The validity of clinical trial results is influenced by researchers’ decisions regarding the mana... more The validity of clinical trial results is influenced by researchers’ decisions regarding the management of missing data. Inadequate management of missing data has been identified as a significant source of bias that can result in an overestimation of drug efficacy. Transparency related to the management of missing data is essential to assess the strength of evidence reported in publications. In a subset of 17 randomised clinical trials for two new antidepressant medications, we present a case study in which we examined investigators’ decisions regarding how to handle missing data and if their chosen method took into account, possible violations of analytic requirements that could affect results. The majority of trials (76%) concluded that there was a benefit of antidepressant treatment and in 94% the methodology for handling missing data was identifiable. Feminists have contributed to a critical analysis of the Diagnostic and Statistical Manual of Men... more Feminists have contributed to a critical analysis of the Diagnostic and Statistical Manual of Mental Disorders (DSM) since it was first published by the American Psychiatric Association in 1952. These analyses have focused on the philosophical and political dimensions of the manual's underlying theory and approach as well as the ways in which many disorders reinforce gender, class, and race bias. Major areas of criticism include the lack of attention to the sociopolitical context of emotional distress, DSM-5's ethnocentric framework, and the widening of diagnostic boundaries. The pharmaceutical industry’s influence on psychiatric research and practice has been profound an... more The pharmaceutical industry’s influence on psychiatric research and practice has been profound and has resulted in exaggerated claims of the effectiveness of psychotropic medications and an under-reporting of harms. After the regulatory approval of fluoxetine, the pharmaceutical industry began promoting (and continues to promote) a chemical imbalance theory of emotional distress. In the last decade, there has been an increased awareness about the limits of this theory and the risks of psychotropic medications. Nonetheless, the medicalization of distress, the sedimented belief in “magic bullets,” and the push to “scale up” mental health treatment have contributed to the meteoric rise in the prescription of psychiatric drugs and of polypharmacy. A major premise of this paper is that the conceptual framework of medical nihilism can help researchers and clinicians understand and address the harms incurred by inflated claims of the efficacy of psychotropic medications. First, identify the institution’s public mission, that is, its obligation to serve the public good. Next, identify the “economies of influence” that are present and may corrupt the institution. Then document the institution’s “corrupt” behaviors. At that point, seek to understand the harm done to society by the corruption. This step, which can be quite difficult, provides a moment to pause and reflect, for the harm done may go beyond what is immediately visible, and instead reach deep into our collective lives.
