ABSTRACT Oxidatively modified LDL (OxLDL) and HDL (OxHDL) are present in the intima of atheromato... more ABSTRACT Oxidatively modified LDL (OxLDL) and HDL (OxHDL) are present in the intima of atheromatous plaques and in plasma. While prothrombotic activity of OxLDL mediated by platelet CD36 is established, the role of oxHDL in thrombosis is not known. We now report that OxHDL, but not native HDL, is a potent inhibitor of platelet activation and aggregation induced by physiological agonists. OxHDL inhibited key platelet activation events including the activation of integrin alpha IIb beta 3, alpha-granule secretion and dense-granule release. Oxidized lipoproteins are ligands for scavenger receptors CD36 and SR-BI, both of which are expressed on platelets. Studies using CD36–/– or SR-BI–/– platelets and blocking antibodies demonstrated that the anti-thrombotic activity of OxHDL is mediated exclusively by platelet SR-BI. Isolated SR-BI–/– platelets were more responsive to agonists than WT platelets supporting a physiological role for platelet SR-BI. Platelets from eNOS–/–, Akt-1–/– and Akt-2–/– mice were as sensitive to oxHDL as WT platelets, indicating that the OxHDL effect is mediated by a pathway different from the eNOS/Akt pathway. These novel findings demonstrate that HDL, upon oxidation, acquires an anti-thrombotic activity mediated by platelet SR-BI and suggest that oxHDL may counterbalance pro-thrombotic activity of oxLDL. Background: This article describes a patient with a history of two early miscarriages. She presen... more Background: This article describes a patient with a history of two early miscarriages. She presented with menorrhagia and a recurrence of multiple fibroids. Laparotomy and myomectomy were performed and were complicated by a myometrial abscess. The diagnosis and management of such a rare complication are described. Case: A 39year-old African American female (gravida 3, para 1) presented with a history of two early miscarriages, menorrhagia, and a recurrence of multiple fibroids. An ultrasound (US) scan revealed an enlarged uterus with at least six uterine fibroids. Laparotomy and myomectomy were performed. Four weeks later, this patient presented with complaints of pelvic pain, nausea, vomiting, constipation, and a fever of 1 day's duration. A US scan revealed a slightly enlarged uterus caused by a possible infected hematoma. The patient was admitted to the hospital and she was started on intravenous antibiotics after a blood culture was obtained. A computed tomography (CT) scan of her abdomen and pelvis confirmed the diagnosis of a myometrial abscess. This patient underwent CT scan-guided placement of a ''pigtail'' catheter in the myometrial abscess for continuous drainage. The result of the culture of the pus obtained from the abscess revealed the presence of multiple bacteria. The pigtail catheter was removed once there was no drainage 2 weeks after its initial insertion. Results: Two months after her discharge, a transvaginal US scan revealed that this patient's condition was essentially normal. Conclusions: This case report describes how a minimally invasive technique was used to manage a myometrial abscess, which is a complication of myomectomy. This report also illustrates the value of transvaginal US scanning for diagnosing such a pathology, and the roles US plays during management and follow-up. (J GYNECOL SURG 30:240

Background: Submucous fibroids can lead to menorrhgia, infertility, recurrent pregnancy loss, and... more Background: Submucous fibroids can lead to menorrhgia, infertility, recurrent pregnancy loss, and obstetric complications. The management of multiple submucous fibroids in patients who would like to preserve their reproductive function can be a challenge. The aim of this report is to discuss the management dilemma of a patient who presented with multiple (more than 20) submucous fibroids. Case: The patient was a 33-year-old Caucasian woman who had an initial complaint of menometrorrhagia and secondary infertility, which led to a diagnostic hysteroscopy. Multiple submucosal fibroids were noted; no other etiology for infertility was identified. She subsequently underwent hysteroscopic resection of several fibroids. Postoperative hyterosalpingogram demonstrated intrauterine scarring, unilateral tubal blockage, and several remaining fibroids. She did, however, conceive with intrauterine insemination. Unfortunately, this pregnancy ended in an early spontaneous abortion of twins. After being counseled on her options, including the possibility of needing a surrogate uterus, she elected to have an abdominal resection of the remaining fibroids. During this procedure, several steps were taken to restore the uterine cavity to a normal shape. After allowing her uterus to heal, she achieved pregnancy through in vitro fertilization. At 29 weeks of gestation, she had preterm premature rupture of membranes and, subsequently, had a cesarean section at 34 weeks of gestation. She had a viable 6 pound 1 ounce baby boy. At the time of her cesarean section, only one submucosal fibroid was identified. Conclusion: We present a unique case of multiple sub-mucous fibroids that failed hysteroscopic management and were subsequently treated successfully with hysterotomy, myomectomy, and uterine reconstructive surgery. Based on our experience with this case, we recommend hysterotomy and myomectomy for management of multiple sub-mucous fibroids from the outset.

The mechanisms underlying the impairment of endothelium-mediated vasorelaxation induced by dietar... more The mechanisms underlying the impairment of endothelium-mediated vasorelaxation induced by dietary hypercholesterolemia and the mechanisms of restoration of endothelial function following reintroduction of low cholesterol diet were evaluated. Feeding rats with high cholesterol diet induced hypercholesterolemia and high blood pressure. This was associated with reduced vasorelaxation in response to acetylcholine, isoproterenol, and adenosine. At the same time, exaggerated contractile responses to serotonin and phenylephrine were observed. Reintroduction of a normal diet to cholesterol fed rats resulted in significant normalization of blood pressure, serum lipid profile, relaxation and contractile responses. The contributions of endothelial derived relaxing factors (EDRF), endothelial derived contractile factors (EDCFs)/prostanoids, and endothelial derived hyperpoalrising factor (EDHF) to the vasorelaxation in each group of animals were assessed. EDCFs constricting activity was increased in both cholesterol fed groups as compared to the control group. EDRF and EDHF were found to be the primary factors involved in the regulation of endothelium-mediated responsiveness. In control animals, EDRF was responsible for 70-90% of relaxation, depending on the agonist used. In cholesterol fed animals, EDRF was significantly reduced while EDHF was maintained or enhanced showing that EDHF had a significant role in maintaining the endothelial responses. Importantly, the restoration of vasorelaxation following reintroduction of normal diet was mediated not only by improvement of EDRF-dependent relaxation, but also to a significant extent by a further increase in EDHF-mediated relaxation. Taken together, the data showed that EDRF was attenuated during hypercholesterolemia and dietary interventions with low fat content restored these responses. However, EDHF-mediated responses were not reduced by hypercholesterolemia and subsequently improved their function after application of low cholesterol diet. The results implicate EDHF-mediated relaxation is also an important mechanism for restoration of endothelial function upon application of dietary restrictions for reduction of serum cholesterol level.

The population over the age of five years had higher rate of amoebal infection compared to less than that of a five-year-old population; however, both age groups had similar patterns of amebiasis during January 1999 to June 2001. To investigate misdiagnosis of amebiasis, 226 patients (passive cases) in three hospitals and 616 individuals (active cases) from three different localities in Kilimanjaro were examined. In passive cases, the prevalences of Entamoeba histolytica and Entamoeba dispar were 1% and 7.3%, respectively. Among active cases, 1% were infected with E. histolytica, and 15% were infected with E. dispar. There were no significant differences in amoebal infection between the male and female populations. Background: There is a growing tendency toward application of human amniotic membrane (HAM) as a ... more Background: There is a growing tendency toward application of human amniotic membrane (HAM) as a biologic substitute in various tissue injuries where a significant tissue loss is a matter of concern. In gastrointestinal injuries especially duodenal ones, some potential limitations in current surgical techniques contribute to not fully acceptable healing outcomes. Thus, this study was carried out to assess repair with HAM patch for duodenal defect in comparison with simple duodenoraphy in an animal model (dog). Materials and Methods: A total of 15 male German shepherd dogs weighing 23-27 kg were randomly divided into two groups. Group A with 10 dogs, which were a candidate for duodenal repair by amniotic membrane patch and Group B consisted of 5 dogs perform simple duodenorraphy. A precise control was made to match all conditions except surgical technique. Macroscopic and microscopic features of the healed duodenal lumen in both groups were recorded. Results: Gross evaluation revealed no difference in luminal diameter in both groups. Statistical analysis of duodenal diameter between both groups after operation also showed no significant difference (P v = 0.789). Histological assessment indicated less inflammation with better wound healing in Group A. Conclusion: It seems that repairing duodenal wall defect with HAM would result in better histological outcomes compared with what is seen in simple duodenoraphy in animal models. However, there is no significant difference regarding surgical findings.

Diagnosis and appropriate management are critical in preventing morbidity and death. Case Description: Four cases of interstitial pregnancy are presented. Diagnostic laparoscopy followed by laparotomy and cornuostomy with removal of products of conception was performed in 1 case. Laparoscopic cornuostomy and removal of products of conception were performed in the subsequent 3 cases with some modifications of the technique. Subsequent successful reproductive outcomes are also presented. Discussion: Progressively conservative surgical measures are being used to treat interstitial pregnancy successfully, with no negative impact on subsequent pregnancies.

This work was conducted (i) to measure the electrical conductance of dry and imbibing seeds, (ii)... more This work was conducted (i) to measure the electrical conductance of dry and imbibing seeds, (ii) to monitor the percentage germination and spread of germination of these species at varying temperature conditions and (iii) to find the optimal germination conditions so that some biochemical and biomolecular studies could be carried out in near future. Seeds of different plant species of Cholistan desert such as Capparis decidua, Salvadora oleiodes and Prosopis cineraria were allowed to germinate at various temperature regimes and their percent age germination was monitored. Results show that optimal germination temperatures for C. decidua is 20EC (90% germination in 8 days) and for P. cineraria (65% in 10 days) and S. oleiodes (85% in 4 days) is 30°C. Imbibition of P. cineraria in sand at 25EC has been effective to get 80 percent seeds germinated. Electrical conductance (μS/cm/10 seeds) measured during early hours/days of imbibition has been correlated with the increase in fresh weights of imbibing seeds, though there is decrease in the rate of electrolytes leakage (μS/cm/hour/10 seeds) with increase in imbibitional time. Work is in progress on the biochemical aspects of these early hours/days of seed germination.

Unreinforced concrete block masonry piers have been tested for lateral strength before and after retrofitting. Welded mesh and injection of cement grout techniques have been used for piers retrofitting. The analysis of experimental results demonstrates that proper retrofitting can decrease the risk to concrete block masonry buildings in future scenario earthquakes. Retrofitting increases not only the overall strength of unreinforced masonry piers but also the ductility.

The control group included 196 consecutive patients with normal endometrial cavity on hysteroscopy who underwent a total of 369 cycles of IVF/ET (Group 2). The mechanisms underlying the impairment of endothelium-mediated vasorelaxation induced by dietar... more The mechanisms underlying the impairment of endothelium-mediated vasorelaxation induced by dietary hypercholesterolemia and the mechanisms of restoration of endothelial function following reintroduction of low cholesterol diet were evaluated. Feeding rats with high cholesterol diet induced hypercholesterolemia and high blood pressure. This was associated with reduced vasorelaxation in response to acetylcholine, isoproterenol, and adenosine. At the same time, exaggerated contractile responses to serotonin and phenylephrine were observed. Reintroduction of a normal diet to cholesterol fed rats resulted in significant normalization of blood pressure, serum lipid profile, relaxation and contractile responses. The contributions of endothelial derived relaxing factors (EDRF), endothelial derived contractile factors (EDCFs)/prostanoids, and endothelial derived hyperpoalrising factor (EDHF) to the vasorelaxation in each group of animals were assessed. EDCFs constricting activity was increased in both cholesterol fed groups as compared to the control group. EDRF and EDHF were found to be the primary factors involved in the regulation of endothelium-mediated responsiveness. In control animals, EDRF was responsible for 70-90% of relaxation, depending on the agonist used. In cholesterol fed animals, EDRF was significantly reduced while EDHF was maintained or enhanced showing that EDHF had a significant role in maintaining the endothelial responses. Importantly, the restoration of vasorelaxation following reintroduction of normal diet was mediated not only by improvement of EDRF-dependent relaxation, but also to a significant extent by a further increase in EDHF-mediated relaxation. Taken together, the data showed that EDRF was attenuated during hypercholesterolemia and dietary interventions with low fat content restored these responses. However, EDHF-mediated responses were not reduced by hypercholesterolemia and subsequently improved their function after application of low cholesterol diet. The results implicate EDHF-mediated relaxation is also an important mechanism for restoration of endothelial function upon application of dietary restrictions for reduction of serum cholesterol level.

The population over the age of five years had higher rate of amoebal infection compared to less than that of a five-year-old population; however, both age groups had similar patterns of amebiasis during January 1999 to June 2001. To investigate misdiagnosis of amebiasis, 226 patients (passive cases) in three hospitals and 616 individuals (active cases) from three different localities in Kilimanjaro were examined. In passive cases, the prevalences of Entamoeba histolytica and Entamoeba dispar were 1% and 7.3%, respectively. In gastrointestinal injuries especially duodenal ones, some potential limitations in current surgical techniques contribute to not fully acceptable healing outcomes. Thus, this study was carried out to assess repair with HAM patch for duodenal defect in comparison with simple duodenoraphy in an animal model (dog). Materials and Methods: A total of 15 male German shepherd dogs weighing 23-27 kg were randomly divided into two groups. Group A with 10 dogs, which were a candidate for duodenal repair by amniotic membrane patch and Group B consisted of 5 dogs perform simple duodenorraphy. A precise control was made to match all conditions except surgical technique. Macroscopic and microscopic features of the healed duodenal lumen in both groups were recorded. Results: Gross evaluation revealed no difference in luminal diameter in both groups. Statistical analysis of duodenal diameter between both groups after operation also showed no significant difference (P v = 0.789). Histological assessment indicated less inflammation with better wound healing in Group A. Conclusion: It seems that repairing duodenal wall defect with HAM would result in better histological outcomes compared with what is seen in simple duodenoraphy in animal models. However, there is no significant difference regarding surgical findings.

Diagnosis and appropriate management are critical in preventing morbidity and death. Case Description: Four cases of interstitial pregnancy are presented. Diagnostic laparoscopy followed by laparotomy and cornuostomy with removal of products of conception was performed in 1 case. Laparoscopic cornuostomy and removal of products of conception were performed in the subsequent 3 cases with some modifications of the technique. Subsequent successful reproductive outcomes are also presented. Discussion: Progressively conservative surgical measures are being used to treat interstitial pregnancy successfully, with no negative impact on subsequent pregnancies.

This work was conducted (i) to measure the electrical conductance of dry and imbibing seeds, (ii)... more This work was conducted (i) to measure the electrical conductance of dry and imbibing seeds, (ii) to monitor the percentage germination and spread of germination of these species at varying temperature conditions and (iii) to find the optimal germination conditions so that some biochemical and biomolecular studies could be carried out in near future. Seeds of different plant species of Cholistan desert such as Capparis decidua, Salvadora oleiodes and Prosopis cineraria were allowed to germinate at various temperature regimes and their percent age germination was monitored. Results show that optimal germination temperatures for C. decidua is 20EC (90% germination in 8 days) and for P. cineraria (65% in 10 days) and S. oleiodes (85% in 4 days) is 30°C. Imbibition of P. cineraria in sand at 25EC has been effective to get 80 percent seeds germinated. Electrical conductance (μS/cm/10 seeds) measured during early hours/days of imbibition has been correlated with the increase in fresh weights of imbibing seeds, though there is decrease in the rate of electrolytes leakage (μS/cm/hour/10 seeds) with increase in imbibitional time. Work is in progress on the biochemical aspects of these early hours/days of seed germination.

Unreinforced concrete block masonry piers have been tested for lateral strength before and after retrofitting. Welded mesh and injection of cement grout techniques have been used for piers retrofitting. The analysis of experimental results demonstrates that proper retrofitting can decrease the risk to concrete block masonry buildings in future scenario earthquakes. Retrofitting increases not only the overall strength of unreinforced masonry piers but also the ductility.
