In this study we aimed to evaluate the upper limb deep vein thrombosis (ULDVT) cases retrospectively according to risk factors, symptoms, diagnosis and clinical course. Methods: Between January 2012 and May 2014, we studied 23 deep vein thrombosis cases which were confirmed with Doppler ultrasound. The patients were ques tioned about risk factors and underlying diseases and were examined with upper extremity venous Doppler ultrasonography. 15 cases had (65.2%) central venous catheter being the most common ULDVT reason. Results: Protocol included low molecular weight heparin in the acute phase and then oral anticoagulant for 6 months. All patients were followed at outpatient clinic with 2 month intervals for a year. Before completing the followup, 5 patients (21.7%) died of other reasons and we didn’t see any pulmonary embolus complication and recurrent D...

The control group (Group 1) (n=6), ischemia/reperfusion (IR) group (Group 2) (n=8), cardiac ischemic preconditioning (CIP) group (Group 3) (n=8), iloprost (ILO) group (Group 4) (n=8), and cardiac ischemic preconditioning + iloprost (CIP+ILO) group (Group 5) (n=8). Pre-ischemia, 15 minutes post-ischemia, 45 minutes post-reperfusion, mean blood pressure (MBP), and heart rates (HR) were recorded. The rate-pressure product (RPP) was calculated. Post-reperfusion plasma creatine kinase-MB (CK-MB), lactate dehydrogenase (LDH), troponin (cTn) vlaues, and infarct size/area at risk (IS/AAR) were calculated from myocardial tissue samples. Arrhythmia and ST segment elevations were evaluated during the ischemia and reperfusion stages. Although the MBP, HR, RPP values, bioc...

The incidence of coronary artery disease is increasing in patients with liver transplantation. Liver transplanted patients are a high risk subgroup for coronary artery disease, even if they are asymptomatic. Coronary artery disease is a predictor of poor outcomes in patients with liver transplantation; therefore, identification of those patients at risk for coronary artery disease should be a key clinical priority. In this study, it was aimed to present coronary artery bypass grafting surgery in a 68-year-old male patient with previous liver transplantation 12 years ago. Coronary artery bypass grafting operation was performed using left internal mammarian artery and autogenous vein graft for three vessels. There was no complication in the preand post-operative periods. Coronary artery bypass grafting operation can be perform...

Reperfusion is the recovery of blood flow and oxygen to a hypoxic tissue. The aim of this study is to investigate the effect of flaxseed (L. Usitatissimum) in a hyper cholesterolemic rat model of lower extremity ischemia-reperfusion injury. Material and method: 38 female Wistar rats weighing 200 to 250 grams were used in the study. The rats were assigned into 4 groups as follows: Group 1 (control, normal diet, n:8); Group 2 (40 mg/kg fed by body lignan complex, n:10), Group 3: (fed by 0.1% cholesterol containing diet, n:10), Group 4 (fed by 0.1% cholesterol containing diet+40 mg/kg body lignan complex, n:10). Ischemia was modeled by performing ligation of the left femoral artery using 6/0 prolene for two hours followed by two hours of perfusion and the blood flow was measured throughout the process in all groups by Doppler ultrasonography. Results: In Group 3, serum total cholesterol and triglyceride levels were higher compared to subjects in Group 1, Group 2 and Group 4 (p<0.05). The difference was insignificant between groups according to oxidative markers (TAC, TOC) (p>0.05). Conclusion: The present study shows that flaxseed reduces the ischemia-reperfusion injury that occurs in femoral artery endothelium and gastrocnemius muscle following femoral artery occlusion-reperfusion in rats. The main findings in support of this conclusion include the significantly lower serum total cholesterol, LDL-cholesterol and triglyceride values in rats fed by flaxseed compared to hyper-cholesterolemic rats that were given flaxseed. In addition, assessment of the femoral artery endothelium and gastrocnemius muscle samples using the immunohistochemical method revealed significantly lower edema and neutrophil infiltration in rats fed by flaxseed.

Hypertension is one of the major risk factors. In this paper, a 56 years old female patient with hypertension and renal failure was presented. In this case after clinical suspicion, aortic dissection was diagnosed with imaging studies. Endovascular aortic repair was performed but after surgery, patient died because of possible hemorrhage complication. In aortic disection, diagnosis and treatment could be fast. In chronic kidney disease the prognosis of patients is adversely affected. Despite proper surgical intervention accompanying medical problems like chronic kidney disease have negative effects on survival.

We present a patient with long-term remission after primary cardiac sarcoma resection. Case Report: A 42-year-old previously healthy female presented to the emergency department after an effort-induced 30-minute episode of chest pain and extreme shortness of breath. Physical examination upon admission was remarkable for a pulse of 99/minute; blood pressure was 101/73 mmHg. Transthoracic echocardiography showed a mass measuring 5.5 × 5.6 cm extending from the left septum to the mitral valve anterior leaflet. A multilobulated broad-based 5.5 × 5.6 × 4 cm3 mass invading a large portion of the left septum to the mitral valve anterior leaflet was completely excised in the open heart surgery. Chemotherapy regimen (paclitaxel 175 mg/m2/day on day 1, every 21 days) was started after operation. Full remission was provided. Metastasis and recurrence have not been observed for 5 years of follow-up by PET. We observed d...

We investigated the association between the presence and severity of carotid artery disease (CrAD) using the SS and SS II in 287 patients who underwent coronary artery bypass grafting. We based this investigation on the known association between coronary artery disease and CrAD. A significant association was observed between the groups with and without CrAD in terms of SS II values (28.4 ± 9.6 vs 21.4 ± 7.7, respectively; P < .001). A significant difference was also observed when stenosis was classified according to severity as <50%, 50% to 70%, and >70% ( P < .001). The results indicated a positive correlation between the presence and severity of CrAD as SS II increased ( r = 0.187, P = .005). According to the results of multivariate logistic regression analysis, the SS II was an independent predictor of CrAD.

In this study, we aimed to comparatively evaluate the effects of the cell saver device on the drainage, need for blood and blood products, postoperative infection complications and length of intensive care unit stay between patients in whom the device was and was not used. Material and Method: 170 patients who underwent open heart surgery between January 2014 and December 2015 were studied. 85 of these patients constituted the control group (group 1) and 85 were the study group (group 2). The two groups were compared in terms of parameters such as the amount of blood lost postoperatively (drainage) through mediastinal drain, need for blood and blood products, length of intensive care unit stay and postoperative infections. Results: For Group 1 and 2, mediastinal drainage amounts were 581±363 ml and 323±158 ml respectively, need for blood and blood products were 2.28 u and 1.03 u respectively, and lengths of intensive care unit stay were 28.9±1.5 hr versus 29.3±9.1 hr respectively. Conclusion: Since the patient's own blood is used, the cell saver method involves no risk of homologous blood transfusion-related infections or development of any allergic reactions or complications. Moreover, since it can also be used in emergency surgeries, it provides comfort and confidence to the surgeon in patients in critical condition and in those with rare blood groups.
