In this research, we aimed to investigate the positivity of SARS-CoV-2 by RT-PCR from distal part of the endotracheal tube (DPET) samples, which have not been investigated in any study yet. Materials and Methods: A total of 48 patients with a diagnosis of COVID-19 hospitalized in the intensive care unit receiving mechanical ventilation and whose conditions resulted in death or extubation were included in the study. The distal 6 cm part of the orotracheal intubation tube was removed from the patient (including the cuff). DPET samples were mixed with viral transport medium and vortexed; then, it was centrifuged at 4500g for 4 minutes. MATERIAL METHOD Thirty-two extensively drug-resistant Klebsiella pneumonia strains were included in the study. Detection of the carbapenemase genes was performed using the Gene-Xpert® System Carba R® kit. IV fosfomycin susceptibility was performed using the agar dilution method. In our study, to see the effect of glucose 6-phosphate (G6-P) on sensitivity results, our agar dilution method was repeated using Muller-Hinton Agar medium without G6-P. RESULTS All of the strains studied had carbapenemase production, and the strains were resistant to all drugs, including carbapenems, piperacillin-tazobactam, ceftriaxone, and ceftazidime. Fosfomycin resistance was detected in 3 (9.3%) of all strains. When the sensitivity test is repeated without using glucose 6-phosphate; In 82.7% of the fosfomycin susceptible strains, fosfomycin resistance was detected. With the Gene-Xpert® System, the association of NDM-1 in 46.8%, OXA-48 in 18.7%, KPC in 3.1%, and NDM-1 + OXA-48 in 21.8% of the strains were detected. While OXA-48 was detected in 1 of the resistant strains, none of the viable genes were detected in 2 of them. CONCLUSION Our study has shown that IV fosfomycin may be an essential alternative option in treating infections caused by common resistant strains. It should be kept in mind that accurate results may not be obtained unless G6-P is used in the agar dilution method used in in vitro susceptibility studies of fosfomycin.

Herpes simpleks virüsü tip 1 (HSV-1) tedavisinde; bir nükleoz(t)it analoğu olan asiklovir kullanı... more Herpes simpleks virüsü tip 1 (HSV-1) tedavisinde; bir nükleoz(t)it analoğu olan asiklovir kullanılmakla birlikte, bu tür ilaçların sık kullanımı sonucunda, son dönemde ilaca dirençli virüslerin ortaya çıkması ve bu ilaçların toksik yan etkileri, tedavide yeni aktif moleküllerin araştırılmasına olan ihtiyacı artırmıştır. Tıp bilimleri ve farmakolojideki sentetik molekül geliştirme çalışmalarına rağmen, biyolojik olarak aktif bileşiklerin doğal bir kaynağı olan bitkiler, tüm dünyada popüler olmaya devam etmektedir. Bu in vitro çalışmada, zeytin yaprağı ekstresi ve propolisin tek başına ya da asiklovir ile kombine olarak HSV-1'e karşı antiviral etkinliklerinin araştırılması amaçlanmıştır. Zeytin yaprağı ekstresi ve propolis ile propolisin çözücüsü olarak dimetilsülfoksitin, Hep-2 (ATCC, CCL-23) hücrelerine toksik dozları konvansiyonel hücre kültürü yöntemi ile belirlenmiştir. "Son nokta" yöntemi kullanılarak doku kültürünün yarısını enfekte eden viral doz (TCID 50) değeri hesaplanmış, virüs miktarları Spearman-Karber yöntemine göre belirlenmiştir. Zeytin yaprağı ekstresi ve propolisin, HSV-1 üzerine antiviral etkileri, konvansiyonel hücre kültürü ve gerçek zamanlı hücre analizi (RTCA) yöntemleriyle araştırılmıştır. Her iki ekstraktın asiklovir ve birbirleriyle kombinasyonları ise RTCA ile değerlendirilmiştir. Combination treatments should also be applied to prevent development of resistance to colistin. This study examines the in vitro synergic effect of the colistin/sulbactam combination in carbapenem-resistant Acinetobacter strains with the time-kill and checkerboard methods. Materials and Methods: Twenty carbapenem-resistant Acinetobacter baumannii-calcoaceticus complex strains, which were isolated from various clinical samples, were included in this study. Strains were identified with mass spectrometry, and antibiotic sensitivity results were determined with the VITEK 2® system. The in vitro effect and synergic activity of the colistin, sulbactam, and colistin/ sulbactam combination on the carbapenem-resistant strains were determined using the time-kill and checkerboard methods. Seventeen strains were examined with the time-kill method, and twenty strains were examined using the checkerboard method. The fractional inhibitory concentration index of strains was calculated for detection of synergic effect. Results: Using the time-kill method applied on the colistin/sulbactam combination showed that the combination had a synergic effect on all 17 strains, while sulbactam alone did not have a bactericidal effect in the studied concentrations. When applying the checkerboard method, it was determined that the colistin/sulbactam combination had a synergic effect on 17 of the strains (85%) and an additive effect on 3 strains (15%), sulbactam had a low effect alone (15%), and colistin was effective on all strains. Conclusion: Study results indicated that the colistin/sulbactam combination had a high level of synergic effect on all studied strains using both methods.

Introduction: In this study, we aimed to compare the place of throat mouthwash in the diagnosis o... more Introduction: In this study, we aimed to compare the place of throat mouthwash in the diagnosis of COVID-19 and the detection values of SARS-Cov-2 in naso-oropharyngeal swab (NOS) and mouthwash samples. Materials and Methods: NOS and mouthwash water samples were taken from the patients simultaneously. Mouthwash sampling was obtained by himself. For the NOS sample;after the swab sample was taken from the patient’s oropharynx with a dacron tip swab, with the same swab. RNA isolation from combined oral/nasal swab samples was performed with the EZ1 (Qiagen, Germany) device. Statistical analyses were performed using the SPSS software version 21.0. Results: The diagnostic efficacy of throat mouthwash and combined NOS samples taken from 106 volunteers were compared (56 males, mean age 42.9 years). The diagnostic sensitivity of the mouthwash was 60.71%, specificity 98.65%, positive predictive value 94.44% and negative predictive value 86.9% compared to the results of the NOS. According to these results, although mouthwash is a specific test, its sensitivity to NOS is not satisfactory. The value of kappa was substantial agreement as 0.668, and it was found statistically significant (<0.001). Conclusion: Throat mouthwash is a promising noninvasive technique for diagnosis, monitoring and infection control in patients with COVID-19 infection and reduces the risk of transmission for the healthcare provider.

Purpose: The study objective was to identify severe acute respiratory syndrome coronavirus 2 (SAR... more Purpose: The study objective was to identify severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) RNA in conjunctival swabs from patients with confirmed SARS-CoV-2 infection. Methods: Forty patients who tested positive by real-time reverse transcription (rRT)-polymerase chain reaction (PCR) of nasopharyngeal and oropharyngeal swabs were enrolled. Conjunctival swabbing was used to collect the tear and conjunctival secretions of patients. Results: Conjunctival swab rRT-PCR was positive for three patients and negative for 37 patients. Ten of the patients (25%) were diagnosed with conjunctivitis during the ophthalmic examination. Of these patients, one was found positive by conjunctival swab rRT-PCR, and nine were found negative. The difference between patients who tested positive or negative using conjunctival swab rRT-PCR was without statistical significance in terms of the presence of conjunctivitis (p = .720). Conclusion: The rate of positivity from conjunctival swab rRT-PCR was 7.5% in patients with confirmed SARS-CoV-2 infection.

Piperasilin-tazobaktam (TZP), genişletilmiş spektrumlu beta-laktamaz (GSBL) üreten Gramnegatif ba... more Piperasilin-tazobaktam (TZP), genişletilmiş spektrumlu beta-laktamaz (GSBL) üreten Gramnegatif bakterilerin neden olduğu enfeksiyonların tedavisi için sıklıkla tercih edilen bir ajandır. Bununla birlikte, GSBL üreten bakterilerin neden olduğu ciddi enfeksiyonlara Piperasilin-tazobaktamın etkinliği hakkında tartışmalar vardır. Bu çalışmada, laboratuvara gönderilen örneklerden izole edilen GSBL pozitif bakterilerde TZP'nin etkinliğini belirlemeyi amaçladık. Servis ve poliklinik hastalarına ait kan, idrar, yara, trakeal aspirat örneklerinden izole edilen 548 GSBL pozitif Escherichia coli ve Klebsiella pneumoniae suşu çalışmaya dahil edildi. Bakteri tanımlaması, antibiyotik duyarlılık testleri ve GSBL tespitinde VİTEK 2 ® otomatize sistemi kullanıldı. Çalışmamıza dahil edilen izolatların 307'si (%56) E. coli, 241'i (%43,9) K. pneumoniae suşu idi. İzolatların TZP direnç oranları incelendiğinde; E. coli suşlarında TZP direnç oranı %44,6 olarak saptanırken en yüksek direnç %47,6 ile trakeal aspirat örneklerinde saptandı. K. peumoniae suşlarında ise TZP direnç oranı %41 olarak bulundu. GSBL pozitif bakterilerin neden olduğu enfeksiyonlarda TZP'nin etkinliğini gösteren yeterli çalışma bulunmamakla birlikte, mevcut çalışmalar bu antibiyotiklerin etkinliğinin enfeksiyon bölgesine ve şiddetine göre değiştiğini göstermektedir. PRP sonuç bildirimi ile ilgili CLSI ve EUCAST arasında bir fikir birliği yoktur. Bu nedenle, bu konuda daha kapsamlı bir çalışmaya ve TZP'nin sonuç raporunun yeniden değerlendirilmesine ihtiyaç vardır.

The emergence and spread of multi-drug-resistant (MDR), extended-spectrum beta-lactamase (ESBL) p... more The emergence and spread of multi-drug-resistant (MDR), extended-spectrum beta-lactamase (ESBL) producing carbapenem-resistant members of Enterobacteriaceae family has become a worldwide health problem. Carbapenem resistance caused by blaKPC, blaNDM gene regions are sporadic and blaOXA-48 gene region is endemic in our country. The aim of this study was to determine the presence of blaOXA-232, blaOXA-181, blaOXA-162, blaOXA-204, blaOXA-244, blaOXA-163, blaOXA-245 genes in OXA-48 like carbapenemase producing Klebsiella pneumoniae isolates. The isolates used in this study were provided from the Medical Microbiology Laboratory collection of Sakarya University Sakarya Training and Research Hospital. Identification and antibiotic susceptibility tests were determined by the VITEK 2® automated system (biomerieux, France) and the carbapenemase production of isolates was determined by the modified Hodge test. Minimal inhibitor concentration (MIC) values were determined with broth microdilution method. The isolates containing the blaOXA-48-like gene region were identified by real-time polymerase chain reaction (Rt-PCR) method using consensus primers. In "High Resolution Melting Analysis (HRMA)" method carried out by using "Type-it HRM PCR" (Qiagen, Hilden, Germany) kit, isolates which showed a deviation in melting temperatures (Tm) were selected with the suspicion of OXA-48 variant. The sequence analysis (ABI 3500, Applied Biosystems, USA) was carried out to determine which variants were present in these isolates. Compatibility of MIC values was determined between VITEK 2® and the microdilution method with the rate of 82% for imipenem, 77% for meropenem and 90% for ertapenem in carbapenemase-producing K.pneumoniae isolates. In 45 of 100 K.pneumoniae isolates, the blaOXA-48-like gene region was found to be positive by the Rt-PCR method. For the determination of OXA-48 variants, these 45 isolates were evaluated by HRMA method. The sequence analysis revealed that 41 (91.2%) isolates contained blaOXA-48/blaOXA-245 gene regions, while 2 (4.4%) isolates were found to contain blaOXA-181 gene regions and 2 (4.4%) isolates were found to contain blaOXA-244 gene regions. This is the first study to determine OXA-48 and OXA-244 positivity in blaOXA-48-like gene regions in Turkey. As a result of this study, the OXA-48-like gene region was found to be 45%, of which 4.4% had blaOXA-181 and 4.4% had blaOXA-244 gene regions. The detection of blaOXA-48-like gene regions will guide for the selection of antibiotics in critical patient groups.

This paper presents the results of a comprehensive examination of current distribution of Hepatit... more This paper presents the results of a comprehensive examination of current distribution of Hepatitis A virus (HAV) seroprevalence and endemicity in Turkey and the possible links between HAV endemicity and socioeconomic development. We performed a systematic search in online resources published between January 2000 and August 2015. The 22 provinces were able to be assigned a hepatitis A endemicity level based on this systematic review. The incidence rates for symptomatic hepatitis A infection are higher in the eastern part of Turkey than in the western and central region. These differences in socioeconomic indicators by region suggest the likelihood of lower seroprevalence rates in the western parts of the country and higher rates in the eastern region. Turkey's current policy of recommending hepatitis A immunization for all children without contraindications is an appropriate one and is likely to remain the best option for at least the next decade or two.

The New Delhi metallo-β-lactamase-1 (NDM-1) is a major problem around the world. The purpose of this article is to address the NDM-1 Klebsiella pneumoniae epidemic detected in eight cases in our hospital. Methods: Bacteria identified in this epidemic were from patients already admitted to the intensive care unit of the Sakarya University Training and Research Hospital during efforts toward establishment of infection surveillance and control program. Antimicrobial susceptibility testing of strains was performed using the VITEK 2 system (bioMérieux, France), E-test gradient strips (bioMérieux, France), and the disc diffusion test. For the metallo-beta-lactamase activity, the combined disc diffusion test and modified Hodge test as phenotypic tests were performed. To identify the resistance gene, the Xpert Carba-R kit (Cepheid Inc., USA) and an in-house multiplex polymerase chain reaction (PCR) method designed for five common carbapenemase genes (IMP, VIM, KPC, NDM-1, and OXA-48) were employed. The clonal relationship of these strains was explored by the repetitive PCR (rep-PCR, DiversiLab System, bioMérieux, France) method. Results: During the December 2014 to March 2015 period, NDM-1 positive K. pneumoniae strains were detected in eight patients. All of these strains were found to produce NDM-1, while two of them also revealed the presence of OXA-48. The rep-PCR results reveal a clonal proximity of 95 % for six of the eight strains. Conclusions: Our findings suggest the tendency of NDM-1-producing strains to spread in our country as well. A carbapenem-resistant K. pneumoniae threat may pose a great risk to our country. It is clear that more comprehensive infection control precautions should be implemented in our hospitals.

In this research, we aimed to investigate the positivity of SARS-CoV-2 by RT-PCR from distal part of the endotracheal tube (DPET) samples, which have not been investigated in any study yet. Materials and Methods: A total of 48 patients with a diagnosis of COVID-19 hospitalized in the intensive care unit receiving mechanical ventilation and whose conditions resulted in death or extubation were included in the study. The distal 6 cm part of the orotracheal intubation tube was removed from the patient (including the cuff). MATERIAL METHOD Thirty-two extensively drug-resistant Klebsiella pneumonia strains were included in the study. Detection of the carbapenemase genes was performed using the Gene-Xpert® System Carba R® kit. IV fosfomycin susceptibility was performed using the agar dilution method. In our study, to see the effect of glucose 6-phosphate (G6-P) on sensitivity results, our agar dilution method was repeated using Muller-Hinton Agar medium without G6-P. RESULTS All of the strains studied had carbapenemase production, and the strains were resistant to all drugs, including carbapenems, piperacillin-tazobactam, ceftriaxone, and ceftazidime. Fosfomycin resistance was detected in 3 (9.3%) of all strains. When the sensitivity test is repeated without using glucose 6-phosphate; In 82.7% of the fosfomycin susceptible strains, fosfomycin resistance was detected. With the Gene-Xpert® System, the association of NDM-1 in 46.8%, OXA-48 in 18.7%, KPC in 3.1%, and NDM-1 + OXA-48 in 21.8% of the strains were detected. While OXA-48 was detected in 1 of the resistant strains, none of the viable genes were detected in 2 of them. CONCLUSION Our study has shown that IV fosfomycin may be an essential alternative option in treating infections caused by common resistant strains. It should be kept in mind that accurate results may not be obtained unless G6-P is used in the agar dilution method used in in vitro susceptibility studies of fosfomycin.

Combination treatments should also be applied to prevent development of resistance to colistin. This study examines the in vitro synergic effect of the colistin/sulbactam combination in carbapenem-resistant Acinetobacter strains with the time-kill and checkerboard methods. Materials and Methods: Twenty carbapenem-resistant Acinetobacter baumannii-calcoaceticus complex strains, which were isolated from various clinical samples, were included in this study. Strains were identified with mass spectrometry, and antibiotic sensitivity results were determined with the VITEK 2® system. The in vitro effect and synergic activity of the colistin, sulbactam, and colistin/ sulbactam combination on the carbapenem-resistant strains were determined using the time-kill and checkerboard methods. Seventeen strains were examined with the time-kill method, and twenty strains were examined using the checkerboard method. The fractional inhibitory concentration index of strains was calculated for detection of synergic effect. Results: Using the time-kill method applied on the colistin/sulbactam combination showed that the combination had a synergic effect on all 17 strains, while sulbactam alone did not have a bactericidal effect in the studied concentrations. When applying the checkerboard method, it was determined that the colistin/sulbactam combination had a synergic effect on 17 of the strains (85%) and an additive effect on 3 strains (15%), sulbactam had a low effect alone (15%), and colistin was effective on all strains. Conclusion: Study results indicated that the colistin/sulbactam combination had a high level of synergic effect on all studied strains using both methods.

Materials and Methods: NOS and mouthwash water samples were taken from the patients simultaneously. Mouthwash sampling was obtained by himself. For the NOS sample;after the swab sample was taken from the patient’s oropharynx with a dacron tip swab, with the same swab. RNA isolation from combined oral/nasal swab samples was performed with the EZ1 (Qiagen, Germany) device. Statistical analyses were performed using the SPSS software version 21.0. Results: The diagnostic efficacy of throat mouthwash and combined NOS samples taken from 106 volunteers were compared (56 males, mean age 42.9 years). The diagnostic sensitivity of the mouthwash was 60.71%, specificity 98.65%, positive predictive value 94.44% and negative predictive value 86.9% compared to the results of the NOS. According to these results, although mouthwash is a specific test, its sensitivity to NOS is not satisfactory. The value of kappa was substantial agreement as 0.668, and it was found statistically significant (<0.001). Conclusion: Throat mouthwash is a promising noninvasive technique for diagnosis, monitoring and infection control in patients with COVID-19 infection and reduces the risk of transmission for the healthcare provider.

Purpose: The study objective was to identify severe acute respiratory syndrome coronavirus 2 (SAR... more Purpose: The study objective was to identify severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) RNA in conjunctival swabs from patients with confirmed SARS-CoV-2 infection. Methods: Forty patients who tested positive by real-time reverse transcription (rRT)-polymerase chain reaction (PCR) of nasopharyngeal and oropharyngeal swabs were enrolled. Conjunctival swabbing was used to collect the tear and conjunctival secretions of patients. Results: Conjunctival swab rRT-PCR was positive for three patients and negative for 37 patients. Ten of the patients (25%) were diagnosed with conjunctivitis during the ophthalmic examination. Of these patients, one was found positive by conjunctival swab rRT-PCR, and nine were found negative. The difference between patients who tested positive or negative using conjunctival swab rRT-PCR was without statistical significance in terms of the presence of conjunctivitis (p = .720). Conclusion: The rate of positivity from conjunctival swab rRT-PCR was 7.5% in patients with confirmed SARS-CoV-2 infection.

Piperasilin-tazobaktam (TZP), genişletilmiş spektrumlu beta-laktamaz (GSBL) üreten Gramnegatif ba... more Piperasilin-tazobaktam (TZP), genişletilmiş spektrumlu beta-laktamaz (GSBL) üreten Gramnegatif bakterilerin neden olduğu enfeksiyonların tedavisi için sıklıkla tercih edilen bir ajandır. Bununla birlikte, GSBL üreten bakterilerin neden olduğu ciddi enfeksiyonlara Piperasilin-tazobaktamın etkinliği hakkında tartışmalar vardır. Bu çalışmada, laboratuvara gönderilen örneklerden izole edilen GSBL pozitif bakterilerde TZP'nin etkinliğini belirlemeyi amaçladık. Servis ve poliklinik hastalarına ait kan, idrar, yara, trakeal aspirat örneklerinden izole edilen 548 GSBL pozitif Escherichia coli ve Klebsiella pneumoniae suşu çalışmaya dahil edildi. Bakteri tanımlaması, antibiyotik duyarlılık testleri ve GSBL tespitinde VİTEK 2 ® otomatize sistemi kullanıldı. Çalışmamıza dahil edilen izolatların 307'si (%56) E. coli, 241'i (%43,9) K. pneumoniae suşu idi. İzolatların TZP direnç oranları incelendiğinde; E. coli suşlarında TZP direnç oranı %44,6 olarak saptanırken en yüksek direnç %47,6 ile trakeal aspirat örneklerinde saptandı. K. peumoniae suşlarında ise TZP direnç oranı %41 olarak bulundu. GSBL pozitif bakterilerin neden olduğu enfeksiyonlarda TZP'nin etkinliğini gösteren yeterli çalışma bulunmamakla birlikte, mevcut çalışmalar bu antibiyotiklerin etkinliğinin enfeksiyon bölgesine ve şiddetine göre değiştiğini göstermektedir. PRP sonuç bildirimi ile ilgili CLSI ve EUCAST arasında bir fikir birliği yoktur. Bu nedenle, bu konuda daha kapsamlı bir çalışmaya ve TZP'nin sonuç raporunun yeniden değerlendirilmesine ihtiyaç vardır.

Carbapenem resistance caused by blaKPC, blaNDM gene regions are sporadic and blaOXA-48 gene region is endemic in our country. The aim of this study was to determine the presence of blaOXA-232, blaOXA-181, blaOXA-162, blaOXA-204, blaOXA-244, blaOXA-163, blaOXA-245 genes in OXA-48 like carbapenemase producing Klebsiella pneumoniae isolates. The isolates used in this study were provided from the Medical Microbiology Laboratory collection of Sakarya University Sakarya Training and Research Hospital. Identification and antibiotic susceptibility tests were determined by the VITEK 2® automated system (biomerieux, France) and the carbapenemase production of isolates was determined by the modified Hodge test. Minimal inhibitor concentration (MIC) values were determined with broth microdilution method. The isolates containing the blaOXA-48-like gene region were identified by real-time polymerase chain reaction (Rt-PCR) method using consensus primers. In "High Resolution Melting Analysis (HRMA)" method carried out by using "Type-it HRM PCR" (Qiagen, Hilden, Germany) kit, isolates which showed a deviation in melting temperatures (Tm) were selected with the suspicion of OXA-48 variant. The sequence analysis (ABI 3500, Applied Biosystems, USA) was carried out to determine which variants were present in these isolates. Compatibility of MIC values was determined between VITEK 2® and the microdilution method with the rate of 82% for imipenem, 77% for meropenem and 90% for ertapenem in carbapenemase-producing K.pneumoniae isolates. In 45 of 100 K.pneumoniae isolates, the blaOXA-48-like gene region was found to be positive by the Rt-PCR method. For the determination of OXA-48 variants, these 45 isolates were evaluated by HRMA method. The sequence analysis revealed that 41 (91.2%) isolates contained blaOXA-48/blaOXA-245 gene regions, while 2 (4.4%) isolates were found to contain blaOXA-181 gene regions and 2 (4.4%) isolates were found to contain blaOXA-244 gene regions. This is the first study to determine OXA-48 and OXA-244 positivity in blaOXA-48-like gene regions in Turkey. As a result of this study, the OXA-48-like gene region was found to be 45%, of which 4.4% had blaOXA-181 and 4.4% had blaOXA-244 gene regions. The detection of blaOXA-48-like gene regions will guide for the selection of antibiotics in critical patient groups.

This paper presents the results of a comprehensive examination of current distribution of Hepatit... more This paper presents the results of a comprehensive examination of current distribution of Hepatitis A virus (HAV) seroprevalence and endemicity in Turkey and the possible links between HAV endemicity and socioeconomic development. We performed a systematic search in online resources published between January 2000 and August 2015. The 22 provinces were able to be assigned a hepatitis A endemicity level based on this systematic review. The incidence rates for symptomatic hepatitis A infection are higher in the eastern part of Turkey than in the western and central region. These differences in socioeconomic indicators by region suggest the likelihood of lower seroprevalence rates in the western parts of the country and higher rates in the eastern region. Turkey's current policy of recommending hepatitis A immunization for all children without contraindications is an appropriate one and is likely to remain the best option for at least the next decade or two.

The New Delhi metallo-β-lactamase-1 (NDM-1) is a major problem around the world. The purpose of this article is to address the NDM-1 Klebsiella pneumoniae epidemic detected in eight cases in our hospital. Methods: Bacteria identified in this epidemic were from patients already admitted to the intensive care unit of the Sakarya University Training and Research Hospital during efforts toward establishment of infection surveillance and control program. Antimicrobial susceptibility testing of strains was performed using the VITEK 2 system (bioMérieux, France), E-test gradient strips (bioMérieux, France), and the disc diffusion test. For the metallo-beta-lactamase activity, the combined disc diffusion test and modified Hodge test as phenotypic tests were performed. To identify the resistance gene, the Xpert Carba-R kit (Cepheid Inc., USA) and an in-house multiplex polymerase chain reaction (PCR) method designed for five common carbapenemase genes (IMP, VIM, KPC, NDM-1, and OXA-48) were employed. The clonal relationship of these strains was explored by the repetitive PCR (rep-PCR, DiversiLab System, bioMérieux, France) method. Results: During the December 2014 to March 2015 period, NDM-1 positive K. pneumoniae strains were detected in eight patients. All of these strains were found to produce NDM-1, while two of them also revealed the presence of OXA-48. The rep-PCR results reveal a clonal proximity of 95 % for six of the eight strains. Conclusions: Our findings suggest the tendency of NDM-1-producing strains to spread in our country as well. A carbapenem-resistant K. pneumoniae threat may pose a great risk to our country. It is clear that more comprehensive infection control precautions should be implemented in our hospitals.
