A prevention policy is needed to avoid deterioration of the health of athletes and reduce costs. Thus, determining a risk factor is a target. To our knowledge no study has examined the performance of dynamic factors such as functional tests. However, it has already been proven that a deficit of postural balance increases the risk of developing a sprain in basketball players. Objectives.-The purpose of this study was to evaluate whether poor performance in two functional tests unipodal was linked to increased incidence of ankle sprain in four months. Methods.-Eighty-three rugby players in 4 teams in the elite amateur championships (federal 1 and 2) were evaluated and followed for 6 months. We recorded the time (in sec.) in two tests, the figure-of-eight hop test (F test) and side-hop test (S test) in September. During part of the sports season, until the following February (22 weeks), the occurrence of sprain was noted. At the end, two groups were formed (sprain vs non-sprain). The initial performance tests were compared using non-parametric tests. Results.-Finally, 11 (13.25%) of the 83 players had a sprained ankle. Statistical analysis revealed that their time tests were not significantly different from those who were not injured. Discussion.-Lack of data does not lead to a definitive conclusion, but it seems that the tests are not suited for rugby players. There should be more specific tests for this sport.

Study Design Descriptive epidemiology study. Methods The patients included had cervical spine injuries causing neurological disorder classified from the ASIA scale, grade A to D. A retrospective review of all cases that occurred between the 1996-1997 and the 2005-2006 seasons was made. Circumstances of the injuries and of the clinical outcome were collected by interview. Results There were 37 cases of catastrophic cervical spine injuries in French rugby for the last 10 years. The incidence of the cervical spine injuries decreased during this period. The rates of injury were 2.1 per 100 000 players per year during the 1996-1997 season and 1.4 during the 2005-2006 season (P <. 01). The second season, 7 out of 21 isokinetic tests were abnormal, 7 players suffered injuries (92 days of lay-off). Before the study, the average days of work-stoppage amounted to 225 days: higher frequency of injuries on the group with normal isokinetic assessment (41% vs. 25%). Conclusions.-These results are encouraging to propose a systematic guideline for all players to prevent them from injuries since players with normal tests but without prevention underwent more injuries.

Objective.-Isokinetic strengthening is a rehabilitation technique rarely used in stroke patients.... more Objective.-Isokinetic strengthening is a rehabilitation technique rarely used in stroke patients. However, the potential benefits of force and endurance training in this population are strongly suspected. Method.-This literature review synthesizes the results of clinical trials on this topic. The research was conducted on PubMed, using ''Stroke'', ''rehabilitation'', ''isokinetic'', ''upper limb'' and ''training'' as keywords. Results.-Seventeen studies focusing on the use of isokinetics in assessment or rehabilitation (six studies) following stroke were reviewed. For the lower limb, muscle strength and walking ability improved after isokinetic rehabilitation programs. For the upper limb, the only two studies found in the literature suggest improvement in the strength of the trained muscles, of grip force, of the Fugl-Meyer motor score and of global functional capacities. This review does not reveal any consensus on the protocols to be implemented: type of muscle contraction, velocities.. .. Conclusion.-While isokinetic strengthening has not proven its efficiency in rehabilitation of the upper limb following stroke, its interest with regard to rehabilitation of the lower limbs has been recognized. Randomized controlled trials in this field are necessary to confirm its efficiency, especially concerning upper arm rehabilitation.

Conclusion.-It seems that quantified gait parameters are not relevant evaluation criteria to assess the efficiency of a treatment with botulinum toxin type A. This evaluation must be done using satisfaction scales fulfilled by the patient, linked with therapeutic objectives that are well specified before the treatment, with the PRM doctor.

Méthode.-Enquête qualitative, par entretiens individuels basés sur un guide semi-dirigé et enregistré sur un dictaphone puis retranscrits textuellement. Les patients recrutés en soins primaires de Médecine Générale et en service de Médecine Physique et de Réadaptation sur la région Auvergne souffraient de lombalgie commune depuis plus de trois mois. Résultats.-Une analyse thématique du contenu réalisée sur 16 patients a permis de mettre en évidence quatre catégories de barrières à l'activité physique. Elles sont d'ordres physiques, psychologiques, socio-professionnelles et environnementales. La douleur, la peur de la douleur et du mouvement, et l'activité physique professionnelle ressentie comme un facteur aggravant ont été rapportés comme des freins importants. Les patients ont également cité le manque de motivation, la dépression, l'anxiété et l'appréhension d'aggraver leurs lombalgies par l'activité physique. Ils ont par ailleurs exprimé l'influence des informations médicales et non médicales reçues sur leurs attitudes et leurs comportements. Les résultats soulignent l'importance des peurs et croyances dans les représentations des patients lombalgiques concernant l'activité physique. Conclusion.-Une évaluation psycho-comportementale des lombalgiques chroniques est indispensable afin de repérer les freins à la pratique de l'activité physique. Le médecin généraliste, acteur de première ligne dans l'éducation thérapeutique des patients, a un rôle clé à jouer dans cette prise en charge.

The hypothesis of the anteriorisation of COP associated with spasticity is not statistically significant but was found with two patients who were assessed twice: anteriorisation of the hemiplegic side COP in the second pass associated with the development of spasticity. This study should be completed by the inclusion of more patients to look for a correlation between these statistical parameters, medical examination and clinical evolution.

We have sought to evaluate the reproducibility over time of isokinetic testing of the hip flexor (FI) and extensor (Ext) muscles and to establish whether there is a significant difference in peak torque (PT) between the left and right hips. Patients and methods.-Ten adults were tested once a week for 3 weeks by the same investigator and according to the same protocol, with two velocities (608/s and 1808/s) for the hip FI and Ext in concentric tests and one velocity (308/s) for the Ext only in eccentric tests. The reproducibility of the measured PT was analyzed by using the intraclass correlation coefficient (ICC) and a Bland and Altman plot. The difference in PT between the right and left hips was tested using Student's T test. Results.-The ICC for the observed PT values revealed very good reproducibility (with a value of between 0.75 and 0.96) for the hip FI and Ext measurements (regardless of the body side, test velocity or contraction mode). We did not observe any significant PT differences between the right and left hips. Conclusion.-The isokinetic assessment of the concentric and eccentric PT values generated by the hip FI and Ext is highly reproducible. There is no difference between dominant and nondominant body sides, which enables the use of the contralateral limb as a reference.

Les évaluations en état cliniquement défini comme OFF (> 12 h après la dernière prise de traiteme... more Les évaluations en état cliniquement défini comme OFF (> 12 h après la dernière prise de traitement dopaminergique) comportent UPDRS-III, score GMT de lever du sol, mouvements alternatifs rapides, écriture et spiralographie (coefficient de symétrie), qualité de vie (PDQ-39), score de dépression (GDS-15), un enregistrement vidéo de 8 activités quotidiennes et des mesures biomécaniques (paramètres de marche, force motrice des extenseurs, postures rachidiennes et équilibre). Un score UPDRS-III est également pratiqué 1 h après la prise du traitement dopaminergique. Ces mesures sont réalisées au début du programme, après les 8 semaines de rééducation et 3 mois après la fin des séances. Conclusions.-Si une supériorité est démontrée en faveur d'un programme de renforcement moteur asymétrique, cela apportera une base contrôlée aux pratiques rééducatives dans la maladie de Parkinson. Quel que soit le programme s'avérant le plus efficace, nous espérons que l'étude quantifiera des améliorations dont l'importance réactivera l'intérêt pour la rééducation dans la maladie de Parkinson et pour des prescriptions de programmes précis de travail physique moteur dès les stades précoces de l'affection.

A contact sport par excellence, it causes a significant number of injuries. In Rugby Union, there are 30 to 91 injuries per 1000 match hours. This epidemiological review of injuries incurred by rugby players mentions the position and type of injuries, the causes, time during the match and season in which they occur and the players' positions as well as the length of players' absences following the injury.

Les évaluations en état cliniquement défini comme OFF (> 12 h après la dernière prise de traiteme... more Les évaluations en état cliniquement défini comme OFF (> 12 h après la dernière prise de traitement dopaminergique) comportent UPDRS-III, score GMT de lever du sol, mouvements alternatifs rapides, écriture et spiralographie (coefficient de symétrie), qualité de vie (PDQ-39), score de dépression (GDS-15), un enregistrement vidéo de 8 activités quotidiennes et des mesures biomécaniques (paramètres de marche, force motrice des extenseurs, postures rachidiennes et équilibre). Un score UPDRS-III est également pratiqué 1 h après la prise du traitement dopaminergique. Ces mesures sont réalisées au début du programme, après les 8 semaines de rééducation et 3 mois après la fin des séances. Conclusions.-Si une supériorité est démontrée en faveur d'un programme de renforcement moteur asymétrique, cela apportera une base contrôlée aux pratiques rééducatives dans la maladie de Parkinson. Quel que soit le programme s'avérant le plus efficace, nous espérons que l'étude quantifiera des améliorations dont l'importance réactivera l'intérêt pour la rééducation dans la maladie de Parkinson et pour des prescriptions de programmes précis de travail physique moteur dès les stades précoces de l'affection.
