Background: The Coordinated Care in Myocardial Infarction Program (KOS-MI) was introduced to impr... more Background: The Coordinated Care in Myocardial Infarction Program (KOS-MI) was introduced to improve prognosis for patients after myocardial infarction (MI). The program includes complete revascularization followed by unrestricted access to rehabilitation, electrotherapy, and cardiac care. Aim: This study aimed to assess major adverse cardiac and cerebrovascular events (MACCE) of patients enrolled in the KOS-MI at 3-year follow-up. Methods: This is a retrospective, multicenter registry of patients treated for MI. The study group (KOS-MI) of 963 patients was compared to the control group (standard care) of 1009 patients. At 3-year follow-up, MACCE including death, MI, stroke, and repeated revascularization were reported. Additionally, hospitalization for heart failure (HF) was analyzed. Propensity score matching (PSM) was utilized for group baseline characteristic adjustment. Results: Patients in the KOS-MI group were younger (65 vs. 68 years; P <0.001), mostly men (70% vs. 62.9%; P <0.001), admitted with ST-segment elevation myocardial infarction (STEMI) (44.6% vs. 36.2%; P <0.001). Patients in the control group had more comorbidities and were admitted more often with non-ST-segment elevation myocardial infarction (63.8% vs. 55.4%; P <0.001) and acute HF (5.1% vs. 2.7%; P = 0.007). Following PSM, 530 well matched pairs were selected. At three years (92.3% follow-up completeness), the relative risk reduction was: 25% in MACCE (P = 0.008), 38% in mortality (P = 0.008), 29% in repeated revascularization (P = 0.04), and 28% (P = 0.0496) in hospitalization for HF in the KOS-MI group. Conclusions: The combination of contemporary invasive techniques, complete revascularization, cardiac rehabilitation, and ambulatory care included in the KOS-MI Program improves long-term prognosis of patients after MI in 3-year follow-up.

Provision of mental health care in Poland has long been characterised by an overreliance on psych... more Provision of mental health care in Poland has long been characterised by an overreliance on psychiatric hospitals and the underdevelopment of community care. The introduction of the first National Mental Health Protection Programme for 2011–2015, with the explicit goal to base provision of mental care on the community mental health centres, failed to achieve any tangible results. The ensuing critique led to the launch of the second National Mental Health Protection Programme for 2017–2022 and the establishment, from mid-2018 onwards, of 41 (33 in operation) mental health centres across Poland. These will be piloted until the end of 2022 but have already shown positive results in terms of access to non-stationary care and a small fall in hospitalisations. They have also performed well during the COVID-19 pandemic, allowing for a quick reorganization of care and continued provision of mental health services. The article describes how the notion of groups at risk of HIV infection changed under the influen... more The article describes how the notion of groups at risk of HIV infection changed under the influence of the development of studies on HIV in the years 1981–1986. The first diagnosed patients with the HIV symptoms were homosexual persons and drug addicts. In the initial phase of virus occurrence in the world these two groups were commonly regarded as the virus carriers and only they were associated with AIDS. With time the progress in research has revealed that infections with HIV cannot be linked solely to lifestyle, habits of homosexuals and drug addicts since the group of subjects potentially vulnerable to the infection is much broader and it started to be related to some risky behaviour. The article shows the process of alteration of the knowledge about the virus causing AIDS in the years 1981–1986. A new model of complex patient care after acute myocardial infarction (AMI) has been in operation... more A new model of complex patient care after acute myocardial infarction (AMI) has been in operation in Poland since late 2017, comprising invasive treatment, cardiac rehabilitation and scheduled outpatient follow-up. Its stated objectives are to improve secondary prevention measures, quality of care and long-term health outcomes in AMI-patients. The model implements all key aspects of post-MI care recommended by the European Society of Cardiology (ESC), representing the first nation-wide model of structured and comprehensive post-MI care that closely follows ESC guidelines. The aim of this paper is to describe the background of this reform, its content and implementation as well as to assess its results. Early outcomes seem promising, with significantly lower mortality rate and lower risk of serious cardiological events in patients participating in the new model of care compared to patients who were not included. During this period the methods of diagnosis and treatment of HIV/AIDS has changed significantly. The progress in this area of examinations which is presently achieved allows unambiguous diagnosis of the virus and the disease. The recognition of the pathogenesis of the HIV and AIDS enabled the beginning of the studies on the medicines having antiretroviral properties. The utilization of the potential of the currently used medicines inhibits the progress of the disease and, in consequence, the elongation of the patients' life span. The paper describes this phenomena on the basis of the Children’s University Hospital of Krakow, one of the leading polish centers in pediatrics. Authors analyze the case of a public clinical hospital asking if marketing activities should be used in such case. The issues focused on public relations and relations with the most important stakeholders are taken into account with a specific regard to the process of the hospital development, potential support for the hospital’s budget and for the future perspectives of such important provider. Background: European societies are ageing rapidly and thus health promotion for older people (HP4... more Background: European societies are ageing rapidly and thus health promotion for older people (HP4OP) is becoming an increasingly relevant issue. Crucial here is not only the clinical aspect of health promotion but also its organisational and institutional dimension. The latter has been relatively neglected in research on HP4OP. This issue is addressed in this study, constituting a part of the EU project ProHealth65+, engaging ten member countries. This paper is based on two intertwining research goals: (1) exploring which institutions/organisations are performing HP4OP activities in selected European countries (including sectors involved, performed roles of these institutions, organisation of those activities); (2) developing an institutional approach to HP4OP. Thus, the paper provides a description of the analytical tools for further research in this area. Methods: The mentioned aims were addressed through the mutual use of two complementary methods: (a) a literature review of scientific and grey literature; and (b) questionnaire survey with selected expert respondents from 10 European countries. The expert respondents, selected by the project's collaborating partners, were asked to fill in a custom designed questionnaire concerning HP4OP institutional aspects. Results: The literature review provided an overview of the organisational arrangements in different HP4OP initiatives. It also enabled the development of functional institutional definitions of health promotion, health promotion activities and interventions, as well as an institutional definition adequate to the health promotion context. The distinctions between sectors were also clarified. The elaborated questionnaires provided in-depth information on countries specifically indicating the key sectors involved in HP4OP in those selected countries. These are: health care, regional/local authorities, NGO's/voluntary institutions. The questionnaire and literature review both resulted in the indication of a significant level of cross-sectorial cooperation in HP4OP. Conclusions: The inclusion of the institutional analysis within the study of HP4OP provides a valuable opportunity to analyse, in a systematic way, good practices in this respect, also in terms of institutional arrangements. A failure to address this aspect in policymaking might potentially cause organisational failure even in evidence-based programmes. This paper frames the perception of this problem.

Systematyczny przegląd literatury został przeprowadzony przy użyciu bazy danych MEDLINE (dostęp poprzez PubMed), jak również Google Scholar w okresie do grudnia 2015 r. Sprawdzono również raporty ze źródeł innych niż medyczne bazy danych oraz piśmiennictwo zawarte w zidentyfikowanych doniesieniach naukowych. The paper describes this phenomena on the basis of the Children’s University Hospital of Krakow, one of the leading polish centers in pediatrics. Authors analyze the case of a public clinical hospital asking if marketing activities should be used in such case. The issues focused on public relations and relations with the most important stakeholders are taken into account with a specific regard to the process of the hospital development, potential support for the hospital’s budget and for the future perspectives of such important provider. The case study explicitly illustrates the importance of different marketing techniques and actions for the process of effective management. Background: European societies are ageing rapidly and thus health promotion for older people (HP4... more Background: European societies are ageing rapidly and thus health promotion for older people (HP4OP) is becoming an increasingly relevant issue. Crucial here is not only the clinical aspect of health promotion but also its organisational and institutional dimension. The latter has been relatively neglected in research on HP4OP. This issue is addressed in this study, constituting a part of the EU project ProHealth65+, engaging ten member countries. This paper is based on two intertwining research goals: (1) exploring which institutions/organisations are performing HP4OP activities in selected European countries (including sectors involved, performed roles of these institutions, organisation of those activities); (2) developing an institutional approach to HP4OP. Thus, the paper provides a description of the analytical tools for further research in this area. Methods: The mentioned aims were addressed through the mutual use of two complementary methods: (a) a literature review of scientific and grey literature; and (b) questionnaire survey with selected expert respondents from 10 European countries. The expert respondents, selected by the project's collaborating partners, were asked to fill in a custom designed questionnaire concerning HP4OP institutional aspects. Results: The literature review provided an overview of the organisational arrangements in different HP4OP initiatives. It also enabled the development of functional institutional definitions of health promotion, health promotion activities and interventions, as well as an institutional definition adequate to the health promotion context. The distinctions between sectors were also clarified. The elaborated questionnaires provided in-depth information on countries specifically indicating the key sectors involved in HP4OP in those selected countries. These are: health care, regional/local authorities, NGO's/voluntary institutions. The questionnaire and literature review both resulted in the indication of a significant level of cross-sectorial cooperation in HP4OP. Conclusions: The inclusion of the institutional analysis within the study of HP4OP provides a valuable opportunity to analyse, in a systematic way, good practices in this respect, also in terms of institutional arrangements. A failure to address this aspect in policymaking might potentially cause organisational failure even in evidence-based programmes. This paper frames the perception of this problem.

Systematyczny przegląd literatury został przeprowadzony przy użyciu bazy danych MEDLINE (dostęp poprzez PubMed), jak również Google Scholar w okresie do grudnia 2015 r. Sprawdzono również raporty ze źródeł innych niż medyczne bazy danych oraz piśmiennictwo zawarte w zidentyfikowanych doniesieniach naukowych. Analizie poddano również główne obszary, które wcześniej były przedmiotem studiów w zakresie relacji pomiędzy reklamą a aktywnością fizyczną, paleniem, żywieniem i dietą, jak również wpływ tego przekazu na rynek leków oraz promocję zdrowia.
