34.6 years (range 12-58 years), underwent bilateral staged thoracotomy. Results: In total 105 lung cysts were removed from 17 patients who underwent staged thoracotomies. The mean count of cysts was 6.7 (range 3-20 cysts). Most of the cysts (38.2%) were located in the right lower lobe. The mean interval between thoracotomies was 4.2 (range 3-5) days. Two patients (11.7%) had cysts associated with hepatic hydatidosis and one (5.8%) had cysts associated with the spleen; they were treated via phrenotomy during thoracotomies. All cysts were removed without lung resection. We observed some complications such as prolonged air leaks (n = 2), atelectasis (n = 3) and empyema (n = 2). No further surgery was required for management of complications. The mean hospital stay was 9.3 days. (range 7-23 days). Oral albendazole was started on the 2nd post operative day after the first thoracotomy in the dose of 10-20 mg/kg and was continued for 3 months with a gap of 1 week after each 21 days. No recurrences or deaths occured during the follow-up period. Conclusions: Although staged thoracotomy applied in 3-5 days after the initial thoracotomy increases the total hospital stay, it decreases the chance of possible complications can occur in cysts in the other lung when long intervals are preferred between the first and the second thoracotomy. In our experience, bilateral staged thoracotomy is an appropriate surgical option because morbidity rates are minimal and the hospital stay is acceptable for the treatment of bilateral widespread lung hydatidosis, even in patients who had a total of 20 hydatid cysts.

We describe the characteristics of 2 patients with adenoid cystic carcinoma who presented with symptoms suggestive of bronchial asthma of 2 years duration. Biopsy specimens revealed that the lesions were adenoid cystic carcinoma of the trachea. Both patients underwent collar incision and median sternotomy, the tumors were resected, and the tracheas were reconstructed primarily. Postoperative radiotherapy was given. Their postoperative clinical courses were uneventful and no local recurrences were seen at the 1-year follow-up.

As pleurodesis causes systemic inflammation and is associated with considerable cost and morbidit... more As pleurodesis causes systemic inflammation and is associated with considerable cost and morbidity during long-term follow up, the identification of patients who will experience an unsuccessful pleurodesis would be desirable. This study was aimed to investigate whether systemic inflammatory reaction induced by insuflation of talc into the pleura can predict the outcome of pleurodesis. A total of 58 consecutive patients (26 men, 32 women) with malignant pleural mesothelioma underwent video-assisted thoracoscopy under general anaesthesia with monopulmonary ventilation between the years 2003 and 2006. Four grammes of asbestos-free and sterile talc were insuflated into the pleural space under direct vision. To assess the success of pleurodesis, chest radiographs were obtained at the 8th and 30th postoperative days. Venous blood samples were drawn both on admission and at the 24th hour after pleurodesis for the analysis of white blood cells, erythrocyte sedimentation rate and C-reactive protein. The mean age (standard deviation) of patients was 59.0 +/- 12.0 years. Pleurodesis was achieved (no effusion on chest radiograph) in 43 of 58 patients (74.1%)(group I), whereas it was unsuccessful in the remaining 15 patients (25.9%)(group II). There was a significant difference between two groups for basal and postpleurodesis levels of measured inflammatory parameters, C-reactive protein and erythrocyte sedimentation rate (for each, P < 0.05). However, the difference was not significant for white blood cells between the groups. Serum levels of inflammatory parameters (C-reactive protein and erythrocyte sedimentation rate) may be used to predict the success of pleurodesis in patients with malign mesothelioma who underwent thoracoscopic talc poudrage.

While investigating the etiology of spontaneous mediastinum, particularly in a young, healthy patient, ecstasy abuse should always be considered. Because the fatal complications that may develop due to ecstasy ingestion may be overlooked.

Compared with children, foreign body aspiration is rare in adults and sometimes could be fatal. The most common symptom is severe coughing that appears after choking. Foreign body aspiration should be considered in the diagnosis of patients with wheezing, chronic cough, voice alteration, hoarseness, or recurrent pulmonary infections. A 78-year-old male patient with tracheostomy due to laryngeal cancer was admitted to the emergency department on account of aspiration of tweezers after coughing when he was cleaning the tracheostomy cannula. The patient complained of shortness of breath and coughing. Physical examination revealed decreased breath sounds on the right side of the lung, and he had dyspnea and central cyanosis. There was a foreign body extending from the carina to the right main bronchus on the posteroanterior radiograph of the lung. In the operating room, rigid bronchoscopy was performed from the tracheostomy stoma and the foreign body was removed using forceps. This case shows that patients with tracheostomy and their relatives must be well educated in the care of stoma. Rigid bronchoscopy by an experienced specialist for foreign body aspiration is a safe and effective method for diagnosis and treatment.

However, in recent years, the aspiration of pins which are used for securing turbans (headscarves) is seen frequently in young women. The aim of this article was to review 105 patients who were admitted to our hospital for turban pin aspiration. Chest X-rays were used for diagnosis. Various methods of treatment were performed: laryngoscopy in 6 patients, flexible fiberoptic bronchoscopy in 16, rigid bronchoscopy in 93, and thoracotomy in one patient, while in the other patient the turban pin was spontaneously expectorated. Localization of the pin in the right bronchial system was common (52%). In 6 patients, turban pins located in the larynx were extracted successfully by direct laryngoscopy. Turban pins were successfully removed with a flexible fiberoptic bronchoscope in 4 patients of the 16 (25%) and by rigid bronchoscope in 93 patients of 94 (99%). The average time until discharge was 18 hours and there was no mortality. Turban pin aspiration is common in Islamic populations and treatment usually requires bronchoscopic procedures. In order to minimize turban pin aspiration frequency, we recommend that turbans should be secured by traditional fastening methods or with an apparatus which cannot be aspirated.

Surgical removal is currently the generally accepted choice of treatment for lung hydatidosis. However, operating on bilateral widespread lung hydatidosis is still controversial. The aim of this retrospective study was to evaluate the results of surgical treatment in bilateral multiple hydatid disease of the lung. Methods: In this study, we reviewed our experience in the surgical treatment of 17 (3.7%) patients with bilateral, and at least three, lung hydatid cysts. These 17 patients (8 male, 9 female), with an average age of 34.6 years (range 12-58 years), underwent bilateral staged thoracotomy. Results: In total 105 lung cysts were removed from 17 patients who underwent staged thoracotomies. The mean count of cysts was 6.7 (range 3-20 cysts). Most of the cysts (38.2%) were located in the right lower lobe. The mean interval between thoracotomies was 4.2 (range 3-5) days. Two patients (11.7%) had cysts associated with hepatic hydatidosis and one (5.8%) had cysts associated with the spleen; they were treated via phrenotomy during thoracotomies. All cysts were removed without lung resection. We observed some complications such as prolonged air leaks (n = 2), atelectasis (n = 3) and empyema (n = 2). No further surgery was required for management of complications. The mean hospital stay was 9.3 days. (range 7-23 days). Oral albendazole was started on the 2nd post operative day after the first thoracotomy in the dose of 10-20 mg/kg and was continued for 3 months with a gap of 1 week after each 21 days. No recurrences or deaths occured during the follow-up period. Conclusions: Although staged thoracotomy applied in 3-5 days after the initial thoracotomy increases the total hospital stay, it decreases the chance of possible complications can occur in cysts in the other lung when long intervals are preferred between the first and the second thoracotomy. In our experience, bilateral staged thoracotomy is an appropriate surgical option because morbidity rates are minimal and the hospital stay is acceptable for the treatment of bilateral widespread lung hydatidosis, even in patients who had a total of 20 hydatid cysts.

We describe the characteristics of 2 patients with adenoid cystic carcinoma who presented with symptoms suggestive of bronchial asthma of 2 years duration. Biopsy specimens revealed that the lesions were adenoid cystic carcinoma of the trachea. Both patients underwent collar incision and median sternotomy, the tumors were resected, and the tracheas were reconstructed primarily. Postoperative radiotherapy was given. Their postoperative clinical courses were uneventful and no local recurrences were seen at the 1-year follow-up.

This study was aimed to investigate whether systemic inflammatory reaction induced by insuflation of talc into the pleura can predict the outcome of pleurodesis. A total of 58 consecutive patients (26 men, 32 women) with malignant pleural mesothelioma underwent video-assisted thoracoscopy under general anaesthesia with monopulmonary ventilation between the years 2003 and 2006. Four grammes of asbestos-free and sterile talc were insuflated into the pleural space under direct vision. To assess the success of pleurodesis, chest radiographs were obtained at the 8th and 30th postoperative days. Venous blood samples were drawn both on admission and at the 24th hour after pleurodesis for the analysis of white blood cells, erythrocyte sedimentation rate and C-reactive protein. The mean age (standard deviation) of patients was 59.0 +/- 12.0 years. Pleurodesis was achieved (no effusion on chest radiograph) in 43 of 58 patients (74.1%)(group I), whereas it was unsuccessful in the remaining 15 patients (25.9%)(group II). There was a significant difference between two groups for basal and postpleurodesis levels of measured inflammatory parameters, C-reactive protein and erythrocyte sedimentation rate (for each, P < 0.05). However, the difference was not significant for white blood cells between the groups. Serum levels of inflammatory parameters (C-reactive protein and erythrocyte sedimentation rate) may be used to predict the success of pleurodesis in patients with malign mesothelioma who underwent thoracoscopic talc poudrage.
