Desenvolveram-se estudos sobre flebotomíneos em área de leishmaniose tegumentar, fazenda Boa Sort... more Desenvolveram-se estudos sobre flebotomíneos em área de leishmaniose tegumentar, fazenda Boa Sorte, Município de Corguinho, Mato Grosso do Sul, Brasil, com vistas a incriminar vetor dessa parasitose. No início dos estudos, encontravam-se bem preservados vários tipos da cobertura vegetal primitiva, com predomínio de cerrado e cerradão, denominado localmente de "croa". Decorridos quatro meses, parte significativa da "croa" e do cerrado foi queimada para transformação em áreas de pastagens. Durante julho/1991 a junho/93, realizaram-se coletas semanais das 18:00 às 6:00 horas, com armadilha CDC (Center on Disease Control), em floresta-galeria, floresta de encostas, cerrado, "croa", peridomicílio (chiqueiro e poleiro) e no interior de uma tulha; coletas mensais com armadilha de Shannon das 18:00 às 24:00 horas em floresta-galeria e "croa". Este trabalho, realizado a partir de outubro de 2000 até janeiro de 2003, descreve as característ... more Este trabalho, realizado a partir de outubro de 2000 até janeiro de 2003, descreve as características clínico-epidemiológicas da leishmaniose visceral em Três Lagoas, Mato Grosso do Sul. Foram confirmados 149 casos da doença, com predominância do sexo masculino (71,1%). A faixa etária mais acometida foi de 0 a 4 anos (42%). Quanto ao quadro clínico, estiveram presente febre (97,3%), esplenomegalia (85,9%) e anemia (75,8%). Em 32 (21,5%) pacientes ocorreu infecção associada, predominando a pneumonia. Sobre as alterações laboratoriais observou-se mediana de hemoglobina de 8mg/dl e de leucócitos de 3.100 cel/mm 3 . O esfregaço de medula foi positivo em 90,6%. Dos 97,9% de pacientes tratados, 78,2% utilizaram antimoniato pentavalente. Ocorreram 8% de óbitos, metade deles portadores de infecção associada. O conjunto destes dados sugere mudanças no padrão fisiográfico de ocorrência da leishmaniose visceral na localidade, com expansão e urbanização da doença, necessitando atenção para o diagnóstico e tratamento precoces. Leishmania infantum is the etiologic agent of visceral leishmaniasis (VL) in the Americas, Medite... more Leishmania infantum is the etiologic agent of visceral leishmaniasis (VL) in the Americas, Mediterranean basin and West and Central Asia. Although the geographic structure of L. infantum populations from the Old World have been described, few studies have addressed the population structure of this parasite in the Neotropical region. We employed 14 microsatellites to analyze the population structure of the L. infantum strains isolated from humans and dogs from most of the Brazilian states endemic for VL and from Paraguay. The results indicate a low genetic diversity, high inbreeding estimates and a depletion of heterozygotes, which together indicate a predominantly clonal breeding system, but signs of sexual events are also present. Three populations were identified from the clustering analysis, and they were well supported by F statistics inferences and partially corroborated by distance-based. POP1 (111 strains) was observed in all but one endemic area. POP2 (31 strains) is also well-dispersed, but it was the predominant population in Mato Grosso (MT). POP3 (31 strains) was less dispersed, and it was observed primarily in Mato Grosso do Sul (MS). Strains originated from an outbreak of canine VL in Southern Brazil were grouped in POP1 with those from Paraguay, which corroborates the hypothesis of dispersal from Northeastern Argentina and Paraguay. The distribution of VL in MS seems to follow the west-east construction of the Bolivia-Brazil pipeline from Corumbá municipality. This may have resulted in a strong association of POP3 and Lutzomyia cruzi, which is the main VL vector in Corumbá, and a dispersion of this population in this region that was shaped by human interference. This vector also occurs in MT and may influence the structure of POP2. This paper presents significant advances in the understanding of the population structure of L. infantum in Brazil and its association with eco-epidemiological aspects of VL.

The aim of this study was to evaluate the sensitivity and specificity of polymerase chain reactio... more The aim of this study was to evaluate the sensitivity and specificity of polymerase chain reaction (PCR) in the detection of Leishmania DNA in archived Giemsa-stained bone marrow slides for diagnosis of visceral leishmaniasis (VL), and to compare PCR with conventional diagnostic techniques, like direct microscopy and parasite culture. Specimens of archived Giemsa-stained bone marrow slides from 91 patients with VL and from 79 controls with other diseases or conditions were studied. PCR showed the highest sensitivity (92.3%) and had good specificity (97.5%). Direct examination detected 79.1% and culture 59% of positive samples. In addition, PCR was able to detect VL in 16 of 19 patients (84.2%) with negative microscopy. PCR in Giemsa-stained bone marrow slides is a suitable tool for confirming diagnosis in patients with VL and may be useful in the diagnosis of difficult cases. Slide smears are easily stored, do not require special storage conditions such as low temperatures, and can be easily mailed to centers where PCR is available, making it an excellent option for diagnosis in the field.

The outspread and urbanization of visceral leishmaniasis (VL) in Campo Grande, state of Mato Gros... more The outspread and urbanization of visceral leishmaniasis (VL) in Campo Grande, state of Mato Grosso do Sul, lead us to undertake the present study over diversity and abundance of sand flies in the urban area to compare with previous search carried out during 1999/2000, before the identification of the disease in the human population.The captures were carried out with automatic light traps, weekly, from February 2004 to February 2005 on three sites including a forested area (Zé Pereira), two peridomicilies (shelters of domestic animals and cultivation areas), and intradomicilie. In the present study 110 collections were obtained during 13 months for 1320 h of collections, resulting in 5004 specimens, 3649 males and 1355 females belonging to the 20 following species: Brumptomyia avellari, Brumptomyia sp., Bichromomyia flaviscutellata, Evandromyia lenti, E. termitophila, E. cortelezzii, E. borrouli, Lutzomyia sp., L. longipalpis, Micropygomyia quinquefer, N. antunesi, N. whitmani, Pintomyia christenseni, Pi. damascenoi, Psathyromyia aragaoi, Ps. campograndensis, Ps. hermanlenti, Ps. shannoni, Pychodopygus claustrei, and Sciopemyia sordellii. L. longipalpis was the most abundant species in the anthropic environment with 92.22% of the captures. This shows an increase of sixty times in the density of L. longipalpis compared to the last sand fly evaluation in 1999/2000. The high density of L. longipalpis in Campo Grande is the main factor of risk in transmission of the disease to human in the urban area. The capture of N. antunesi, typical specie from Amazonian region, in Mato Grosso do Sul is reported for the first time.

Aspects of phlebotomine behavior were investigated in the city of Campo Grande, Mato Grosso do Su... more Aspects of phlebotomine behavior were investigated in the city of Campo Grande, Mato Grosso do Sul state. The insects were captured weekly during December 2003 to November 2005, with Centers for Disease Control light traps at seven different sites including forests and residential areas. In total, 11,024 specimens (7,805 males and 3,219 females) were collected, from which 9,963 (90.38%) were identified as Lutzomyia longipalpis, the proven vector of American visceral leishmaniasis agent. The remaining 9.62% comprised 21 species. L. longipalpis was the most frequent species in all sampled sites, and the first in the ranking of standardized species abundance index. In residential areas this species clearly predominated in the peridomicile (90.96%), in contrast to the intradomicile (9.04%); in animal shelters, it was more numerous in hen houses and prevailed at ground level, inside, and at forest edge around the residences; this aspect is worrying because this insect may remain sheltered in forested environments during the use of insecticides in homes. In the forest environment, other probable or proven vector of cutaneous leishmaniasis agents were also captured such as Lutzomyia whitmani (=Nyssomyia whitmani, sensu Galati), Lutzomyia antunesi (=Nyssomyia antunesi, sensu Galati), and Lutzomyia flaviscutellata (=Bichromomyia flaviscutellata, sensu Galati).

Leishmaniasis, a zoonotic disease caused by parasites of the genus Leishmania, has expanded beyon... more Leishmaniasis, a zoonotic disease caused by parasites of the genus Leishmania, has expanded beyond its natural range and is becoming increasingly urban. Using PCR and PCR-RFLP, we detected Leishmania (Viannia) braziliensis in two bats (Chiroptera) in Mato Grosso do Sul, Brazil, an endemic area. This is the first record of L. (V.) braziliensis in bats. It is also the first record of any Leishmania sp. in bats in the state. The animals testing positive were found in both a rural site and an urban site. These results indicate the need for further research into the viability of Leishmania in bats and could potentially have implications for public health in Mato Grosso do Sul, given the large populations of urban bats, their mobility, and their ability to roost at close proximity to humans within residences and other buildings.

Sand flies were captured weekly with CDC light traps from December 2003 to November 2005 in three... more Sand flies were captured weekly with CDC light traps from December 2003 to November 2005 in three areas of Campo Grande, in the Brazilian state of Mato Grosso do Sul. These areas incorporated two patches of remnant forest and five houses. The blood meals of engorged female sand flies were identified using the avidin-biotin system of immunoenzymatic ELISA capture. Most (327/355) of the females analysed were Lutzomyia longipalpis, of which 66.4% reacted with human blood, 64.8% with that of birds and 8.9% with that of dogs. Females that had taken human blood predominated in the residential areas and two forest patches. The following combinations of blood were also detected for L. longipalpis in some of the samples analysed: bird + human (43.4%), bird + human + dog (6.1%). The combination bird + human + dog + pig was also found for Nyssomyia whitmani. Dogs and pigs appear to have little attractiveness for L. longipalpis. The aim of the study was to detect asymptomatic infection by Leishmania sp. in blood donors. Seru... more The aim of the study was to detect asymptomatic infection by Leishmania sp. in blood donors. Serum samples (430) were tested by Immunofluorescent Antibody Test, and an interview with the blood donors was carried out. Antibodies were detected in 15.6% of samples. The variables associated with the infection were: origin of the donor, presence of builds, parks or squares, sick dog in the neighborhood, and neighboring with leishmaniasis. It was observed association between origin of donors and the presence of sick dog. It is important a careful screening of donors, due to the risk of infection through blood transfusion.
