In this sense, given the repercussions of the pandemic caused by the SARS-CoV-2 virus, we researched the clinical outcomes of COVID-19 in patients with rheumatoid arthritis using antirheumatic drugs, with the objective of describe the incidence of COVID-19 in patients with rheumatoid arthritis and identify the more frequent clinical outcomes, comparing the prevalence of clinical complications of the disease caused by the new coronavirus between patients with rheumatoid arthritis and the general population, and evaluating the implications of treatment with antirheumatic drugs on the clinical outcomes of COVID-19. METHODS We conducted a systematic review of the literature based on a bibliographic survey in the PubMed/MEDLINE, EMBASE and LILACS databases about COVID-19 and rheumatoid arthritis, from December 2020 to September 2021, including articles in English, Portuguese and Spanish, observational studies, studies with primary or secondary objective of evaluating the proposed theme, and studies with available full text. A systematization and a descriptive analysis of the information was performed, as well as analysis of the methodological quality of the included studies. RESULTS The incidence of COVID-19 in patients with rheumatoid arthritis is 25% higher than in the general population. Hospitalizations, ICU admission and deaths due to COVID-19 were more prevalent in the population with rheumatoid arthritis, but these outcomes seemed to be more associated with the prevalence of comorbidities in this population. The main comorbidities that influenced the emergence of complications of coronavirus infection were cardiovascular diseases, diabetes mellitus, obesity and chronic kidney disease. The disease modifying antirheumatic drugs (DMARDs) that were consensually associated with complications of COVID-19 were methotrexate, sulfasalazine and rituximab. The drugs that had no influence on the course of infection were tumor necrosis factor inhibitors, abatacept and hydroxychloroquine. Other DMARDs evaluated had divergences between the studies. CONCLUSION Rheumatoid arthritis seems to be a disease that increases the risks of clinical complications of COVID-19 due to its coexistence with other chronic diseases that increase the morbidity and mortality of this population. However, the current literature still lacks publications with good methodological quality and a prospective design to define the additional risk of complications by COVID-19 when having rheumatoid arthritis, and further studies are needed in this sense.
