The aim of any documentation is to make the documented facts available. In most cases, the focus of such documentations is the gathering of data itself instead of making existing data available. The requirements for a modern wound documentation are far beyond traditional paper-based (even if converted into computer-based forms) documentation. The required medical features for wound documentation have already been well defined by experts. As stated by [8, 15], from a medical point of view, the following requirements are essential to ensure a modern and up-to-date wound documentation:

Electronic health records (EHRs) have been successfully used in data science and machine learning... more Electronic health records (EHRs) have been successfully used in data science and machine learning projects. However, most of these data are collected for clinical use rather than for retrospective analysis. This means that researchers typically face many different issues when attempting to access and prepare the data for secondary use. We aimed to investigate how raw EHRs can be accessed and prepared in retrospective data science projects in a disciplined, effective, and efficient way. We report our experience and findings from a large-scale data science project analyzing routinely acquired retrospective data from the Kepler University Hospital in Linz, Austria. The project involved data collection from more than 150,000 patients over a period of 10 years. It included diverse data modalities, such as static demographic data, irregularly acquired laboratory test results, regularly sampled vital signs, and high-frequency physiological waveform signals. Raw medical data can be corrupted in many unexpected ways that demand thorough manual inspection and highly individualized data cleaning solutions. We present a general data preparation workflow, which was shaped in the course of our project and consists of the following 7 steps: obtain a rough overview of the available EHR data, define clinically meaningful labels for supervised learning, extract relevant data from the hospital's data warehouses, match data extracted from different sources, deidentify them, detect errors and inconsistencies therein through a careful exploratory analysis, and implement a suitable data processing pipeline in actual code. Only few of the data preparation issues encountered in our project were addressed by generic medical data preprocessing tools that have been proposed recently. Instead, highly individualized solutions for the specific data used in one's own research seem inevitable. We believe that the proposed workflow can serve as a guidance for practitioners, helping them to identify and address potential problems early and avoid some common pitfalls.

This study aims to provide prediction models for discharge versus admission for ward observation or intensive care, and 30 day-mortality for patients triaged with the Manchester Triage System. Methods This is a single-centre, observational, retrospective cohort study from data within ten minutes of patient presentation at the interdisciplinary emergency department of the Kepler University Hospital, Linz, Austria. We trained machine learning models including Random Forests and Neural Networks individually to predict discharge versus ward observation or intensive care admission, and 30 day-mortality. For analysis of the features’ relevance, we used permutation feature importance. Results A total of 58323 adult patients between 1 December 2015 and 31 August 2020 were included. Neural Networks and Random Forests predicted admission to ward observation with an AUC-ROC of 0.842 ± 0.00 with the most important features being age and chief complaint. For admission to intensive care, the models had an AUC-ROC of 0.819 ± 0.002 with the most important features being the Manchester Triage category and heart rate, and for the outcome 30 day-mortality an AUC-ROC of 0.925 ± 0.001. The most important features for the prediction of 30 day-mortality were age and general ward admission. Conclusion Machine learning can provide prediction on discharge versus admission to general wards and intensive care and inform about risk on 30 day-mortality for patients in the emergency department.

This paper presents the results of the research project BurnCase in the field of realistic and an... more This paper presents the results of the research project BurnCase in the field of realistic and anatomically correct deformations of 3D models of the human body. The project goal is to develop a software system named BurnCase 3D, which supports and enhances the documentation and diagnosis of human burn injuries. The medical treatment of burn victims strongly depends on size, depth, degree and location of the burnt skin. The size of the affected region is usually expressed as a percentage of the total body surface area (TBSA). Standardized 2D charts (e.g. Lund and Browder, Rule-Of-Nines, etc.) help to determine the percentage of the burnt surface area in relation to the total body surface area. However, body proportions highly influence the distribution of body surface area along the body [Livingston and Lee, 2000]. Thus, standard charts can only give rough approximations of the burnt surface area compared to the real size of an injury on a specific patient. The software system BurnCase 3D will enhance this commonly applied 2D approximation process by introducing a 3D model of the patient's body. This 3D model provides a higher accordance to the real patient's surface area than any 2D chart does and allows determining the burnt surface areas more exactly. BurnCase 3D is based on an extendable library of currently 7 standard models representing different sex, age and body shape. In order to meet the physical constitution of the real patient, the best fitting model is chosen and has to be adapted according to the patient's height and weight. There exist several possibilities of adapting a 3D model to these parameters. This paper describes the three methods of body adaptations that are realized in the software system BurnCase 3D based on the thesis of Doris Siegl ([Siegl, 2003]).

Since the introduction of applications (apps) for smartphones, the popularity of medical apps has... more Since the introduction of applications (apps) for smartphones, the popularity of medical apps has been rising. The aim of this review was to demonstrate the current availability of apps related to burns on Google's Android and Apple's iOS store as well as to include a review of their developers, features, and costs. A systematic online review of Google Play Store and Apple's App Store was performed by using the following search terms: "burn," "burns," "thermal," and the German word "Verbrennung." All apps that were programmed for use as medical apps for burns were included. The review was performed from 25 February until 1 March 2014. A closer look at the free and paid calculation apps including a standardized patient was performed. Four types of apps were identified: calculators, information apps, book/journal apps, and games. In Google Play Store, 31 apps were related to burns, of which 20 were calculation apps (eight for estimating the total body surface area (TBSA) and nine for total fluid requirement (TFR)). In Apple's App Store, under the category of medicine, 39 apps were related to burns, of which 21 were calculation apps (19 for estimating the TBSA and 17 for calculating the TFR). In 19 out of 32 available calculation apps, our study showed a correlation of the calculated TFR compared to our standardized patient. The review demonstrated that many apps for medical burns are available in both common app stores. Even free available calculation apps may provide a more objective and reproducible procedure compared to manual/subjective estimations, although there is still a lack of data security especially in personal data entered in calculation apps. Further clinical studies including smartphone apps for burns should be performed.

We created an ontology-based data acquisition infrastructure which is able to store data of almos... more We created an ontology-based data acquisition infrastructure which is able to store data of almost arbitrary structure and can be set up for a certain domain of application within hours. An ontology editor helps the domain expert to define and maintain the domain specific ontology. Based on the user-defined ontology, a web-based data acquisition system and an ETL data import interface are automatically created at runtime. Furthermore, rules for semantic data plausibility can be established in the ontology to provide semantic data quality for subsequent processing of the collected data. After a comprehensive requirement analysis we decided to use a special meta model instead of standard OWL ontologies. In this paper, we describe our metamodel and the reason for not using OWL in our case in detail as well as we present the infrastructure and the project it is currently used for.

Unfortunately, the commonly applied methods of estimation have revealed inaccuracies, which are mostly related to human error. To calculate the %TBSA (quotient), it is necessary to divide the burned surface area (Burned BSA) (numerator in cm2) by the total body surface area (Total BSA) (denominator in cm2). By using everyday objects (eg. credit cards, smartphones) with well-defined surface areas as reference for estimations of Burned BSA on the one hand and established formulas for Total BSA calculation on the other (eg. Mosteller), we propose an approximation method to assess %TBSA more accurately than the established methods. Since the introduction of applications (apps) for smartphones, the popularity of medical apps has... more Since the introduction of applications (apps) for smartphones, the popularity of medical apps has been rising. The aim of this review was to demonstrate the current availability of apps related to burns on Google's Android and Apple's iOS store as well as to include a review of their developers, features, and costs. A systematic online review of Google Play Store and Apple's App Store was performed by using the following search terms: "burn," "burns," "thermal," and the German word "Verbrennung." All apps that were programmed for use as medical apps for burns were included. The review was performed from 25 February until 1 March 2014. A closer look at the free and paid calculation apps including a standardized patient was performed. Four types of apps were identified: calculators, information apps, book/journal apps, and games. In Google Play Store, 31 apps were related to burns, of which 20 were calculation apps (eight for estimating the total body surface area (TBSA) and nine for total fluid requirement (TFR)). In Apple's App Store, under the category of medicine, 39 apps were related to burns, of which 21 were calculation apps (19 for estimating the TBSA and 17 for calculating the TFR). In 19 out of 32 available calculation apps, our study showed a correlation of the calculated TFR compared to our standardized patient. The review demonstrated that many apps for medical burns are available in both common app stores. Even free available calculation apps may provide a more objective and reproducible procedure compared to manual/subjective estimations, although there is still a lack of data security especially in personal data entered in calculation apps. Further clinical studies including smartphone apps for burns should be performed.

We created an ontology-based data acquisition infrastructure which is able to store data of almos... more We created an ontology-based data acquisition infrastructure which is able to store data of almost arbitrary structure and can be set up for a certain domain of application within hours. An ontology editor helps the domain expert to define and maintain the domain specific ontology. Based on the user-defined ontology, a web-based data acquisition system and an ETL data import interface are automatically created at runtime. Furthermore, rules for semantic data plausibility can be established in the ontology to provide semantic data quality for subsequent processing of the collected data. After a comprehensive requirement analysis we decided to use a special meta model instead of standard OWL ontologies. In this paper, we describe our metamodel and the reason for not using OWL in our case in detail as well as we present the infrastructure and the project it is currently used for.

Estimation of the total body surface area burned (%TBSA) following a burn injury is used in deter... more Estimation of the total body surface area burned (%TBSA) following a burn injury is used in determining whether to transfer the patient to a burn center and the required fluid resuscitation volumes. Unfortunately, the commonly applied methods of estimation have revealed inaccuracies, which are mostly related to human error. To calculate the %TBSA (quotient), it is necessary to divide the burned surface area (Burned BSA) (numerator in cm2) by the total body surface area (Total BSA) (denominator in cm2). By using everyday objects (eg. credit cards, smartphones) with well-defined surface areas as reference for estimations of Burned BSA on the one hand and established formulas for Total BSA calculation on the other (eg. Mosteller), we propose an approximation method to assess %TBSA more accurately than the established methods.
