Introduction. Les photoallergies au ketoprofene et les reactions croisees avec les substances con... more Introduction. Les photoallergies au ketoprofene et les reactions croisees avec les substances contenant le noyau diphenylcetone (oxybenzone, acide tiaprofenique, fenofibrate) sont bien connues. Nous rapportons des cas de photoreactions avec des substances qui ne contiennent pas a priori ce noyau. Malades et methodes. II s'agissait d'une etude retrospective entre juin 1998 et juillet 2000 et prospective entre aout 2000 et janvier 2001. Nous avons teste systematiquement par photopatch tests les malades qui nous etaient adresses pour photoallergie au Ketum®. Ces malades etaient testes avec le Ketum®, le ketoprofene, l'oxybenzone, l'acide tiaprofenique, le fenofibrate, mais aussi la chlorphenesine, la batterie « filtres solaires » et toute la batterie des photobiologistes sauf le « lichen mix » et la benzocaine. Les tests etaient poses en double a Jo, la moitie etait irradiee a J 1 en UVA, les temoins restant couverts. La lecture avait lieu a J 3 et J 4 en utilisant le s...

INTRODUCTION Methylisothiazolinone chloride (Kathon CG) and its derivatives, used as preservative... more INTRODUCTION Methylisothiazolinone chloride (Kathon CG) and its derivatives, used as preservatives in cosmetics, have been shown to be allergenic when used in humans although preliminary studies in Guinea pigs failed to reveal any sensitization. Dibromodicyanobutane, associated with phenoxyethanol in Euxyl K400, has been proposed to replace the isothiazolinone derivatives in cosmetics. Preliminary study has shown that these thiazolinone derivatives are not allergenic in Euxyl K400. The similarity of these situations led us to hypothesize that dibromodicyanobutane could become allergenic. METHODS We tested 0.1 p. 100 dibromodicyanobutane in vaseline in the additional battery of the ICDRG battery in patients with contact eczema. Among the 310 patients tested, 1.94 p. 100 had a positive test for this allergen (during this same period, 1.29 p. 100 of the patients were positive for isothiazolinones). Three patients were hospitalized because of generalized eczema and 1 patient had changed...

To determine the clinical features of contact dermatitis caused by antiseptics and to ascertain w... more To determine the clinical features of contact dermatitis caused by antiseptics and to ascertain whether the substance responsible is the antiseptic itself or the excipients. A multicenter, retrospective study based on analysis of all cases reported over a 2-year period to the Dermato-Allergology Vigilance network known as Revidal. Each dossier contained details of the clinical characteristics of lesions, the incriminated antiseptic, the mode of exposure and the results of patch tests done with the antiseptic in question. 75 patients (mean age: 44 years) were sensitized to chlorhexidine (14 cases), hexamidine (20 cases), povidone iodine (14 cases), mercuric antiseptics (3 cases), triclocarban (Septivon, 17 cases), hexamidine-chlorhexidine-chlorocresol (Cytéal, 4 cases), or chlorhexidine surfactant (Hibiscrub), cetrimide or chlorhexidine digluconate (Diaseptyl) (1 case each). To develop a standard panel of photopatch tests, the French Society of Photodermatology conducted... more To develop a standard panel of photopatch tests, the French Society of Photodermatology conducted a prospective study from 1991 to 2001 on the frequency of photoallergens encountered in France and on the relevance of the choice of the various photoallergens. Thirteen photobiology centers participated in the study from 1991 to 1995, and ten centers from 1995 to 2001. A set of 3 samples of photopatch tests was applied on any patient suspected of photoallergy. On Day 2, two sets were irradiated with ultraviolet A (UVA) and total spectrum (DEM 0.75); with the third set being used as control. Readings were made on D3 and D4. Two thousand sixty-seven patients were tested. Eight hundred fifty-six, i.e., 41% exhibited one or several positive tests. In the majority of cases it was a photoallergy (39.7 to 60% of cases) and eczema (29.5 to 45.6%). Photoaggravation was infrequent (7.9 to 10.3%). Cases of phototoxicity were rare.
