A total of three dominant approaches emerged: (1) integration of community nursing services with an acute hospital provider, (2) integration with a mental health provider and (3) the establishment of a stand-alone organisation, i.e. without structural integration. The authors explored how these approaches influenced the trends in emergency hospital admissions and bed day use for older people.Design/methodology/approachThe methodology was a longitudinal ecological study using panel data over a ten-year period from April 2006 to March 2016. This study’s outcome measures were (1) emergency hospital admissions and (2) emergency hospital bed use, for people aged 65+ years in 140 primary care trusts (PCTs) in England.FindingsThe authors found no statistically significant difference in the post-intervention trend in emerg...

A great deal of scientific research has been carried out to determine the consequences of exposure of produced water on the environment. Some of this research has given alarming results. Also, some of the toxic components in produced water may cause irreversible damage to the surrounding environment. Because of this potential risk, very considerable efforts oil separation technologies and researches are being expended. The corrugated plate interceptor is an efficient technique used for oil/water separation from 30 µm to150µm. In the present work, a two-dimensional mathematical model has been developed to investigate the effect of different Reynolds numbers (Re) and droplet diameter (d) on the corrugated plate interceptor performance (efficiency and catch length). The properties of water flow are solved using continuous phase governing equations (Eulerian approach). After that discrete phase equation of motion is solved, through the water moving stream (Lagrangian approach). The governing equations were solved between two corrugated plates using a finite volume FLUENT solver. The investigation showed that the separation efficiency is inversely proportional to the Reynolds number and directly proportional to the oil droplet diameter. It also shows that when the Reynolds number is less than 10 and the droplet diameter great than 30 µm the separation efficiency is almost 98%.

All the newly synthesized hybrids were characterized by their 1 H NMR, F-TIR, Mass spectral data and melting points. The cytotoxic activities (in vitro) of selected hybrids against esophageal cancer of human cell line (SKG) were evaluated by MTT assay. Among them, hybrid 13 exhibited a potent inhibition activity with the IC50 value of 1.61±0.01 μM against esophageal cancer cell (SKG). Cellular mechanism investigations in esophageal carcinoma cells (SKG) elucidated that hybrid 13 inhibited cell growths in vitro and arrested cell cycle at an environmental phase. These results revealed that hybrid 13 holds a promising anticancer agent with the enhancement of further clinical applications in drug discovery field.

However, standard practice for diagnosis and management of CSU does not include testing and treatment of H. pylori. This systematic review aims to assess whether eradication of H. pylori leads to clinical improvement in CSU. METHODS: We searched the PubMed, EMBASE, and Web of Science databases for the Mesh terms Helicobacter pylori and urticaria. Included were studies looking at clinical outcomes of CSU after H. pylori eradication; H pylori infection and eradication were confirmed by biopsy, breath urea test, or fecal antigen test. RESULTS: The literature search yielded over 800 articles published between 1996-2018. Using Rayyan, 633 unique articles were identified, narrowed to 110 articles based on abstract review. Preliminary results from 13 studies yielded a total of 433 out of 738 (59%) subjects with CSU who tested positive for H. pylori. 321 subjects were treated according to local standards, after which 293 had proven H. pylori eradication. Approximately 64% (186/293) had complete or partial resolution of CSU, compared to 36% (106/293) who had no resolution of symptoms after eradication at 3-12-months follow up. Rates of H. pylori positive patients who either did not receive treatment or had unsuccessful eradication were unavailable. By contrast, the literature reports persistence of CSU in >70% of patients at 1-year follow up, not accounting for H. pylori status. CONCLUSIONS: Preliminary results suggest a tendency towards more rapid resolution of CSU with eradication of H. pylori, but further analysis is required.

Brazing process was done with high purity nitrogen gas atmosphere. ER4047 and ER4043 fillers were used with fillers thickness 80µm approximately. Five holding times were employed (5, 10, 15, 20 and 25). The microstructures examination showed two bounding layers. First layer from stainless steel side with Fe-rich phases and second layer from aluminum side with Al-rich phase. Joint strength was mainly depended on thickness of these two layers. EDS results demonstrated high diffusion between fillers and two metals to form different intermetallic compounds and there is a clearly relationship between holding time and filler elements diffusion. XRD indicated existing of Mg 2 Si and Al-Fe-Si phases at joining line and diffusion zone, which gives high strength. When holding time raised more than 20 minutes brittle phases (Fe 3 Al, FeAl and FeAl 3) would form or growth speedily in joining line and diffusion zone. High hardness and brittle nature of these intermetallic phases will decrease mechanical properties of braziments. Shear results recorded 18MPa for the AA5086-H116 joining with ER4047 for 20 minutes and 16Mpa when joining with ER4043 for 20 minutes also.

METHODS Healthy 61 shamma users and healthy 61 non-users participated in this study after informed consent. A mouth swap was taken from each study subject and investigated for cellular changes using a Papanicolaou stain. The results were analyzed using the t-test per cent of the StatPac statistical program. RESULTS Keratinization, binucleation and combined results were associated with shamma use. The number of shamma users with abnormal histology was 14 (22.9%) (p≤0.0001). The shamma types associated with the abnormal cytology were: Suhaily (9/36; p=0.0014), black (2/9; p=0.08), white (2/6; p=0.41), and red (1/1; p=1.00). The percentage of the tissues with abnormal histology decreased with the increase in the duration and frequency of smokeless tobacco use, except for the white shamma which increased with the increase in the frequency. CONCLUSIONS Shamma use affected the mouth epithelial cytology of 22.9% of its users. The histological changes were inflammation, keratinization, binucleation, and atypia. The Suhaily smokeless tobacco was associated with the highest percentage of the abnormal cytology results, while the duration and frequency of smokeless tobacco use decreased the number of tissues with abnormal histology.

It continues to grow across the world due to factors such as population growth, and economic and infrastructure development. Within the European Union, buildings account for 40% of the total energy requirements and 30% of carbon dioxide emissions. The building sector is keen to improve its sustainability standards and also to help achieve the 20-20-20 targets set by the European Union. The present work aims to design a nearly zero energy sports gymnasium building in Calolziocorte, Italy. Various sustainability techniques are applied in an integrated design project approach using ECOTECT software to undertake the energy modelling exercise. Firstly, the base-case is modelled with conventional building materials and the total energy demand is calculated. Duly considering the local climatic conditions, sustainable materials are chosen for walls, the floor, the roof, and windows and a 38% reduction is noted in the total energy demand of the building compared to the base-case. The impact of louvers as a passive design technique has also been examined on the total energy demand of the building. The monthly load/discomfort analysis is undertaken for various individual functions inside the building to identify the critical areas that consume more energy. The monthly load/discomfort analysis is performed with the proposed materials and the air infiltration rate is improved through the building envelope and 63% reduction is noted in the total energy demand of the building compared to the base-case. A solar access analysis is conducted to understand the on-site energy production and then the building net energy demand is calculated, which is reduced to 90% compared to the base-case.

We trained and tested each model using data from one hospital ( n = 24,696) and compared the performance of these models in data from another hospital ( n = 13,477). We used two performance measures – the calibration slope and area under the receiver operating characteristic curve. The logistic model performed reasonably well – calibration slope: 0.90, area under the receiver operating characteristic curve: 0.847 compared to the other machine learning methods. Given the complexity of choosing tuning parameters of these methods, the performance of logistic regression with transformations for in-hospital mortality prediction was competitive with the best performing alternative machine...

In this retrospective study, serum albumin and bilirubin levels measured before chemoembolization were used to calculate ALBI score in 123 patients treated with 187 high-risk chemoembolizations. Procedures were considered high risk if Child-Turcotte-Pugh score before chemoembolization was ≥ 8. ACLF was objectively measured using chronic liver failure-sequential organ failure assessment score at 30 and 90 d. The 30-day mortality and morbidity from new or worsening ascites and/or hepatic encephalopathy (HE) were assessed. Univariate and multivariate analyses were used to identify clinical and procedural predictors of ACLF in this high-risk population. ACLF occurred after 15 (8%) high-risk chemoembolizations within 30 days and an additional 9...

Such risk equations may be useful in supporting the clinical decision-making process. We aim to develop and externally validate a computer-aided risk of mortality (CARM) score by combining the first electronically recorded vital signs and blood test results for emergency medical admissions.DesignLogistic regression model development and external validation study.SettingTwo acute hospitals (Northern Lincolnshire and Goole NHS Foundation Trust Hospital (NH)—model development data; York Hospital (YH)—external validation data).ParticipantsAdult (aged ≥16 years) medical admissions discharged over a 24-month period with electronic National Early Warning Score(s) and blood test results recorded on admission.ResultsThe risk of in-hospital mortality following emergency medical admission was 5.7% (NH: 1766/30 996) and 6.5% (YH: 1703/26 247).

To provide a sustainable mechanism for the PRASE intervention, a 2-year improvement project explored the potential for hospital volunteers to facilitate the collection of PRASE feedback. To explore the implementation of the PRASE intervention delivered in collaboration with hospital volunteers from the perspectives of key stakeholders. A qualitative case study design was utilized across three acute NHS trusts in the United Kingdom between March 2016 and October 2016. Ward level data (staff interviews; action planning meeting recordings; implementation fidelity information) were analysed taking a pen portrait approach. We also carried out focus groups with hospital volunteers and interviews with voluntary services/patient experience staff, which were analysed thematically. Whilst most ward staff reported feeling engaged...

Due to a lack of previous studies, no information is available on local transmission of the disease in these areas. Therefore, a pilot entomological and epidemiological study was conducted in Al-Malha Locality during the year 2013, to investigate possibility of local transmission and places and times of the year where and when people contract the infection. Kala-azar incidence data were obtained from records of Ministry of Health, North Darfur; Al-Malha rural hospital; and the Federal Ministry of Health, Division of Communicable and Non-communicable Diseases. Sand flies were collected using sticky paper and rodent burrow traps from five different microhabitats during three different phases of the year. Species identification was undertaken using appropriate taxonomic keys. Data were statistically analyzed to determine the distribution of kala-azar among different age groups and between...

This study describes the development and initial validation of an instrument specifically designed to measure MRB on functioning and well-being-the Medication-Related Burden Quality of Life (MRB-QoL) tool. An initial pool of 76-items for MRB-QoL was generated. The link to MRB-QoL survey was sent to a sample of consumers living with at least one chronic medical condition and taking ≥3 prescription medicines on a regular basis. Exploratory factor analysis (EFA) was used to determine the underlining factor structure. Internal consistency (Cronbach's α) and construct validity were examined. The latter was examined through correlation with Medication Regimen Complexity Index (MRCI), Drug Burden Index (DBI) and Charlson's Comorbidity Index (CCI). 367 consumers completed the survey (51.2% male). EFA resulted in a 31-i...
