Objective: Dengue cases in pregnancy have high morbidity and mortality. More so if it leads to im... more Objective: Dengue cases in pregnancy have high morbidity and mortality. More so if it leads to immune thrombocytopenic purpura which causes a drastic decrease in platelet, increasing chances of bleeding and mortality and pregnancy itself being a state of hemodynamic instability. Case report: Here, we present a case of dengue causing secondary immune thrombocytopenia. Managing these cases is challenging and need a multidisciplinary approach and should be done at a higher center. In previous reports, thrombocytopenia in such cases responded to steroids or IVIG. But in our case patient did not respond to either of them but to Romiplostim. There are only a few studies on the use of Romiplostim in dengue and dengue induced ITP and more study is required. Conclusion: Dengue induced persistent thrombocytopenia is rare but should always be kept in mind in managing these cases.

Although uterine inversion is a potentially life-threatening complication of childbirth, there ar... more Although uterine inversion is a potentially life-threatening complication of childbirth, there are only six case reports of cardiac arrest due to acute inversion to date. We report a successful outcome in a patient with sickle cell disease who had a witnessed cardiac arrest due to acute puerperal uterine inversion. Revival from cardiac arrest and resuscitation was followed by manual repositioning of the uterus. She gradually recovered from the acute kidney injury that developed as sequelae of the cardiac arrest and subsequent circulatory shock. Awareness of this rare complication and timely intervention is the key to a successful outcome in these patients.

Aims: Dengue fever is a major health problem with high morbidity and mortality especially during ... more Aims: Dengue fever is a major health problem with high morbidity and mortality especially during epidemic season; pregnant females being no exception. But, there is paucity of published data on dengue fever during pregnancy. Hence, this study was planned to study the clinical profile, maternal outcome and predictors of poor outcome in pregnant dengue patients. Materials and Methods: All pregnant females attending labour room of Tata Main Hospital, Jamshedpur from April 2016 to October2020 with acute febrile illness caused by dengue virus at any gestational age were included in the study. Diagnosis of dengue was made by detection of NS1 antigen or dengue serology. A predesigned proforma was used to record materno-foetal outcomes and were analysed. Results: Dengue was the cause of fever in 7.1% febrile patients. Maternal complications included abortions (26%), abruptio (1.9%), postpartum haemorrhage (11.9%) . Of all the pregnant dengue patients, five had severe dengue(SD) with high mortality (3/5; 60%). Fetal complications were intrauterine death(7.7%),preterm(42.3%). Thrombocytopenia and elevated transaminases were associated with adverse outcome. Conclusions: Dengue fever in pregnancy is associated with poor outcomes more in cases of SD rather than dengue fever . Pregnant females with high risk predictors should be identified and managed aggressively in intensive care units to improve outcomes.

We report a case of torsion in the uterus didelphys at term which is rare and a lifetime experience for an obstetrician. The patient, a 25-year-old gravida 2 para 1 was admitted to the labor ward at 37 weeks and six days of gestation with abdominal pain. Her previous delivery was a caesarean section performed four years back. She was taken to the operating room for an emergency caesarean section for fetal distress and the lie was transverse. On entering the peritoneal cavity, we found an engorged infundibulopelvic ligament with the fallopian tube and ovary covering the lower segment of the uterus. The baby was successfully delivered by breech extraction. Due to uterine torsion of more than 180 degrees, the posterior surface of the uterus was placed anteriorly, and the incision was made on the posterior surface of the uterus. There was a hemi uterus on the left side of the pelvic cavity with the fallopian tube and ovary attached to it; a diagnosis of uterus didelphys was made. The diagnosis of uterine torsion is intraoperative and prompt and timely decision by surgeons is crucial. We had favorable maternal and fetal outcomes in this rare and interesting case. The diagnosis, though rare, should be kept in mind in all cases of abdominal pain during pregnancy, especially in those with malpresentation.

In these days of fast travel, it should not take 23 years for Ritchie Russell's work to cross the Atlantic. All in all, a very uneven book, likely to confuse rather than educate.

Objectives: To assess the role of mid-trimester transvaginal sonographic assessment of cervix in ... more Objectives: To assess the role of mid-trimester transvaginal sonographic assessment of cervix in predicting the risk of primary cesarean section. Methods: Hundred pregnant women, who attended outpatient department of Tata Main Hospital for ante natal examination, from 1 st December 2011 to 1 st December 2012, were taken up for the study. Only those who had confirmed dates were included in the study. Transvaginal sonography was done at 18-26 week to measure cervical length. Parameters studied were gestational age at delivery, whether spontaneous or induced, duration of labor, pregnancy outcome and mode of delivery and indication for cesarean section. Results: Majority of LSCS were in the group with cervical length ≥4 cm (57.1%) with "p" value of <0.0001. Only 13.8% women who had vaginal delivery had cervical length ≥4 cm. Out of the 32 women with cervical length ≥4 cm, 26 (81.3%) required induction of labor and 10 (31.3%) delivered after 40 weeks. Mean cervical length for spontaneous onset of labor was 3.11 ± 0.85 cm and for induction of labor was 4.36 ± 1.11 cm ("p" value ≤ 0.001). Multiple logistic regression analysis was performed to study outcome variable of mode of delivery. Cervical length and induction to delivery interval were found to be independent predictor of mode of delivery. In the receiver operating characteristic (ROC) curve analysis, the best cutoff point for prediction of primary cesarean section was 40 mm for cervical length. Sensitivity, specificity, positive predictive value and negative predictive value of cervical length as a predictor of mode of delivery was 54.8%, 91.4%, 82.1% and 73.6% respectively. R 2 (coefficient of determination) was 0.271. Conclusion: Transvaginal cervical length measurement at mid trimester can be used as a predictive tool to determine the risk of primary cesarean section as well as the need of induction of labor. * Capsule: This study assesses the efficacy of transvaginal ultrasound measurement of cervical length as predictor of primary caesarean section. # Corresponding author.

A large number of patients tend to be asymptomatic. Ovarian filariasis is an extremely rare manifestation of lymphatic filariasis. This is a case report of bilateral ovarian filariasis presenting as ovarian mass with associated lower abdominal pain, weight loss and chyluria. This is a very rare diagnosis, more so as it was diagnosed preoperatively by ultrasound and managed with anti-filarial drugs and confirmed by biopsy. Most cases of ovarian filariasis reported in literature are incidental diagnosis on histopathological examination of postoperative specimen.

There has been a consistent rise in ectopic pregnancies due to increase in maternal age, infertil... more There has been a consistent rise in ectopic pregnancies due to increase in maternal age, infertility treatment, cesarean sections, smoking among women, and use of contraceptives. With the extratubal ectopic pregnancies accounting for only 4% of the total ectopic pregnancies, scar pregnancies are even more of a rare entity with their incidence being less than 1%. We hereby present two cases of ectopic pregnancies, which though managed successfully presented a huge diagnostic challenge in the first case while the diagnosis was completely missed in the second case, hence, emphasizing the need for ectopic mindedness when dealing with early pregnancies.

The thalassemias are the most common single-gene disorders of hemoglobin synthesis. The salient f... more The thalassemias are the most common single-gene disorders of hemoglobin synthesis. The salient features of beta thalassemia major, in which both alleles of the HBB gene are affected, are transfusion dependency and iron overload. Although with advances in treatment, the life expectancy in such patients has increased, they have difficulty in conceiving. We report a case of pregnancy in a beta thalassemia major patient who was transfusion independent and had no iron overload. Genetic analysis revealed IVS 1-5 (G-C) mutation in the homozygous state which usually manifests in severe disease and blood transfusion dependency. On the contrary, she did not need blood transfusion, had a smooth antenatal period and a vaginal delivery at term with a favorable childbirth experience. This case report highlights complex genetic interplay and the role of fetal hemoglobin (HbF) enhancer loci which modulates HbF levels thereby raising total hemoglobin levels.
