However, occupational methacrylate allergy in dental personnel is a growing phenomenon. The aims of this retrospective observational study are to evaluate the risk of occupational contact dermatitis following exposure to methacrylates in dental personnel and to identify possible preventive measures. Materials and Methods: A total of 126 subjects exposed to acrylic and methacrylic resins in their professional context and who reported clinical manifestations were included from our outpatient department database. These were subdivided into two groups: 81 dental technicians and 45 dental hygienists. All the subjects had undergone patch testing with a “methacrylate series” (FIRMA) and readings were taken after 2 days (D2), 4 days (D4), and 7 days (D7). Results: A significantly higher incide...

Research paper thumbnail of Morphea induced by SARS‐CoV‐2 infection: A case report

Morphea induced by SARS-CoV-2 infection: A case report Dear Editor, The widespread diffusion of s... more Morphea induced by SARS-CoV-2 infection: A case report Dear Editor, The widespread diffusion of severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2), a member of the coronavirus family responsible for Coronavirus disease (COVID-19), is still in the headlines for the impressive health and economic burden worldwide. The immune response against this novel virus is still undergoing extensive research, and the possible long-term outcomes remain basically unknown. Immunity plays a key role in the defense against several microorganisms, and SARS-CoV-2 is no different. The infections can result in a hyperactive immune response with excessive inflammatory reactions. These are generally accompanied by the release of impressive amounts of proinflammatory cytokines, an event known as “cytokine storm,” that is correlated with lung damage, organ failure, and a negative prognosis. The possibility that these immunologic derangements could result in the induction and/or the exacerbation of other immune-mediated diseases, including cutaneous ones, has not been completely investigated. We report the case of a 61-year-old female patient referred to our dermatologic department for the development of asymptomatic bilateral sclerotic cutaneous lesions on the forearms which appeared 2 months before. The physical examination revealed the presence of several brownish and violaceous plaques with mild erythematous borders on forearms, with sclerotic appearance, partially tending to confluence. The lesions showed a symmetrical and bilateral distribution and were about 5–10 cm in diameter (Fig. 1a). The patient referred that the skin manifestations started about 1 month after the dismissal from hospitalization for COVID-19 pneumonia (confirmed through RT-PCR). Her familiar and personal history was negative for autoimmune and chronic inflammatory skin disorders. On suspicion of localized morphea, blood examination tests and skin biopsy were performed. Blood tests, including antinuclear antibodies (ANA), antibodies to single-stranded DNA (a-ssDNA), autoantibodies to extractable nuclear antigens (ENAs), and COVID-19 swab test, were negative excluding systemic involvement. Histological examination revealed the presence of thin epidermis with moderate dermal sclerosis and thickening of collagen fibers, confirming the diagnosis of morphea. A topical therapy with a high-potency steroid (clobetasol ointment) and vitamin E emollient was started, with a remarkable improvement after 8 weeks and a complete clinical resolution after 16 weeks of treatment (Fig. 1b). Morphea, also known as localized scleroderma, is an inflammatory skin condition that is characterized by the presentation of single or multiple inflammatory or sclerotic plaques. While the pathogenesis remains largely unknown, several factors can contribute to the emergence of autoimmune disease including the genetic predisposition, environmental triggers such as bacterial and viral infections, and intrinsic factors, such as hormonal and immunologic dysregulation. In this case, the negative personal anamnesis for autoimmune disorders and the development of cutaneous manifestations after COVID-19 infection suggested a possible link between autoimmune disease and SARS-CoV-2. Some authors proposed that SARS-CoV-2 could act as a trigger in the development of organ-specific autoimmune disorders, in genetic

Inflammation is an important reaction in rosacea not only due to inflammatory reactions to cutaneous microorganisms, such as Demodex follicolorum, but also to ultraviolet damage that generates reactive oxygen species. This study evaluated the efficacy and tolerability of a nonsteroidal anti‐inflammatory 18‐beta glycyrrhetinic acid cream for the treatment of mild rosacea by means of noninvasive methods. A total of 24 subjects suffering from erythemato‐telangiectatic or mild papulo‐pustular rosacea were recruited in the trial. Twelve patients applied an anti‐inflammatory cream with 18‐beta glycyrrhetinic acid twice daily for 20 days and 12 patients, recruited as control, applied the same formulation without 18‐beta glycyrrhetinic acid. After 10 days of treatment, a significant reduction of erythema was recorded in the patient sample who applied the 18‐beta glycyrrhetinic acid cream, the mean change from baseline showed an increase in hydration level of the skin surface but it was not statistically significant. The use of 18‐beta glycyrrhetinic acid cream can be helpful in managing symptoms and condition of rosacea skin, especially in the management of erythema.

The blast cell populations studied were characterized by morphologic, cytochemical, immunochemical, and molecular criteria. With all the methods used the populations were found to be highly homogeneous and showed a typical lymphoid phenotype. In particular, the Ig heavy-chain gene rearrangement was largely prevalent, and the germ line configuration was almost absent. However, in three of eight cases, high levels of MPO mRNA were detected. The remarkable homogeneity of the cell populations examined suggests that the MPO mRNA observed was present in cellular elements certainly identified as lymphoid. The absence of contamination by myeloid cells was confirmed by the results of Western blot analysis of the proteins of the cell population studied: no MPO protein was detectable. The levels of mRNA observed were hi...

Clinical presentation and localization are related to the specific instrument. Cheilitis in woodwind players (eg, saxophonists, flautists, and clarinetists) and brass instrumentalists (eg, trumpeters) is frequently observed. It is mostly an irritant dermatitis, but allergic contact sensitization (eg, to colophony, nickel, and exotic woods) has been described. A 26-year-old saxophonist was first referred to us with a 6-month history of a patch of erythema, desquamation, hyperkeratosis, and peripheric vesiculation of the median area of the lower lip (Fig 1). The disease developed about 2 months after he replaced the plastic reed on his saxophone with a wood reed. The result of patch tests with the Società Italiana di Dermatologia Allergologica Professionale e Ambientale standard series was negative. The result of patch testing with fragments of the wood reed wetted with water was positive (++). The same test was performed in 10 volunteers, including another saxophonist, and showed negative results. The patient returned to playing with plastic reeds, and the cheilitis subsided after a few weeks.

Methods: Different peelings (binary system pyruvic acid and trichloroacetic acid-TCA, and ternary system pyruvic acid and TCA) were tested on the volar forearm of 20 volunteers of both sexes between 28 and 50 years old. The outcomes were evaluated at the baseline, 10 minutes, 24 hours, and 1 week after the peeling by means of noninvasive skin diagnosis techniques. In vivo reflectance confocal microscopy was used for stratum corneum evaluation, transepidermal waterloss, and Corneometry for skin barrier and hydration, Laser Doppler velocimetry in association with colorimetry for irritation and erythema analysis. Results: The instrumental data obtained showed that the efficacy and safety of the new ternary system peel compounds were significantly higher compared with the binary system formulations tested. The new formulation peels improved chemexfoliation and reduced complications such as irritation, redness, and postinflammatory pigmentation compared to the traditional aqueous solutions. Conclusion: The study showed that ternary system chemexfoliation, using a controlled delivery technology, was able to provide the same clinical effects in term of stratum corneum reduction with a significantly reduced barrier alteration, water loss, and irritation/erythema compared to traditional binary system peels.

The histopathology of the lesion after incisional biopsy demonstrated irregularly dilated capillary vessels surrounded by lymphocytic perifollicular infiltrates in the upper dermis, dilated follicular infundibula with hyperplastic sebaceous glands, solar elastosis, and increase in fat tissue (Fig. 2). Given this clinical and histologic findings, she was diagnosed with gnatophyma. The patient was then treated with azelaic acid and applied daily sunscreens. At dental examination, poor oral health was revealed: bacterial plaque was present on every tooth, and a general inflammation of the marginal gingiva was noted. Dental care was started. At the 1-month follow-up visit, a significant clinical improvement of the skin lesion was detected. Case 2, a 50-year-old Caucasian female, presented with a history of an asymptomatic erythema and edema on her chin that had begun about 2 years earlier. There was nothing relevant in her clinical history. There were no dental abnormalities. Physical examination was remarkable for a not well-defined and erythematous patch (Fig. 3). A punch biopsy was taken from the lesion and demonstrated a follicular and capillary vessel dilatation and sebaceous gland hyperplasia as per gnatophyma (Fig. 4). Rosacea is a common skin disease. Its main clinical manifestations are redness, telangiectasia, papules, pustules, skin thickening, and phymas [1, 2]. Phyma is considered the last evolutionary stage of rosacea. Its most frequent localization is in the nose (rhinophyma), but more rarely, it can occur at chin level (gnatophyma), forehead (metophyma), ears (otophyma), and eyelids (blepharophy
