Body Mass index for RYBP patients before surgery was 47.3±8.2 kg/m2, and after surgery 31.3±5 kg/... more Body Mass index for RYBP patients before surgery was 47.3±8.2 kg/m2, and after surgery 31.3±5 kg/m2. All patients had a % of excessive body weight loss >50%. RYGBP patients and non-obese healthy control subjects had significantly different citrulline (25.5±4.3 vs. 33.0±7.5 umol/L respectively; p < 0.01) and arginine (56.0±12.2 vs. 71.2±14.9mg/dL; p < 0.05) plasma levels. Participants had normal creatinine values (0.7 1.4mg/dL for men and 0.6 1.2mg/dL for women). Conclusion: Following Roux-en-y gastric bypass, the decrease in enterocyte mass is illustrated by a citrullinemia lower than normal, which in turn likely explains hypoargininemia. The clinical significance (e.g. in term of immune and healing functions) of the latter should be explored as well as the interest of supplementation.

This study aimed to report a case of gossypiboma that was mistaken for a hydatid cyst in the preoperative evaluation. A 34-year-old male patient with a history of Nissen Fundoplication presented with a large mass palpable in the epigastrium and both the left upper and lower quadrants of the abdomen. Computerized tomography was reported to show a 20x18 cm cystic mass with a collapsed germinative membrane inside it. Laparotomy, which was performed with a suggested diagnosis of type 3 hydatid cyst, revealed that the mass was caused by a 30x30 cm surgical abdominal compress. We believe gossypiboma should be kept in mind in the differential diagnosis of abdominal hydatid cysts in the presence of a former abdominal operation, especially when the result of indirect hemagglutination test is negative.

Our study aims to compare the surgical outcome of Limberg transposition flap, Karydakis flap, and... more Our study aims to compare the surgical outcome of Limberg transposition flap, Karydakis flap, and primary closure after excision to treat sacrococcygeal pilonidal sinus disease. A total of 634 patients with pilonidal sinus who underwent surgery were evaluated retrospectively from January 2014 to January 2016. The patients were divided into three groups. Limberg transposition flap (LTF) was performed in 131 patients (group 1), Karydakis flap (KF) was performed in 232 patients (group 2) and primary closure (PC) after excision was performed in 271 patients (group 3). Patient demographics, operative and postoperative outcomes were recorded and analyzed retrospectively. The mean age (p=0.98), sex ratio (p=0.74) and removed sinus volume (p=0.67) were not statistically different between groups. Helikobakter pilori (HP) toplumda yaygın olarak görülen, peptik ülser hastalığı ve malignitelerle... more Helikobakter pilori (HP) toplumda yaygın olarak görülen, peptik ülser hastalığı ve malignitelerle ilişkili gram negatif bir bakteridir. Dispeptik şikâyetlerle başvuran hastalara yapılan endoskopik biyopsi sonucu HP sıklığı ve intestinal metaplazi birlikteliği incelenmiştir. Gereç ve Yöntem: Siverek Devlet Hastanesi Genel Cerrahi Polikliniğine Kasım 2013-Ocak 2015 tarihleri arasında medikal tedaviye dirençli dispeptik şikayetlerle başvuran hastalara yapılan gastroskopi sonuçları ve antrumdan alınan biyopsi sonuçları retrospektif olarak incelendi. Patoloji sonuçları HP değerlendirmesi için Giemsa, intestinal metaplazi değerlendirmesi için PAS-AB ile değerlendirildi. Bulgular: Toplam 682 hasta değerlendirildi. Hastaların 304 ü erkek, 378'i kadın hastaydı, yaş ortalaması 38,22 (±14,64); en genç hasta 18 yaşında, en yaşlı hasta ise 88 yaşındaydı. Yapılan endoskopik değerlendirme sonucu 555 (%81,4) antral gastrit; 81 (%11,9) pangastrit; 6 (%0,9) ülserovejetan kitle tespit edildi. Patolojik olarak 469 (%69,6) kronik aktif gastrit; 201 (%29,5) kronik inaktif gastrit; 6 (%0,9) adenokarsinom tespit edildi. 475 (%69,6) hastada HP; 56 (%8,2) hastada intestinal metaplazi, 11 (%1,6) hastada ise atrofi olduğu görüldü. Adenokarsinom tespit edilen 6 hastanın 4'ünde (%66,6) HP varlığı gösterildi. HP olan hastaların 48'inde intestinal metaplazi birlikteliği görüldü; 8 hastada ise intestinal metaplazi varlığında HP görülmedi. Helikobakter pilori varlığında intestinal metaplazi olması istatistiksel olarak anlamlı bulundu (p: 0,006). Tartışma: Helikobakter pilori midede yüksek oranda prekanseröz lezyon olan intestinal metaplaziye yol açabilmektedir. Genel cerrahi pratiğinde medikal tedaviye dirençli dispeptik şikâyetleri olan hastalar daha dikkatli değerlendirilmelidir.

Traumatic abdominal wall hernia after blunt trauma is a rare entity. They can easily be overlooke... more Traumatic abdominal wall hernia after blunt trauma is a rare entity. They can easily be overlooked in patients who have multiple trauma, as its signs and symptoms may be variable due to the presence of multiple injuries. Imaging with computed tomography or ultrasound confirms the diagnosis as well as identifying any associated injuries. Although surgery is the standard treatment for traumatic abdominal wall hernias, there is no consensus on the early or late repair of the defect. Some authors recommend early surgical intervention in order to avoid the risk of intra-abdominal organ injury, incarceration, and strangulation. In this study, we report our experience in three cases, which did not involve emergency surgery. Long-term outcome is successful. Elective hernia repair may be safe and feasible in stable patients.

OBJECTIVES: This study reports the results of a single center experience on the use of pharmacolo... more OBJECTIVES: This study reports the results of a single center experience on the use of pharmacological venous thromboembolism (VTE) prophylaxis in laparoscopic cholecystectomy patients. BACKGROUND: The prevention of VTE is of crucial importance in surgical practice. However, the severity of thromboembolism risk and the necessity of thromboprophylaxis for laparoscopic cholecystectomy is still being debated. METHODS: The data of the patients, who underwent laparoscopic cholecystectomy for symptomatic cholelitiasis in a single center between the years 2005 and 2015 were analysed retrospectively for incidents of symptomatic VTE and bleeding complications. Fisher Exact Test was used to compare the outcomes of the patients who did and did not receive thromboprophylaxis. RESULTS: Of the 1485 patients who were included in the study, 307 (20.67 %) having a low VTE risk, did not receive any thromboprophylaxis; while 1178 (79.33 %) with a medium, high or a very high risk received VTE prophylaxis. A bleeding complication occurred in 14 (1.18 %) patients receiving prophylaxis and in 2 (0.65 %) patients not receiving prophylaxis (p = 0.548). No patients in this study experienced clinically symptomatic VTE. CONCLUSIONS: The fi ndings of this study indicate that the selective use of thromboprophylaxis does not signifi cantly increase the risk of bleeding after laparoscopic cholecystectomy and probably decreases the incidence of symptomatic thrombotic complications (Ref.

Wound healing is a complex process, dependent on available nutrition substrates. When used togeth... more Wound healing is a complex process, dependent on available nutrition substrates. When used together with β-hydroxy β-methylbutyrate, arginine and glutamine have been shown to increase collagen deposition in human subjects. However, there are no experimental investigations on the influence of this amino acid mixture with regard to secondary wound healing. The aim of this study is to investigate the effects of the supplementation of these 3 amino acids on the healing of open wounds in otherwise healthy animals. Materials and Methods: Twelve rats were divided into control and treatment groups. Two 2-cm × 1-cm full-thickness skin defects were prepared on each subject. The rats in both groups received a diet containing 1.2 g of protein per 100 g of body weight per day. The treatment group, in addition, received 200 mg/kg L-arginine, 200 mg/kg L-glutamine, and 40 mg/kg β-hydroxy β-methylbutyrate every day. Wound sizes were measured every 2 days. On the 10th day, tissue samples were taken for histopathologic evaluation and also for the measurement of hydroxyproline concentrations. Results: There was no statistically significant difference between mean wound sizes for the 2 groups (P &amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;gt; .05). There was also no statistically significant difference between the groups with regard to histological healing parameters (reepithelialization [P = 1.00], granulation tissue [P = 1.00], collagen accumulation [P = .455], inflammatory cell accumulation [P = .455], angiogenesis [P = .242]) or tissue hydroxyproline concentrations (P = .240). Conclusion: Diet supplemented with arginine, glutamine, and β-hydroxy β-methylbutyrate is not beneficial in enhancing secondary healing of open wounds in rats. Further research regarding this topic is warranted.
