o modelo visava aumentar a captação dos casos e diminuir o abandono de tratamento. São também des... more o modelo visava aumentar a captação dos casos e diminuir o abandono de tratamento. São também descritos 1) a metodologia do planejamento conjunto com pactuação das metas e recursos necessários; 2) o treinamento das equipes multiprofissionais e dos agentes de saúde; 3) a implantação da supervisão da administração do tratamento; 4) o sistema de visita domiciliar e de busca ativa de faltosos, de contatos e de sintomáticos respiratórios; 5) o sistema de registro com controle de qualidade e 6) as estratégias para o envolvimento da comunidade. Resultados: com a implantação do modelo, de novembro/99 a abril de 2000, houve aumento de 85,7% na captação dos casos novos; de 181,8% na captação dos sintomáticos respiratórios e o abandono foi reduzido a zero. Conclui-se que a busca da excelência no controle da tuberculose passa pela integração dos vários atores responsáveis, demandando esforço e uma metodologia apropriada em torno de objetivos comuns pactuados. Este pode ser o caminho para que o demonstrado na UISHL possa se reproduzir em outros locais do país.

Bacille Calmette-Guérin (BCG) efficacy against pulmonary disease is highly variable; until very r... more Bacille Calmette-Guérin (BCG) efficacy against pulmonary disease is highly variable; until very recently there was no evidence of protection after 10 years. In the control arm of a trial of efficacy of revaccination of schoolchildren in Brazil we found substantial protection (39%; 95%CI 9-58) of neonatal BCG against all forms of tuberculosis (TB) 15-20 years after vaccination, much longer than previously believed. This confirms recent findings from an earlier trial, and must be considered in the design of trials of new TB vaccines and in policy decisions based on assumed lack of neonatal BCG protection with time.

To compare estimates of Mycobacterium tuberculosis infection prevalence obtained using traditional tuberculin skin test (TST) criteria and dual skin test (DST) data. A total of 1070 schoolchildren received DST with tuberculin and four environmental mycobacteria sensitins. Responses were classified as sensitin-dominant, tuberculin-dominant or non-dominant. Positive predictive values (PPV) were defined using 'narrow' and 'wide' standards based on DST responses. These predictive values were derived for each category of tuberculin indurations, and were used to calculate the prevalence estimates. Using DST data, the estimates of M. tuberculosis prevalence for scar-negative children were 7.4% (M. avium) and 7.8% (M. scrofulaceum) using the 'narrow' standard, and 16.9% (M. avium) and 15.2% (M. scrofulaceum) using the 'wide' standard. Rio de Janeiro (RJ) State prisons, where tuberculosis (TB) is highly endemic. To measure TB preva... more Rio de Janeiro (RJ) State prisons, where tuberculosis (TB) is highly endemic. To measure TB prevalence, identify risk factors and ascertain the most appropriate screening method among inmates of the RJ prisons. Systematic chest X-rays (CXRs) were performed in 1696 male inmates of three RJ prisons. Inmates were selected for sputum examination and culture if their CXRs showed evidence of any pulmonary, pleural or mediastinal abnormality. TB diagnosis was based on bacteriological results or, if bacteriological results were negative, on response to TB treatment. TB prevalence was 2.7% (46/1696), and 32/46 cases (69%) were bacteriologically confirmed, including 19 smear-positive cases. CXR lesions were extensive in 43% of cases. In the logistic regression model, TB-associated variables were being illiterate (adjusted OR 2.10, 95%CI 1.02-4.34), cough >or=3 weeks (aOR 2.85, 95%CI 1.54-5.27), history of TB treatment (aOR 3.61, 95%CI 1.76-7.39), and living in Rio City suburbs (aOR 4.54, 9...
