OBJECTIVE To estimate the burden of type 2 diabetes mellitus and its percentage attributable to o... more OBJECTIVE To estimate the burden of type 2 diabetes mellitus and its percentage attributable to overweight and obesity in Brazil. METHODS The burden of diabetes mellitus was described in terms of disability-adjusted life years, which is the sum of two components: years of life lost and years lived with disability. To calculate the fraction of diabetes mellitus attributable to overweight, obesity, and excess weight, we used the prevalence of these risk factors according to sex and age groups (> 20 years) obtained from the 2008 Pesquisa Dimensões Sociais das Desigualdades (Social Dimensions of Inequality Survey) and the relative risks derived from the international literature. RESULTS Diabetes mellitus accounted for 5.4% of Brazilian disability-adjusted life years in 2008, with the largest fraction attributed to the morbidity component (years lived with disability). Women exhibited higher values for disability-adjusted life years. In Brazil, 49.2%, 58.3%, and 70.6% of diabetes mellitus in women was attributable to overweight, obesity, and excess weight, respectively. Among men, these percentages were 40.5%, 45.4%, and 60.3%, respectively. Differences were observed with respect to Brazilian regions and age groups. CONCLUSIONS A large fraction of diabetes mellitus was attributable to preventable individual risk factors and, in about six years, the contribution of these factors significant increased, particularly among men. Policies aimed at promoting healthy lifestyle habits, such as a balanced diet and physical activity, can have a significant impact on reducing the burden of diabetes mellitus in Brazil.

This study compares socio-demographic factors, mothers' biological characteristics, and quali... more This study compares socio-demographic factors, mothers' biological characteristics, and quality of care at maternity hospitals in the City of Rio de Janeiro, Brazil. A sample of 10,072 post-partum women in 47 hospitals was selected. Data were collected by interviewing mothers in the immediate post-partum and from medical records. The chi2 test was used to analyze homogeneity of ratios. Significant differences were found between patients in public and private hospitals in relation to family support, healthy habits during pregnancy, reproductive history, access to and satisfaction with prenatal care and delivery, and particularly adverse effects in the newborns. Private maternity hospitals showed better results, although they displayed excessive cesarean and neonatal inter-hospital transfer rates. A coproparasitologic and domiciliary survey was conducted in 19 selected census districts of the state of Rio de Janeiro, Brazil. A sample of 1,664 children aged between 1 - 9 years was selected and plotted in their own home' centroid. Geostatistics techniques allowed spatial exploratory analysis, variographic study, and ordinary kriging. Student t-test, odds ratio and confidence intervals were used in the statistical analysis. A prevalence of 27.5% was found for A. lumbricoides. Household income, housewife's education level and peridomiciliary conditions were identified as significantly associated factors to the occurrence of ascariasis. An isotropic spherical semivariogram model with 150-m reach, contribution of 0.45 and nugget effect of 0.55 was employed in ordinary kriging. The objective of this study was to evaluate the quality of data in the research project entitled ... more The objective of this study was to evaluate the quality of data in the research project entitled "Study on Neonatal and Perinatal Morbidity and Mortality and Care in the City of Rio de Janeiro", analyzing the completeness of patient records, inter-observer reliability, and concordance of collected data. The study interviewed a sample of 10,072 post-partum women, corresponding to 10.0% of the deliveries in the City of Rio de Janeiro. This article analyzed the concordance between data on patient records and the reproducibility of questionnaires by mean of rest/ retest, using common and prevalence-adjusted Kappa as well as the intra-class correlation coefficient. Losses totaled 4.5%, and the proportion of unknown data on patient records varied from 3.0 to 90.0%. Whoqol-bref was used to assess to what extent sociodemographic and health variables, the admission process and year of graduation simultaneously influence the quality of life (QOL) domains of medical students. 394 students with a mean age of 23 years participated in the study: 61% females, 43% scholarship holders and 20% with referred chronic morbidity (RCM). The lowest QOL scores were observed among women, with RCM, scholarship holders, economic class C and students in the 3rd and 6th years. Multiple linear regression analysis showed that all the independent variables analyzed had a negative association with QOL domains, and when assessed jointly contributed partly to its explanation, achieving 22% in the "environment" domain, influenced by their social class and the admission process. The presence of RCM had a negative influence on the physical, psychological and social relations domains. The last two domains were also influenced by the year of graduation. Variables with a positive influence on QOL need to be explored further. The data obtained are enough to serve as the base for care strategies for the most vulnerable students during medical training, giving special attention to scholarship students.

RESUMO O WHOQOL-bref foi utilizado em estudo transversal para avaliar a influência do sexo, da fo... more RESUMO O WHOQOL-bref foi utilizado em estudo transversal para avaliar a influência do sexo, da forma de ingresso, do ano de graduação, da classe econômica e da presença de morbidade crônica referida (MCR), na qualidade de vida (QV) de 394 estudantes de medicina da UERJ (72% dos 545 matriculados em 2010, erro 6,5%). A análise estatística foi feita no SPSS v17 e utilizados testes qui-quadrado, ANOVA/ pós Hoc de Bonferroni (p valor = 5%) e razão dos produtos cruzados ("Odds Ratio" -OR) com respectivos intervalos de confiança (IC 95%). A idade média foi de 23 anos, sendo 61% mulheres, 43% cotistas (dos quais 64% mulheres) e 20% referiu morbidade crônica. De 0-100, a média da QV referida foi 66 e da satisfação com saúde 62. Os menores escores de QV foram observados nos estudantes do sexo feminino, com morbidade crônica referida (MCR), que ingressaram por reserva de vagas, da classe C e do terceiro e sexto ano. Os resultados apontam para a necessidade de estudos qualitativos para aprofundar as informações sobre a QV dos estudantes da UERJ. ABSTRACT WHOQOL-bref scoring was used in this cross-sectional study to assess the influence of sex, form of admission, year of graduation, economic class and the presence of referred chronic morbidity (RCM) on the quality of life (QOL) of 394 medical students from UERJ (72% of 545 enrolled in 2010, margin of error 6.5%). The statistical analysis was performed in SPSS v17 using chi-squared tests, Bonferroni post-hoc ANOVA (pvalue=5%) and odds ratio (OR) with corresponding confidence intervals (CI 95%). Mean age was 23 years, with 61% women, 43% admitted under positive discrimination ethnic quotas (64% women) and 20% referred chronic disease. From 0 to 100, the average referred QOL was 66 and satisfaction with health was 62. The lowest QOL scores were observed among 3 rd and 6 th year students who were women, with RCM, who had been admitted via reserved university places and belonged to economic class C. The results indicate the need for qualitative studies to improve our understanding of the QOL of UERJ students.

The study Pesquisa Dimensões Sociais das Desigualdades (PDSD) (Social Dimensions of Inequalities)... more The study Pesquisa Dimensões Sociais das Desigualdades (PDSD) (Social Dimensions of Inequalities) involves 12,423 randomly selected Brazilian men and women aged 18 years old or more from urban and rural areas of the five Brazilian regions, and the information collected included the SF-36 as a measure of health-related quality of life. This provided a unique opportunity to develop age and gender-adjusted normative data for the Brazilian population. Brazilian men scored substantially higher than women on all eight domains and the two summary component scales of the SF-36. Brazilians scored less than their international counterparts on almost all of SF-36 domains and both summary component scales, except on general health status (US), pain (UK) and vitality (Australia, US and Canada). The differences in the SF-36 scores between age groups, genders and countries confirm that these Brazilian norms are necessary for comparative purposes. The data will be useful for assessing the health status of the general population and of patient populations, and the effect of interventions on health-related quality of life.

It is a cross-sectional study based on data obtained from a population-based survey carried out in Brazil in 2008. The sample composed of 12,423 Brazilians, older than 20 years. Physical and mental quality of life were both measured by SF-36 and scores were grouped in "above the mean" and "below the mean" to set binary outcomes. Logistic regression analysis was carried out to verify the impact of occupational position on the chances of better physical and mental quality of life, controlling it by socio-demographic and health variables. Results showed that Brazilians included on the labour market have better chances of a good physical and mental quality of life, even if controlled by other variables. The objective of this study is to evaluate the psychometric properties of a user satisfaction sca... more The objective of this study is to evaluate the psychometric properties of a user satisfaction scale regarding the Brazilian National STD/AIDS Program, specifically related to dispensing AIDS medicines. The scale was developed and applied in a study covering 10 Brazilian States that evaluated the quality of medicine dispensing. The questionnaire was answered by 1,412 people living with HIV and undergoing antiretroviral therapy. Construct validation involved two stages of factor analysis. The item-total correlation matrix was analyzed, and tests for associations between the target variable, socio-demographic variables, and related constructs were performed. Reliability was studied by means of the sub-scales' internal consistency, estimated by Cronbach's alpha. Five relevant satisfaction dimensions were identified. A moderate level of internal consistency was found for these dimensions, suggesting they were adequate. The results of the association tests agreed with other studies reported in the literature. We conclude that the instrument is appropriate for application in similar populations with adequate psychometric characteristics and serves to measure users' assessments of the pharmaceutical services received and helps to orient improvements in such services.

In Brazil the disease is prevalent in the Amazon and the control rationale is based on early diagnosis and treatment. Quality pharmaceutical services are considered a key element for control. A proposal for evaluating pharmaceutical services for malaria is presented here. A theoretical outline composed of a logical model and an indicator framework is discussed and strives to establish a basis for assessment and judgment of the way in which these services are actually delivered. The aim is to contribute to the understanding of pharmaceutical services for malaria and other endemic diseases, complying with the directives of the Brazilian National Medicines Policy.

Whoqol-bref was used to assess to what extent sociodemographic and health variables, the admission process and year of graduation simultaneously influence the quality of life (QOL) domains of medical students. 394 students with a mean age of 23 years participated in the study: 61% females, 43% scholarship holders and 20% with referred chronic morbidity (RCM). The lowest QOL scores were observed among women, with RCM, scholarship holders, economic class C and students in the 3rd and 6th years. Multiple linear regression analysis showed that all the independent variables analyzed had a negative association with QOL domains, and when assessed jointly contributed partly to its explanation, achieving 22% in the "environment" domain, influenced by their social class and the admission process. The presence of RCM had a negative influence on the physical, psychological and social relations domains. The last two domains were also influenced by the year of graduation. Variables with a positive influence on QOL need to be explored further. The data obtained are enough to serve as the base for care strategies for the most vulnerable students during medical training, giving special attention to scholarship students.

RESUMO O WHOQOL-bref foi utilizado em estudo transversal para avaliar a influência do sexo, da fo... more RESUMO O WHOQOL-bref foi utilizado em estudo transversal para avaliar a influência do sexo, da forma de ingresso, do ano de graduação, da classe econômica e da presença de morbidade crônica referida (MCR), na qualidade de vida (QV) de 394 estudantes de medicina da UERJ (72% dos 545 matriculados em 2010, erro 6,5%). A análise estatística foi feita no SPSS v17 e utilizados testes qui-quadrado, ANOVA/ pós Hoc de Bonferroni (p valor = 5%) e razão dos produtos cruzados ("Odds Ratio" -OR) com respectivos intervalos de confiança (IC 95%). A idade média foi de 23 anos, sendo 61% mulheres, 43% cotistas (dos quais 64% mulheres) e 20% referiu morbidade crônica. De 0-100, a média da QV referida foi 66 e da satisfação com saúde 62. Os menores escores de QV foram observados nos estudantes do sexo feminino, com morbidade crônica referida (MCR), que ingressaram por reserva de vagas, da classe C e do terceiro e sexto ano. Os resultados apontam para a necessidade de estudos qualitativos para aprofundar as informações sobre a QV dos estudantes da UERJ. ABSTRACT WHOQOL-bref scoring was used in this cross-sectional study to assess the influence of sex, form of admission, year of graduation, economic class and the presence of referred chronic morbidity (RCM) on the quality of life (QOL) of 394 medical students from UERJ (72% of 545 enrolled in 2010, margin of error 6.5%). The statistical analysis was performed in SPSS v17 using chi-squared tests, Bonferroni post-hoc ANOVA (pvalue=5%) and odds ratio (OR) with corresponding confidence intervals (CI 95%). Mean age was 23 years, with 61% women, 43% admitted under positive discrimination ethnic quotas (64% women) and 20% referred chronic disease. From 0 to 100, the average referred QOL was 66 and satisfaction with health was 62. The lowest QOL scores were observed among 3 rd and 6 th year students who were women, with RCM, who had been admitted via reserved university places and belonged to economic class C. The results indicate the need for qualitative studies to improve our understanding of the QOL of UERJ students.

The study Pesquisa Dimensões Sociais das Desigualdades (PDSD) (Social Dimensions of Inequalities)... more The study Pesquisa Dimensões Sociais das Desigualdades (PDSD) (Social Dimensions of Inequalities) involves 12,423 randomly selected Brazilian men and women aged 18 years old or more from urban and rural areas of the five Brazilian regions, and the information collected included the SF-36 as a measure of health-related quality of life. This provided a unique opportunity to develop age and gender-adjusted normative data for the Brazilian population. Brazilian men scored substantially higher than women on all eight domains and the two summary component scales of the SF-36. Brazilians scored less than their international counterparts on almost all of SF-36 domains and both summary component scales, except on general health status (US), pain (UK) and vitality (Australia, US and Canada). The differences in the SF-36 scores between age groups, genders and countries confirm that these Brazilian norms are necessary for comparative purposes. The data will be useful for assessing the health status of the general population and of patient populations, and the effect of interventions on health-related quality of life.

It is a cross-sectional study based on data obtained from a population-based survey carried out in Brazil in 2008. The sample composed of 12,423 Brazilians, older than 20 years. Physical and mental quality of life were both measured by SF-36 and scores were grouped in "above the mean" and "below the mean" to set binary outcomes. Logistic regression analysis was carried out to verify the impact of occupational position on the chances of better physical and mental quality of life, controlling it by socio-demographic and health variables. Results showed that Brazilians included on the labour market have better chances of a good physical and mental quality of life, even if controlled by other variables. The objective of this study is to evaluate the psychometric properties of a user satisfaction sca... more The objective of this study is to evaluate the psychometric properties of a user satisfaction scale regarding the Brazilian National STD/AIDS Program, specifically related to dispensing AIDS medicines. The scale was developed and applied in a study covering 10 Brazilian States that evaluated the quality of medicine dispensing. The questionnaire was answered by 1,412 people living with HIV and undergoing antiretroviral therapy. Construct validation involved two stages of factor analysis. The item-total correlation matrix was analyzed, and tests for associations between the target variable, socio-demographic variables, and related constructs were performed. Reliability was studied by means of the sub-scales' internal consistency, estimated by Cronbach's alpha. Five relevant satisfaction dimensions were identified. A moderate level of internal consistency was found for these dimensions, suggesting they were adequate. The results of the association tests agreed with other studies reported in the literature. We conclude that the instrument is appropriate for application in similar populations with adequate psychometric characteristics and serves to measure users' assessments of the pharmaceutical services received and helps to orient improvements in such services.

In Brazil the disease is prevalent in the Amazon and the control rationale is based on early diagnosis and treatment. Quality pharmaceutical services are considered a key element for control. A proposal for evaluating pharmaceutical services for malaria is presented here. A theoretical outline composed of a logical model and an indicator framework is discussed and strives to establish a basis for assessment and judgment of the way in which these services are actually delivered. The aim is to contribute to the understanding of pharmaceutical services for malaria and other endemic diseases, complying with the directives of the Brazilian National Medicines Policy.
