A repar...

The present study reports the clinical case of a patient with self-induced traumatic injury in the trochanter region, treated with HA 0.2% coverage combined with LPL. This study is part of an experimental research project coordinated by teachers and nurses who work on the development of new protocols for wound treatment. The patient D.M.M.N. was 74 years old, hypertensive, diabetic, with grade I obesity, with diagnosis of chronic ulcerative lesion in the left lower limb trochanter region due to intense itching caused by skin dryness. Initially, a wound bed preparation protocol with the use of hydrogel and conservative instrumental debridement was recommended, followed by a second protocol with weekly LPL application, combined with daily topical administration of HA 0.2%. The analysis of the effects of the treatments was performed by means of photographic record and the images were analyzed with the aid of the software ImageJ. The dressings were performed daily and, in 49 days, the total healing of the lesion was observed. No discomfort or complications due to the use of the products and LPL were observed. It was concluded that the proposed therapy presented good tolerability and therapeutic ef cacy for this particular case. Descriptors: wounds and injuries; low-level light therapy; hyaluronic acid; wound healing; wound closure techniques.

Vector-Borne and Zoonotic Diseases, 2020

Bartonella spp. are emerging and neglected bacteria. A large spectrum of clinical manifestations has been linked to bartonella bloodstream infection in the last two decades that can cause fatal outcomes, especially in immunodeficient patients. The goal of this study was to evaluate the prevalence of bartonella infection in SCD patients. Materials and Methods: We evaluated Bartonella spp. prevalence in 107 SCD patients. Blood samples and enrichment blood cultures were analyzed by molecular detection of Bartonella spp. DNA. Bartonella DNA was amplified using conventional genus-specific Bartonella PCR which amplifies the Intergenic Transcribed Spacer region and Bartonella henselae-specific nested PCR which amplifies the FtsZ gene. Positive patient DNAs were tested with ssrA conventional PCR. All amplicons were sequenced. Findings: Ten of 107 patients tested positive for B. henselae infection in at least one molecular test. All obtained amplicons were sequenced and similar to B. henselae sequences deposited in GenBank (accession number BX897699). Based on statistical results, bloodstream infection with B. henselae was not associated with animal contact or blood transfusions. Conclusion: We detected B. henselae DNA in 10 (9.3%) SCD studied patients. These patients were notified and treatment was offered to them.

Coffee is widely consumed and its oil contains essential fatty acids. We evaluated the local (skin) and systemic effects associated with the topical use of coffee oils in rats. Punch skin wounds (6 mm) incisions were generated on the backs of 75 rats. Saline (SS), mineral oil (MO), green coffee oil (GCO), roasted coffee oil (RCO), green coffee ground oil (GCGO) or roasted coffee ground oil (RCGO) were topically applied to the wounds. Healing was evaluated by visual and histological/morphometric optical microscopy examination; second harmonics generation (SHG) microscopy, wound tissue q-PCR (values in fold-change) and blood serum (ELISA, values in pg/mL). RCO treated animals presented faster wound healing (0.986 vs. 0.422), higher mRNA expression of IGF-1 (2.78 vs. 1.00, p = 0.01), IL-6 (10.72 vs. 1.00, p = 0.001) and IL-23 (4.10 vs. 1.2, p = 0.05) in early stages of wound healing; higher IL-12 (3.32 vs. 1.00, p = 0.05) in the later stages; and low...

Bartonella spp. are causative agents of cat-scratch disease, endocarditis, bacillary angiomatosis, bacillary peliosis, trench fever, neurocognitive dysfunction and regional pain syndrome (2). Chronic Bartonella spp. infection and subsequent manifestations of persistent infection may be the result of the bacteria growing in biofilms. In humans, at least four Bartonella species have been associated with infectious endocarditis, a biofilmassociated infectious disease (3). Biofilms are sessile communities of interacting unicellular organisms that adhere to a surface and to each other and create unique, complex structures, covered with a protective layer of extracellular polymeric substances (EPS). Biofilms and their inherent resistance to antimicrobial agents are often key players in many persistent and chronic bacterial infections (4, 5). We acquired skin biopsies from 5 patients with blood PCR-positive B. henselae. 60-100 micron-thick sections were immunostained with antibodies to B. henselae. Stained sections were imaged using both single-and multi-photon imaging to capture epifluorescence emission and second harmonic generation signal of non-stained collagen I and II (Fig 1). The crystalline triple-helix of native fibrillar collagen (I, II, III and V) generates a strong second harmonic generation (SHG) signal (6). In patients with longstanding bartonellosis, clusters of immuno-reactive B. henselae were found intimately associated with fibrillar dermal collagen (Fig 2). Additionally, we found a Bartonella spp. biofilm on a peripheral intravenous catheter line removed from a patient with persistent Bartonella (Fig 3). Further, B. henselae, cultured 14 or 21 days, on collagen I and II-coated coverslips revealed a B. henselae biofilm matrix (Fig 4). Identification and characterization of Bartonella spp. biofilms in vivo and in situ will lead to a better understanding of persistent Bartonellosis and provide direction in treatment modalities.
