Although endometriosis is a disorder commonly found in reproductive-age women, it does not involv... more Although endometriosis is a disorder commonly found in reproductive-age women, it does not involve the bowel very often. The circumferential involvement of the rectum is rare and the obstructive symptoms can be difficult to differentiate from those of inflammatory or malignant diseases. We report two patients with rectal endometriosis whose first prominent symptoms were those of intestinal obstruction. A 26-year-old woman was admitted with obstructive symptoms. In order to alleviate the obstruction and extend the preoperative evaluation, a decompressive colostomy was done. The diagnosis of endometriosis was made by laparoscopy and biopsies of the thickened cul-de-sac peritoneum. Another woman, 40 years of age, was referred to us with a colostomy. She had undergone a laparotomy due to an obstructive acute abdomen a year before, and a frozen pelvis was found. Biopsy specimens had been collected and the pathological report revealed endometrioma. A rectosigmoidectomy, encompassing the stenotic rectal segment, was done along with primary anastomosis. The pathological examination confirmed rectal endometriosis. The conclusion is that, although rare, rectal endometriosis can cause significant stenosis of the organ, leading to obstructive symptoms. Despite its low frequency, it should always be considered in the differential diagnosis of rectal stenosis involving women of childbearing age.

Objective To compare the clinical efficacy of capsule-rupturing versus capsule-preserving ultraso... more Objective To compare the clinical efficacy of capsule-rupturing versus capsule-preserving ultrasound-guided hydrodilatation in patients with shoulder adhesive capsulitis (AC). To determine potential factors affecting the outcome over a 6-month follow-up. Materials and methods Within a 2-year period, 149 consecutive patients with AC were prospectively enrolled and allocated into (i) group-CR, including 39 patients receiving hydrodilatation of the glenohumeral joint (GHJ) with capsular rupture and (ii) group-CP, including 110 patients treated with GHJ hydrodilatation with capsular preservation. Demographics, affected shoulder, and AC grade were recorded. Disabilities of the Arm, Shoulder and Hand (DASH) questionnaire and visual analog scale (VAS) were used for clinical assessment at baseline/1/3/6 months. Comparisons were performed with Mann-Whitney U test and Kolmogorov-Smirnov test. Linear regression was used to identify predictors of outcome. P value < 0.05 defined significance. Results DASH and VAS scores in both groups improved significantly compared to baseline (P < 0.001) and were significantly lower in the CP compared to CR group at all time-points following intervention (P < 0.001). Capsule rupture was a significant predictor of DASH score at all time-points (P < 0.001). DASH scores correlated to initial DASH score at all time-points (P < 0.001). DASH/VAS scores at 1 month were correlated to the AC grade (P = 0.025/0.02). Conclusion GHJ hydrodilatation results in pain elimination and functional improvement till the mid-term in patients with AC, with improved outcome when adopting the capsule-preserving compared to the capsule-rupturing technique. Higher initial DASH score is predictive of impaired functionality in the mid-term.

However, recent reports suggest that up to 40% of patients report persistent symptoms, therefore intervention strategies are needed to improve function and quality of life. One such intervention is the hydrodilatation of the glenohumeral joint with a high-volume injection, comprising of local anaesthetic, corticosteroid and normal saline, making a total volume of up to 40 ml. This service evaluation assessed the outcomes in 28 patients (23 females and 5 males; mean age 54.29 years) with AC who underwent ultrasound-guided (USG) hydrodilatation in a clinical outpatient setting. There was a statistically significant improvement in pain and all shoulder movements (all changes p value < 0.01). No adverse effects were encountered. Our objective was to evaluate the effect of type 2 diabetes (T2D) on the severity of adhesive capsulitis of the shoulder (AC) and on the efficacy of ultrasound guided hydrodistension treatment. Methods We conducted a retrospective longitudinal observational study, of patients with AC who underwent ultrasound guided hydrodistension at our Centre. Severity was measured with DASH (Disabilities of Arm, Shoulder and Hand) score and pain was evaluated with a score between 0 and 10. A sua etiologia resulta de uma sobrecarga crónica desencadeada por desportos com movimento de salto/pliométricos mediante diferentes predisposições intrínsecas e extrínsecas. O tratamento deve ser escolhido de acordo com o estadio da doença e geralmente opta-se inicialmente por uma abordagem não cirúrgica. Um tratamento proposto para tendinopatia é o brisement ecoguiado. Este procedimento envolve a passagem suave de uma agulha várias vezes pelo tendão anormal para transformar um processo degenerativo crónico numa condição aguda com maior probabilidade de cicatrização. É importante estabelecer um protocolo de reabilitação após a técnica. Os autores fazem uma revisão sucinta da literatura e apresentam o resultado de um caso clínico em que uma atleta foi tratada com recurso a esta técnica. Jumper's knee has been defined as painful chronic overuse injury of the extensor mechanism of the knee joint. The etiology is a chronic overload which is triggered by jumping/plyometric movements as well as different intrinsic and extrinsic dispositions. Therapy should be chosen according to the stage of the disease and usually starts with a non-surgical approach. A potential treatment for the tendinopathy is the eco-guided brisement. This procedure involves the smooth passage of a needle through the abnormal tendon several times to transform a chronic degenerative process into an acute condition that is more likely to heal. It's important to have a rehabilitation program postbrisement. The authors made a concise review of the literature and present a clinical case which an athlete was treated using this technique.

Diabetes has an important role in the development of several musculoskeletal disorders, such as a... more Diabetes has an important role in the development of several musculoskeletal disorders, such as adhesive capsulitis of the shoulder (ACs) and stenosing flexor tenosynovitis of the finger (SfTf). The etiopathophysiology of ACs and SfTf in diabetic patients is associated with both chronic hyperglycemia, increased amounts of visceral adiposity and chronic inflammation. Chronic hyperglycemia stimulates the creation of cross-links between collagen molecules, impairing degradation and resulting in the build-up of excessive collagen deposits in the cartilage, ligaments, tendon sheaths and tendons. Increased adipocytes in diabetic patients secrete proteins and cytocines such as TNF-α, IL-6 and IL-13 which result in overproduction of pro-inflammatory factors, destruction of normal tissue architecture and fibrosis.
