IPRO is the federally designated Medicare review organization in New York State. The CHF Management Project focused on (1) documentation of left ventricular function (LVF), (2) use of ACE Inhibitor/Angiotensin Receptor Blocker (ACEI/ARB), and (3) patient education. National studies reveal a woeful deficiency in these areas.

One tissue-bank recovered tissue from donors who did not meet eligibility requirements; in addition, blood submitted for screening had not come from the persons from whom the linked tissues had been obtained. At the other tissue bank there was concern regarding blood-screening practices and manufacturing practices. Recipients were advised to be tested for blood-borne viruses (BBV) and syphilis. Objective: To describe: 1) what infectious agents have been transmitted by musculoskeletal tissues, 2) how implicated tissue was processed and stored, 3) survival of BBV under different conditions, 4) risks associated with implanted tissue stored at room temperature for 30 days and/or processed using sporicidal methods. Tissue allografts are commonly used in orthopedic surgical procedures; in 1999, approximately 650... more Tissue allografts are commonly used in orthopedic surgical procedures; in 1999, approximately 650,000 musculoskeletal allografts were distributed by tissue processors (1). A rare complication of musculoskeletal allografts is bacterial infection (2,3). After the reported death of a recipient of an allograft contaminated with Clostridium spp. (an anaerobic spore and toxin-forming organism) (3), CDC investigated this case and solicited additional reports of allograft-associated infections; 26 cases have been identified. This report summarizes the investigation of these cases and describes additional steps given to a tissue processor to enhance tissue transplant safety.
