Hintergrund und Ziel: Die linksventrikulare (LV) Dilatation und das myokardiale Remodeling sind c... more Hintergrund und Ziel: Die linksventrikulare (LV) Dilatation und das myokardiale Remodeling sind charakteristische Merkmale fur eine Herzinsuffizienz auf der Grundlage einer dilatativen Kardiomyopathie (DCM). Das interstitielle Kollagen ist fur die LV-Integritat und die Funktion essentiell. Es wird hingegen vermutet, das eine Kollagendegradation durch Kollagenasen (z. B. Matrixmetalloproteinasen (MMP)) zur Ventrikeldilatation beitragt. In der vorliegen Arbeit wurden Serummarker des Kollagenmetabolismus untersucht, mit klinischen Parametern korreliert und auf ihre prognostische Wertigkeit hin untersucht. Methoden und Ergebnisse: Bei 43 Patienten mit DCM und 47 altersentsprechenden Kontrollpersonen wurden zum Zeitpunkt t 0 die Serumspiegel von MMP-1, -3 und -9, dem Inhibitor TIMP-1 sowie der Marker fur die Kollagendegradation (ICTP) und die -synthese (PICP) mittels ELISA beziehungsweise RIA bestimmt. The prevalence of left ventricular thrombus in patients with dilated cardiomyopathy has been esti... more The prevalence of left ventricular thrombus in patients with dilated cardiomyopathy has been estimated at 29-36% in relatively small studies, and is thought to contribute to systemic embolization. Methods We evaluated the prevalence of LV thrombus on 2D echocardiography in the WATCH trial, a multicenter prospective clinical trial of warfarin and antiplatelet agents in dilated cardiomyopathy. Ethos were performed prospectively in 1343 patients with NYHA class II-IV CHF and EF<35% who were in sinus rhythm (age 63 * 11 years, EF 29 f 6%, LV end-diastolic volume 194 271 ml). Patients with recent (~3 mos) myocardial infarction were excluded. Results: LV apical mural thrombus was noted in 26 patients (2.1%), and rated as probable or definite in 22, and suspected in 6. The presence of LV thrombus was associated with younger age (59 * 10 vs 62 * 11 years, p=. 029), lower EF (24 * 6 vs 29 t 6%, p=, 001). higher regional wall motion score (2.2 + .33 YS 1 .Ql f .39, p = .OOOOl) higher velocity of early diastolic filling (65 + 22 vs 77 * 26 cmlsec, p=. The presence of LV thrombus was not related to race, gender, the duration of CHF, prior myocardial infarction, coronary revascularization, or angina, diastolic function pattern, or RV systolic pressure. Conclusions: The prevalence of left ventricular thrombus in chronic dilated cardiomyopathy is lower than that previously reported, possibly due to case selection and to improved echocardiographic techniques which lessen near-field artifact. Left ventricular (LV) dilation and myocardial remodelling are hallmarks of heart failure in idiop... more Left ventricular (LV) dilation and myocardial remodelling are hallmarks of heart failure in idiopathic dilated cardiomyopathy (DCM). Interstitial collagen is essential for LV integrity and function while degradation of collagen by collagenases, especially matrix-metalloproteinases (MMPs), are suggested to contribute to ventricular dilation. In the present study, serological markers of collagen metabolism were investigated. Serum levels of MMP-1 and its inhibitor (TIMP-1), the markers for collagen degradation type I (collagen carboxyterminal telopeptide (ICTP)) and synthesis (carboxyterminal propeptide of type I procollagen (PICP)) were quantified by ELISA and RIA of 43 patients with DCM and 47 age-matched control subjects. Free MMP-1 serum concentration was significantly increased in the DCM group (5.29+/-0.83 vs. 2.22+/-0.29 ng/ml; P=0.01) as well as the free TIMP-1 concentration (206.54+/-12.65 vs. 181.44+/-8.55 ng/ml; P=0.05). The free MMP-1/TIMP-1-ratio was higher in DCM than in the control group (0.030+/-0.005 vs. 0.012+/-0.001; P=0.01). ICTP was significantly increased (7.60+/-1.21 vs. 3.44+/-0.19 microg/l; P&amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;lt;0.001). PICP was not significantly increased (125.29+/-8.93 microg/l vs. 113.11+/-5.47 microg/l; P=n.s.). Free MMP-1 and MMP-1/TIMP-1-ratio correlated with LV end diastolic diameter [cm/m(2) body surface area (BSA)] (r=0.28; P=0.03 and r=0.34; P=0.01, respectively) as well as with cardiac index (CI) (r=-0.32; P=0.04 and r=-0.33; P=0.04, respectively) in patients with DCM. Serum markers of collagen degradation are elevated and might be valuable markers for progression of LV dilation in patients with DCM.
