Those with acute coronary syndrome (ACS) are at high risk for myocardial infarction (MI) if not treated with dual antiplatelet therapy (DAPT). We sought to assess pre and post-operative outcomes of patients maintained on Clopidogrel and aspirin up to the time of surgery and compare them with those on aspirin alone. Methods From the cardiac surgery database, 240 patients were retrospectively registered between January and May 2017. There were 126 patients with ACS who underwent CABG on DAPT (Clopidogrel group [CG]) and 114 patients who underwent elective CABG on aspirin alone (control). The CG received intraoperative prophylactic platelet transfusion (PPT). Six ecologically distinct zoogeographical regions are targeted in a collection scheme. The ecological and historical biogeography of the present-day distribution of the scorpion fauna of Egypt is discussed.

he present study was performed to evaluate the concentration of some heavy metals in liver, kidne... more he present study was performed to evaluate the concentration of some heavy metals in liver, kidney and muscles of the spiny mouse Acomys cahirinus from natural (Dakhla Oasis) and contaminated (Helwan) areas in Egypt. The copper, manganese and nickel concentrations showed highly significant differences in mice's livers between the two study areas. However, there was no difference between sites in measures of heavy metals in the kidney, except copper, which showed highly significant differences. Iron, manganese and zinc concentrations in muscles were significantly different between sites. The pollution factors showed that the kidney is much affected with copper, iron and zinc pollutants higher than in liver and muscles, while nickel and lead concentrations were lower in kidney than in other organs. In general, the tissues of spiny mouse from Dakhla Oasis were affected by heavy metal pollutants, especially copper, iron and manganese, which may affect wildlife and consequently humans' health.

Objectives: Decreasing in albumin levels are widely used to predict mortality. We evaluate the ov... more Objectives: Decreasing in albumin levels are widely used to predict mortality. We evaluate the overall 28-day mortality prognostic value of percentage changes in albumin levels during first week of admission (%∆ ALB 1wk) in comparison to either albumin level at admission (ALB 1) or albumin level after 1 week of admission (ALB 2) taking into consideration the instability of critically ill patients. Methods: We retrospectively obtained serial measurements of albumin levels of patients admitted to our adult Intensive Care Unit (ICU) between April 2017 and Sep 2018. Patients will be excluded if they discharged or died before 1 week of ICU admission. A receiver operating characteristic (ROC) analysis was conducted to compare the prognostic efficacy of the three tested prognosticators. Results: A total of 163 critically ill patients were finally included in this study. The mean age was 58.37±9.96 years, and 112 (68.71%) were male. The overall 28-day ICU mortality rate was 39.26% (64 patients). The %∆ ALB 1wk was significantly higher in nonsurvivors than in survivors (-11.47%±8.23% vs 0.69%±4.61%; P<0.05). The prognostic value of the % ΔALB 1wk (0.929) was significantly greater than that of ALB 1 and ALB 2 (0.266and 0.655, respectively; P<0.001). Conclusion In comparison to Alb 1 , the %ΔALB 1wk and Alb 2 have a significantly prognostication which is significantly higher in % ΔALB 1wk than ALB 2 with a sensitivity and performance of (93.94% and 75.19% vs 59.59% and 29.91%, respectively; P<0.001). The % Δ ALB 1wk may serve a novel independent prognosticating indicator for critically ill patient's mortality who are taking intravenous human albumin.

Despite a significant number of studies on the taxonomy of small mammals, very little of this wor... more Despite a significant number of studies on the taxonomy of small mammals, very little of this work has been conducted on species in Egypt. External, cranial and dental morphometric analysis of the closely related Gerbillus andersoni, G. campestris and G. amoenus from different ecogeographical regions of Egypt were studied. Statistical analyses of cranial and dental variability allowed us to discriminate three morphological groups which are congruent with the three clusters suggested by previous morphological studies. Higher similarity is observed between G. andersoni populations in the Sinai and Western Mediterranean Coastal Desert despite the fact that they are separated from each other by about 200 km of the Nile Delta. Cluster and principal component analysis, show higher degree of divergence between the subgenus Dipodillus and the other two subgenera Gerbillus and Hendecapleura. This fact suggests that the morphometric differences observed among species within the genus Gerbillus are not mainly related to its phylogeny.

The study investigates the occurrence of age-related allometric changes in a number of cranial an... more The study investigates the occurrence of age-related allometric changes in a number of cranial and dental measurements of the Egyptian wolf Canis anthus lupaster from different areas of Egypt.The age structure of a sample of 55 specimens of both sexes of C.a. lupaster was investigated using counting dentine layers on longitudinally sanded canine roots.The results showed that most of the wolves were between one and three years of age. The oldest individual was 11 years old. A set of isometric cranial and dental characters that qualify as diagnostic characters for analyzing interaspecific, morphological differentiation among populations of this and other closely related canids are identified.

Background: Amino acid polymorphisms in the mitochondrial cytochrome b (cytb) gene of four Gerbil... more Background: Amino acid polymorphisms in the mitochondrial cytochrome b (cytb) gene of four Gerbillus species have been investigated for their geographical distribution and possible functional significance. The sequences were obtained from a total of 20 specimens representing four species of genus Gerbillus collected from Siwa Oasis, Dabaa, Wadi El Natron, El Faiyum, and Baltim in Egypt. Results: Our results identified a group of amino acid variant polymorphisms that were useful for both species taxonomic and biogeographic assignments. The results demonstrated that amino acid variants L>F173 (Leucine>Phenylalanine), A>M203 (Alanine>Methionine), and I>V221(Isoleucine>Valine) were specific to G. andersoni, while the variant V>M283 (Valine>Methionine) was only specific to G. andersoni from Baltim. The variants, L>P263 (Leucine>Proline) and M>T311 (Methionine>Threonine) were specific only to G. amoenus collected from El Faiyum. Compared to other amino acid variants, L>P263 was remarkably less frequent, and it was predicted using PROVEAN database tool to have non-neutral effects. Conclusion: Amino acid polymorphisms within the cytochrome b gene could be assigned to specific geographic locations. They might prove suitable to track accumulated and recent environmental changes as they could represent signs of adaptive evolution.

P-WAB-1 is a clinical linguistic measuring tool to determine severity and type of aphasia in brain damaged patients based on Aphasia Quotient (AQ) as a functional measure. For the purposes of a quick clinical screening of aphasia in Persian, we adapted the bedside version of WAB-R to assess the performance of Persian aphasic patients. The data we reported on adaptation, validity and reliability of P-WAB-1 are based on faithful translation and criterion validity ratio (CVR) taken from the expert panel and the performance of 60 consecutive brain damaged patients referred to different university clinics for rehabilitation and 30 healthy subjects as norms and 40 age-matched epileptic patients as the control group. Background: We report herein a case of fecal microbiota transplantation (FMT) used for severe Clo... more Background: We report herein a case of fecal microbiota transplantation (FMT) used for severe Clostridium difficile infection for a 65-year-old Lebanese man who underwent left ventricular assist device implantation. To the best of our knowledge this is the first case report from Lebanon and the region presenting such technique. Case presentation: The patient experienced diarrhea and rectal bleeding and was diagnosed of pseudomembranous colitis (PMC). His condition failed to improve on maximal pharmacological therapy. Protocolectomy, an invasive operation consisting in resection of the entire colon and rectum seemed to be the last resort before the patient responded to FMT given through gastroscopy. Conclusion: Despite the increasing experience with FMT for C. difficile infection, published evidence in severe related cases from this region is very limited. Hence, we promote adjunctive FMT, an effective noninvasive method, to be considered as a promising early treatment option in severe C. difficile infection.

Materials and methods:we examine a museum sample of 179 specimens from different parts of Egypt to determine age structure using dentine layers counting method. Specimens were grouped into three population groups according to their geographical region of origin. Results:the average age of foxes in the sample was 1.46 years and ranging from 1 to 7 years of age. Differences in age structure of populations from the three geographical regions and differences between sexes are presented and discussed.

Background: We report herein a case of fecal microbiota transplantation (FMT) used for severe Clo... more Background: We report herein a case of fecal microbiota transplantation (FMT) used for severe Clostridium difficile infection for a 65-year-old Lebanese man who underwent left ventricular assist device implantation. To the best of our knowledge this is the first case report from Lebanon and the region presenting such technique. Case presentation: The patient experienced diarrhea and rectal bleeding and was diagnosed of pseudomembranous colitis (PMC). His condition failed to improve on maximal pharmacological therapy. Protocolectomy, an invasive operation consisting in resection of the entire colon and rectum seemed to be the last resort before the patient responded to FMT given through gastroscopy. Conclusion: Despite the increasing experience with FMT for C. difficile infection, published evidence in severe related cases from this region is very limited. Hence, we promote adjunctive FMT, an effective noninvasive method, to be considered as a promising early treatment option in severe C. difficile infection.

Materials and methods:we examine a museum sample of 179 specimens from different parts of Egypt to determine age structure using dentine layers counting method. Specimens were grouped into three population groups according to their geographical region of origin. Results:the average age of foxes in the sample was 1.46 years and ranging from 1 to 7 years of age. Differences in age structure of populations from the three geographical regions and differences between sexes are presented and discussed.
