Currently, little is known about Irish hormonal contraceptive trends to date since the 1995 British media contraceptive controversy. The aim of this study was to examine recent trends in contraceptive use in Ireland and to determine the frequency of coprescriptions with important interacting medications. Approximately 40% of the Irish population are prescribed 70% of total medicines under the Irish GMS scheme. Medicines were identified using the WHO Anatomical Therapeutic Chemical (ATC) classification system. Regression analysis was used to examine trends over time. Of all contraceptives dispensed in 2013, oral contraceptives were used the most (74%) and long acting reversible contraceptives (LARCs) the least (7.5%). Fourth generation combined oral contraceptives (COCs) predominated, although a slight significant decline was shown (P < 0.0001). Second and third generation COCs were significantly increasing and decreasing, respectively (P < 0.0001). Progestin-only pills were significantly increasing (P < 0.0001 across age groups). Low rates of contraceptive co-prescribing with important interacting drugs are shown. However, 93.6% of those on enzyme-inducing anti-epileptic medications were co-prescribed ineffective contraception containing <50 μg oestrogen.Irish prescribing trends of second and third generation COCs have remained consistent since 1995. The slow decline in fourth generation COC uptake follows new evidence of an increased risk of venous thromboembolism (VTE) reported in 2011. The low, but increasing, uptake of LARCs is consistent with other countries. Co-prescribing practices involving hormonal contraceptives requires continued vigilance. This study emphasizes the need to optimize coprescribing practices involving hormonal contraceptives and anti-epileptic medications and highlights the need to address the barriers to the currently low uptake of LARC methods in Ireland.

This study aimed to quantify the clinical and economic impacts of poor adherence and to evaluate the potential cost-effectiveness of improving patient adherence by using hypothetical behavioral interventions. Methods: A previously validated Markov microsimulation model was adapted to the Irish setting to estimate lifetime costs and outcomes (fractures and quality-adjusted life-year [QALY]) for three adherence scenarios: no treatment, real-world adherence, and full adherence over 3 years. The real-world scenario employed adherence and persistence data from the Irish Health Services Executive-Primary Care Reimbursement Services pharmacy claims database. We also investigated the cost-effectiveness of hypothetical behavioral interventions to improve medication adherence (according to their cost and effect on adherence). Results: The number of fractures prevented and the QALY gain obtained at real-world adherence levels represented only 57% and 56% of those expected with full adherence, respectively. The costs per QALY gained of real-world adherence and of full adherence compared with no treatment were estimated at €11,834 and €6,341, respectively. An intervention to improve adherence by 25% would result in an incremental cost-effectiveness ratio of €11,511 per QALY and €54,182 per QALY, compared with real-world adherence, if the intervention cost an additional €50 and €100 per year, respectively. Discussion: Poor adherence with osteoporosis medications results in around a 50% reduction in the potential benefits observed in clinical trials and a doubling of the cost per QALY gained from these medications. Depending on their costs and outcomes, programs to improve adherence have the potential to be an efficient use of resources.

Background: Increasing use of the Quality-Adjusted Life-Year to inform resource allocation decisi... more Background: Increasing use of the Quality-Adjusted Life-Year to inform resource allocation decision-making has highlighted the importance of relating clinical and health-related quality of life (HRQoL) outcomes in multiple sclerosis (MS) patients. Objective and Methods: To investigate the relationship between the Expanded Disability Status Scale (EDSS) and HRQoL utility, using the 5-level EQ-5D (EQ-5D-5L). The discriminatory power of the EQ-5D-5L was assessed using Shannon’s indices. Results: A linear decline in utility was observed with changes in EDSS score from 0 to 6, after which point the relationship exhibited greater variability. Mean utility values ranged from −0.22 at EDSS 9 to 0.88 at EDSS 0. We found that the discriminative capacity of the EQ-5D-5L was considerably lower for the domains self-care and anxiety/depression, compared with other health-related domains. There are few data available to health care providers regarding the costs of treating patients wi... more There are few data available to health care providers regarding the costs of treating patients with psoriasis, and specifically the cost of phototherapy. As narrowband UVB (TL-01) has now become an established therapy for patients with psoriasis requiring phototherapy, we determined the annual cost of delivering TL-01 treatment in a university hospital. The costing evaluation was from a hospital perspective and the strategy used was a microcosting detailed collection of resources used. The annual cost of TL-01 treatment in our teaching hospital was 53,555.00 euros. Staffing accounted for 70% of the cost. The average individual costs were 325.00 euros (range: 57.20-972.40). These costs are significant but remain less expensive than inpatient treatment.

Investigating the pharmacoeconomic impact of any diagnostic or therapeutic intervention in the Ir... more Investigating the pharmacoeconomic impact of any diagnostic or therapeutic intervention in the Irish healthcare setting is currently compromised by the lack of detailed cost data. Consequently, we conducted a number of microcosting studies in the areas of acute myocardial infarction, cardiac failure and HIV, from the hospital perspective. The results of these microcosting studies were compared with the costing estimates assigned to hospital admissions, based on the diagnosis-related group system. Differences ranged from-9 to 66%. It was conducted that the diagnosis related group system is a useful estimate of costs for patient admissions in the absence of detailed cost of illness data. However, supplementary costing studies should be performed for certain therapeutic areas-particularly those where investigation and/or treatment costs are high.

This study estimated the potential savings in Ireland in 2003 if a system of generic substitution... more This study estimated the potential savings in Ireland in 2003 if a system of generic substitution were introduced, under the two main Community Drug Schemes (General Medical Services, GMS, and Drugs Payment, DP). The GMS and DP schemes accounted for 82% of state expenditure on the Community Drug Schemes in 2003. Twenty one per cent of prescription items on the GMS scheme and 23% of items on the DP scheme were dispensed as a proprietary preparation when a generic equivalent was available. This study estimated the potential savings in Ireland in 2003 if a system of generic substitution... more This study estimated the potential savings in Ireland in 2003 if a system of generic substitution were introduced, under the two main Community Drug Schemes (General Medical Services, GMS, and Drugs Payment, DP). The GMS and DP schemes accounted for 82% of state expenditure on the Community Drug Schemes in 2003. Twenty one per cent of prescription items on the GMS scheme and 23% of items on the DP scheme were dispensed as a proprietary preparation when a generic equivalent was available. Comparison of these results with a similar analysis of 2001 data illustrates the potential for an increase in savings over time.
