This cross sectional study was done to find out the level of hearing improvement in Type-I tympan... more This cross sectional study was done to find out the level of hearing improvement in Type-I tympanoplasty with cortical mastoidectomy in tubotympanic chronic otitis media. Data were analyzed from 60 patients with audiometric studies with a minimum of 6 months follow up. Patients who under went tympanoplasty alone were kept in Group I, and who under went tympanoplasty with cortical mastoidectomy were kept in Group II. All patients prior to tympanoplasty were examined thoroughly after taking detailed history. On otoscopy, site and size of the tympanic membrane perforation, condition of the rest of the tympanic membrane, condition of the middle ear mucosa and the ossicles were noted. Tuning fork test, test for facial nerve integrity and fistulae test were performed. Hearing level was assessed by pure tone audiometry with masking. Methods: This cross-sectional study was done in the department of otolaryngology and head neck surgery, BSMMU, Sahbag, Dhaka during the period of January 2009 to December 2010.A total of 60 patients were under went myringoplasty operation after taking detailed history, clinical examination and investigation. Preoperative and postoperative hearing assessment was done. Analysed data presented by various tables, graphics and figures. Results: In case of small size perforation preoperative mean bone conduction threshold was 7.66 dB, mean air conduction threshold was 34.14 dB and mean air bone gap was 26.48 dB. In case of medium size perforation preoperative mean bone conduction threshold was 9.61 dB, mean air conduction threshold was 44.48 dB. Mean air bone gap was 34.87 dB. A Prospective study was carried out from Jan 2000 Jan 2003, to find out the prevalence of OME amo... more A Prospective study was carried out from Jan 2000 Jan 2003, to find out the prevalence of OME among School children The study was done in Dhaka City and adjacent areas of Dhaka City in Savar, Munshigonj and Keranigonj, Altogether 280 Students aged 4-9 years were interviewed and examined. Nineteen percent children were found to have OME. In the urban community it was 17%, but in rural areas it was 21%. In our study children with OME were more in poorer people, smoker parents and bottle fed babies. An attempt has been made to evaluate metastatic neck node clinically. 60 cases were studied. Primary lesion identified in 53 cases (88.33%) and remain undetected in 7 cases (11.67%). Among primary sixty (60) diseases, 43 (81.13%) arises from upper aerodigestive tract which were squamous cell carcinoma & 10 (18.87%) came from thyroid & parotid gland. Commonest primary site was Larynx (36.67%). Majority (55%) of patients were age group 41-60. Unilateral lymph node was involved in 49 cases (81.67%), bilateral in 10 (16.67%), contra lateral in 1 case (1.66%), Single node in 22 (36.67%), Multiple in 38 (63.33%) cases. Size of the node >6cm was found in 24 cases (40%), 3-6cm in 20 cases (63.33%), <3cm in 16 cases (26.67%). Most of the involved nodes were in level- II. Tracheostomy is used when a formal opening or stoma is made 1. The early history of tracheostomy started with Galen & Aretaeus. Galen approved the operation & successfully opened the wind pipe of a goat. In 1934 E.W. Goodall was able to find only twenty eight successful cases of tracheostomy recorded up to the time of Bretonneau 3. The chief indication were acute inflammations of the pharynx & larynx and the removal of the foreign bodies 3. Some authors credited Asclepiades of Bittynia (second century A.D) with honour of being the first to perform the operation 2. The development of tracheostomy has been divided into five periods: the 'period of legend' dating from 2000 B.C. to A.D.

All patients (n=154) of thyroid malignancy admitted in the Otolaryngology Department of Bangaband... more All patients (n=154) of thyroid malignancy admitted in the Otolaryngology Department of Bangabandhn Sheikh Mujib Medical University (former IPGMR) between 1986 and 2000 were retrospectively analyzed to find out the extent and result of surgery used for thyroid carcinoma. The other objectives were to find out the incidence of differentiated thyroid carcinoma among the thyroid malignaney and also to find out the age, sex anti clinical presentation of papillary and fallicular carcinoma. Among all the thyroid malignancy (n=154), Differentiated Thyroid Carcinoma (DTC) was seen in 130 (84.41%) cases, where as papillary carcinoma occurred in 98(63.64%), and follicular carcinoma in 32(20.77%)cases. On the basis of risk factors, the DTC were designated as low and high risk.The year-wise incidence of DTC revealed increasing trend from 1986 (3 cases) to 2000 (23 cases). Among the 98 papillary thyroid carcinoma highest number of cases (35.71%) were seen in 31-40 year age group. The male to female ratio was 1: 1.64. In .follicular carcinoma, highest number (35.25%) o f cases were also seen in 31-40 year age groap. The male to female ratio was 1:1.66. The commonest presentation in papillary carcinoma was thyroid swelling (96.93%). The other presentations were occult thyroid (3.06%), Cervical lymph node metastasis (38.77%) and distant metastasis (2.04%). In Follicular carcinoma, the presentations were thyroid swelling (100%), Cervical lymph node metastasis (6.25%) and Distant metastasis (21.87%). In this series, low risk DTC were treated by Lobectomy & Isthmusectomy plus Thyroxin. In low risk group the rate of recurrence was 6.89% and the mortality was nil in five years fallow-up. Except two inoperable cases, all high risk patients were managed by Total thyroidectamy (with or without neck dissection, plus removal o f metastatic lesion when required) with Radioiodine ablation plus Thyroxin.. The rate of recurrence was 7.81% and mortality was 1.56% in high risk group in similar period of time. Vocal cord palsy were noted in 5 (3.84%) unilateral, and inone (0.76%) bilateral cases. Hypoparathyroidism was f o u n d in 4.61%.
