Maternal age is related to a higher risk of adverse maternal, fetal, and neonatal outcomes in twi... more Maternal age is related to a higher risk of adverse maternal, fetal, and neonatal outcomes in twin pregnancies. However, whether this increase in adverse outcomes is due solely to age or due to risk factors that are more common in women over 40 remains unknown. The aim of this study is to assess if maternal age over 40 years old is an independent risk factor for obstetric adverse outcomes in dichorionic diamniotic twin gestations. In this single-center retrospective cohort study, we compared the obstetric outcomes of women with dichorionic diamniotic twin pregnancies below and over 40 years of age. A twin pregnancy cohort enrolled between 2013 and 2019 was included in the study. Maternal, fetal, and labor complications were recorded. A total of 510 women were analyzed in two groups: 266 women below 40 years old and 244 women over 40 years old. Transperineal and transabdominal ultrasound was independently performed by a trained midwife and an obstetrician. There is a scarcity of literature about Previable Premature Rupture of Membranes in Down Syndrome... more There is a scarcity of literature about Previable Premature Rupture of Membranes in Down Syndrome (DS) Pregnancies. The present report concerns the difficulty in prenatal counselling in a Previable Premature Rupture of Membranes in a DS Pregnancy. The outcome appears improved in our case. We report the case of a DS Pregnancy with severe oligohydramnios since the 17th week of gestational age due to Previable Premature Rupture of Membranes. She delivered at the gestational age of 33 weeks and 3 days a healthy DS baby without signs or symptoms of neonatal pulmonary hypoplasia, skeletal abnormalities or infectious morbidity. In Previable Premature Rupture of Membranes, foetal death is common. When a viable gestational age is reached, respiratory complications and other morbidities such as sepsis are frequent. Newborns with DS have an increased risk of respiratory tract infections, what added to the risk of congenital diseases, made us believe in a poor prognosis. More reports are needed in order to provide a better prenatal counselling.

Background: Intrapartum ultrasound (ITU) techniques have been described only in maternal semi-rec... more Background: Intrapartum ultrasound (ITU) techniques have been described only in maternal semi-recumbent position. However, other birthing positions, such as side-lying, are more comfortable for many mothers. The present study aimed to test the hypothesis that ITU measurements can be obtained in maternal lateral positions during the second stage of labor. Methods: A single-center, prospective observational study was conducted. Women who preferred a side-lying position during the second stage of labor participated in the study. The women were asked to grasp their upper knee and flex it to perform the ultrasound scan. The transducer was first placed in the frontal plane and then rotated to the midsagittal plane (the probe was placed at the level of the fourchette). Results: 34 women participated in the study. Position of the fetal head, angle of progression, and head-perineum distance with and without contraction were obtained in all women. There was a moderate correlation between the head-perineum distance with contraction and the ITU to birth time (r = 0.466; p = 0.009). Conclusions: ITU in maternal lateral position (side-lying) is a simple technique that avoids unnecessary changing of position during the second stage of labor.

Each obstetrician performed eight vacuum assisted deliveries using the Bird technique and eight vacuum assisted deliveries using the Vacca 5-Steps technique.ResultsA total of 96 vacuum assisted deliveries were performed. The mean distance from the centre of the cup to the flexion point was 1.78±0.96 cm for the Bird technique and 1.05±0.60 cm for the Vacca technique (p<0.001). The lateral distance (over the parietal bone) was 1.16±0.69 cm for the Bird technique and 0.66±0.52 cm for the Vacca technique (p<0.001). The vertical distance (towards the frontal or occipital bone) was 1.12±1.02 cm for the Bird technique and 0.67±0.55 cm for the Vacca technique (p=0.009). Se presenta el caso clinico de una mujer de 29 anos que comenzo en semana 34 con un cuadro de dolor lo-calizado en cadera derecha con impotencia funcional e inestabilidad en la bipedestacion y dificultad para la marcha. Tras finalizar su gestacion en semana 39 con un parto eutocico sin complicaciones, se incremento el cuadro doloroso durante el puerperio precoz, con bloqueo de rotacion interna y externa de miembro inferior derecho a la exploracion. En la radiografia y en la resonancia magnetica se evidencio una fractura intracapsular desplazada de femur derecho, que preciso correccion quirurgica. A pesar del tratamiento qui-rurgico, el cuadro empeoro, localizandose posteriormente tambien en el miembro contralateral. Female sexual dysfunction (FSD) includes different disorders that are highly prevalent in perimen... more Female sexual dysfunction (FSD) includes different disorders that are highly prevalent in perimenopausal and older women which require precise diagnosis and management. Screening of FSD should be based on clinical history and the use of screening tools such as the 14-item Changes in Sexual Functioning Questionnaire, the 19-item Female Sexual Function Index (FSFI-19) and its short version (FSFI-6), or the Decreased Sexual Desire Screener. The clinical exam should include the assessment of menopausal symptoms, vulvovaginal atrophy, and comorbidity including pelvic floor disorders, endocrine disorders, depressive symptoms, cancer diseases, and chronic medication use that may interfere with sexuality. The management of genito-pelvic pain/penetration disorders includes the appropriate treatment of the genitourinary syndrome of menopause, urinary incontinence, and pelvic floor disorders with appropriate selection for surgery and/or other treatments. The management of female sexual interest/arousal disorder (also known as hypoactive sexual desire disorder) may include systemic or topical steroid hormone therapy, including dehydroepiandrosterone sulfate, central-acting agents (flibanserin, bupropion, bremelanotide), and/or natural remedies (Tribulus terrestris, Trigonella foenum-graecum). Female orgasmic disorder may be managed by conductual methods and/or drugs.

A system for tracking multiple objects in an image is typically composed of a chain form of an object detection algorithm and an object tracking algorithm. However, chain-type systems composed of several modules require a high performance computing environment and have limitations in their application to actual applications. In this paper, we propose a method that enables real-time operation in low-performance computing environment by adjusting the computational process of object detection module in the object detection-tracking chain type system.

Doppler ultrasound cardiotocography is a non‐invasive alternative that, despite its poor specific... more Doppler ultrasound cardiotocography is a non‐invasive alternative that, despite its poor specificity, is often first choice for intrapartum monitoring. Doppler ultrasound suffers from signal loss due to fetal movements and is negatively correlated with maternal body mass index (BMI). Reported accuracy of fetal heart rate monitoring by Doppler ultrasound varies between 10.6 and 14.3 bpm and reliability between 62.4% and 73%. The fetal scalp electrode (FSE) is considered the reference standard for fetal monitoring but can only be applied after membranes have ruptured with sufficient cervical dilatation and is sometimes contra‐indicated. A non‐invasive alternative that overcomes the shortcomings of Doppler ultrasound, providing reliable information on fetal heart rate, could be the answer. Non‐invasive fetal electrocardiography (NI‐fECG) uses a wireless electrode patch on the maternal abdomen to obtain both fetal and maternal heart rate signals as well as an electrohysterogram. We aimed to validate a wireless NI‐fECG device for intrapartum monitoring in term singleton pregnancies, by comparison with the FSE.
