Periprosthetic joint infection (PJI) is a devastating complication following total hip replacemen... more Periprosthetic joint infection (PJI) is a devastating complication following total hip replacement (THR). Management of PJI depends upon proper diagnosis and correct treatment, both of which are challenging conditions. Yet in the absence of a true gold standard, diagnosis is the result of a combination of clinical suspicion, serological tests and culture results. Treatment options include irrigation and debridement (I &D) with retention of implants for acute infections and exchange arthroplasty either as a 1-stage or 2-stage procedure for late infections. A multidisciplinary team approach is necessary in the treatment of patients with PJI.

Total hip arthroplasty in cases of dwarfism may be a demanding procedure, as the small-sized join... more Total hip arthroplasty in cases of dwarfism may be a demanding procedure, as the small-sized joints are difficult to replace with standard prostheses, even more so in the presence of dysplastic hips. The authors report a case of bilateral customized total hip arthroplasty performed in a young patient with proportionated dwarfism with bilateral hip dysplasia. To ensure sufficient stabilization of the cup, roof bone grafting was performed. The custom-made acetabular component was a trabecular acetabular shell manufactured using selective laser melting technology to ensure an optimal porous structure to enhance primary stability and bone ingrowth. Selective laser melting may be a promising alternative technology for manufacturing highly porous customized acetabular components for complex joint reconstruction procedures in which enhanced osseointegration ability is particularly required. In the adolescent, the trochanter minor fracture occurs in pathological traction of the iliopsoas... more In the adolescent, the trochanter minor fracture occurs in pathological traction of the iliopsoas tendon, the minor fragment dislocates cranially. The therapy is conservative, the fragment remains dislocated cranially, where it heals with the femur. A limited hip function in terms of impingement symptoms is not known. We present the case of a 14-year-old boy: the lesion occurred during sports activities (running) through hyperextension and rotation trauma. The treatment was conservative, functional limitations did not remain.

Is... more Is the cam deformity of the proximal femur the consequence of ephiphyseolysis capitis femoris? Is the same pathological mechanism at the origin of both? We report the clinical case of a young boy, with a slipped femoral epiphysis on the left side and who developed a Cam deformity on the contralateral right healthy hip which has undergone prophylactic screw fixation.

Pathologies related to this disease are: scoliosis, short stature, eye problems, articular deformities. We report a case of tarda form. The main problem was habitual and later fixed dislocation of both patellae together with valgus deformities of the knees. We describe the surgical procedures to gain reduced patellae and correction of the mechanical axis. (www.actabiomedica.it)
