It seems reasonable that detecting ILD in its early, preclinical stage could improve outcomes and prolong survival given that these new treatments target multiple pathways that are involved in fibroproliferation in the lungs. CT is one of the main tools in the diagnosis of ILD, having a high positive predictive value of 94-97% [5]. Hallmarks of ILD on CT scans are traction bronchiectasis, reticular markings, and honeycombing [6]. Because there is a well-established relation between cigarette smoking and ILD, it is to be expected that ILD would be more frequently identified in older smokers undergoing CT screening for lung cancer. We wanted to determine the prevalence of ILD in a cohort undergoing low-dose CT screening for lung cancer, identify the most common patterns of ILD in this population of smokers, and determine factors predictive of the likelihood of disease progression.
