The prevalence of heart fail- ure continues to increase as the population ages and as therapies f... more The prevalence of heart fail- ure continues to increase as the population ages and as therapies for once fatal cardiovascular diseases improve. Although heart failure is to a large extent prevent- able, once it develops, the natural history is that of progressive deterioration. Despite advances in medical therapy, mortality and morbidity remain high. In patients with heart failure, sudden cardiac death occurs at 6 to 9 times the rate of the general population. For selected patients with chronic systolic heart failure, implantable cardioverter defibrillators and cardiac resynchronization therapy are treatment options in conjunction with optimal medical therapy. Current evidence clearly demonstrates that the appropriate use of these devices improves outcomes.
PURPOSE We sought to determine whether significant differences exist in clinical characteristics ... more PURPOSE We sought to determine whether significant differences exist in clinical characteristics and echocardiographic features between black men and black women who presented to an outpatient heart failure clinic. METHODS A retrospective review of the charts of 114 consecutive patients with systolic dysfunction who presented to a heart failure clinic in Brooklyn from 1999--2003 was performed. The first echocardiogram within six months of presentation to the clinic was used for analysis. The clinical characteristics included are: age, sex, body mass index (BMI) and diabetes status. A significance level of <0.05 was used throughout. SUMMARY Based on the analysis of 108 black men and women, overall, men have a significantly larger left ventricular end-diastolic diameter (LVEDD) compared to women (p<0.006). However, there are several constraints to the current prevention strategies including low condom use, low acceptance of testing, low awareness of vulnerability and more emphasis on treatment. Prevention strategy is redirected towards reducing acquisition of HIV. Pre-exposure prophylaxis or PrEP is the latest groundbreaking innovation in biomedical research in the prevention of HIV transmission.The purpose of this paper is to review preex ding the current guidelines in the use of PreP.

Because heart failure is a preventable disease, the 2002 heart failure guidelines devise a new staging system that expands the continuum of care to encompass prevention.

Our study retrospectively evaluates differential diagnosis and therapy using interferon beta-1b i... more Our study retrospectively evaluates differential diagnosis and therapy using interferon beta-1b in DCM and chronic virus persistence under clinical routine. Between 1999 and 2007 endomyocardial biopsies were taken from the right ventricular septum in DCM. Using PCR and Southern Blot analysis in 53 patients viral genome could be detected. Using immunohistochemical staining for CD2, CD3, CD68, HLA I and HLA II myocardial inflammation was evaluated. In patients with persisting viral genome off label use with interferon beta-1b was initiated (n 5 32). Patients whose health insurance companies did not reimburse the costs for specific therapy represented the control group (n 5 21). 8x10 6 units interferon beta-1b were administered subcutaneously every other day for 24 weeks. Effects of antiviral therapy were evaluated using follow-up of echocardiography, ECG, exercise ECG and NYHA class. Our study population included 37 men and 16 women with a median age of 51 years (20e76). The median of left ventricular ejection fraction was 34% (11e56) and NT-proBNP levels were 592 pg/ml (10-4399). Follow-up period was 1 to 84 months. In 1 patient adenovirus, in 10 patients enterovirus, in 1 patient human herpes virus 6 and in 38 patients parvovirus B19 could be detected. Myocardium of 3 patients showed more than one type of viral genome at the same time. In 26 patients myocardial inflammation could be observed concomitantly. In 22 patients with interferon beta-1b treatment adverse reactions occurred. No severe adverse events of interferon beta-1b therapy could be observed. In our study population we were not able to detect a significant benefit for interferon betae1b treatment for exercise ECG, left ventricular function, fractional shortening, NYHA class and end diastolic diameter at any time of follow-up. Off label use with interferon betae1b in patients suffering from chronic viral DCM is feasible and safe under clinical routine. Promising results from phase II studies could not be confirmed in our study population. The retrospective study design, different follow-up periods and the dominance of parvovirus B19 represent limitations. Results from multicenter, placebo-controlled and randomised studies are required.
