Routine prophylactic oophorectomy for postmenopausal women at abdominal hysterectomy is standard ... more Routine prophylactic oophorectomy for postmenopausal women at abdominal hysterectomy is standard practice but, unfortunately, not when hysterectomy is performed by the vaginal route because the surgeon is apprehensive about technical difficulties. Gynecologists today are stretching themselves to add three or four abdominal punctures to perform laparoscopic oophorectomy or salpingo-oophorectomy to assist vaginal hysterectomy but are not willing to learn an additional step to perform oophorectomy or salpingo-oophorectomy at vaginal hysterectomy. Based on extensive personal experience, we present a surgical technique of vaginal oophorectomy at vaginal hysterectomy, in the hope that more vaginal surgeons will master this simple surgical step. Objective: To determine whether hysterectomy by the vaginal route is safe and feasible in patient... more Objective: To determine whether hysterectomy by the vaginal route is safe and feasible in patients with previous cesarean section. Methouk A retrospective study of the records of private and public hospital patients who underwent vaginal hysterectomy performed by the senior author. Two hundred twenty patients who had had previous cesarean sections were compared with a control group of 200 patients who had not had previous pelvic surgery, with special reference to operative difficulties, intraoperative complications, surgical time and length of hospital stay. Results: It was possible to perform vaginal hysterectomy safely in patients with previous cesarean sections. Three of 200 (1 .S%) patients had inadvertent intraoperative urological trauma because of dense adhesions. Conclusions: The vaginal route is the route of choice for performing a hysterectomy in patients with previous cesarean section.

This is the largest series published in the literature. The advantages of the vaginal route in performing hysterectomies in these patients is discussed and the technique is described.
