We have found a pose used by the patients to describe the pain in the gluteal region in cases of suspected piriformis syndrome. This pose in our study has been described as ‘hand on hip sign’ (HHS). In this study, we aim to assess the significance of this sign in screening of patients with piriformis syndrome. Materials and methods: Seventy patients presenting with pain in the gluteal region with radiculopathy between the age group of 18 years and 60 years were included. All patients were asked to locate the site of the pain in the gluteal region. The demonstration of pain by the patients was noted. If the description matched the description of HHS, it was considered to be positive, else it was considered negative. Management of these fractures is a debatable topic with treatment varying from conservative management to surgical fixation. In our study we used a pre-bent k wire in antegrade fashion to achieve fracture fixation. We found that pre-bent K wire provides an excellent reduction with good correction of neck shaft angle and good restoration of movements at the metcarpo-phalyngeal and inter-phalyngeal joints. Pre-bent K wire provides an efficient and economical modality of fixation in fracture neck of fifth metacarpal fractures.Methods: All patients presenting with fracture neck of fifth metacarpal were managed by pre-bent K wire in antegrade fashion by a single surgeon using a single prebent K wire.Results: All the fractures involved the dominant hand (16 right and 6 left). Calcifying aponeurotic fibroma is a rare benign fibrous tumor with typical radiological and histo... more Calcifying aponeurotic fibroma is a rare benign fibrous tumor with typical radiological and histopathological features seen in young children and adolescents. It commonly involves the extremities and is characterized by a very high chance of local recurrence. A complete excision of the tumor is considered to be the treatment of choice. We report a case of the tumor involving the substance of achilles tendon and extending up till its insertion in the calcaneum. To the best of our knowledge, this is the largest reported case of calcifying fibroma, with the tumor measuring 10 cm in its largest diameter. The tumor was excised in toto followed by reconstruction of the tendon defect with V-Y plasty of the native tendon and supplemented with semitendinosus and gracilis graft. There was no evidence of recurrence during the follow up period of 1 year.

A complex fracture involving the distal humerus is a difficult fracture to treat and more so when... more A complex fracture involving the distal humerus is a difficult fracture to treat and more so when it is involved with the ipsilateral shaft of the humerus. Open reduction and internal fixation of the humeral shaft with articular reconstruction have been described for a successful outcome of these complex fractures. However, it has drawbacks, especially in terms of soft tissue dissection and subsequent scarring and non-union. A 42-year-old female presented to the emergency department with a fracture of the intercondylar humerus with an ipsilateral shaft of the left humerus. Combined olecranon osteotomy with posterior minimal plate osteosynthesis was used to treat this fracture. At the one-year follow-up at the postoperative fracture clinic, there was no pain, the range of motion (ROM) of the elbow was 10 degrees to 140 degrees and the radiograph showed a healed fracture with the implant in situ. We present and review a novel technique to treat complex humerus fractures. Articular fragments can be directly visualized and fixed simultaneously. This approach allows for the biological fixation of the fracture and forms a reliable option for treating such complex fractures.

We report a primary parosteal osteosarcoma involving the proximal phalanx of the right middle finger in a 20-year-old woman, which was managed by ray amputation. There was no evidence of local recurrence or systemic metastasis in the postoperative follow-up period of 2 years.

Tophaceous or tumoral pseudogout is a rare form of this disease which is seen around the joints of extremities. It can be misdiagnosed as a neoplastic condition because of its clinicoradiological similarities, and thus, a proper histopathological examination is indispensable. We report one such case of extra-articular deposition of the CPPD crystals in a 65-year-old man who presented with an asymptomatic swelling around the left ankle. Radiographs showed a dense homogenous calcification, and FNAC revealed dense calcium deposits with numerous rhomboid-shaped crystals. In this cross-sectional study, the screening for antibiotic residues in kidneys was done using Microbial Inhibition six (6) plate method to provide baseline data for the prevalence of antibiotic residues in animals slaughtered in Abattoir and non-Abattoir slaughter house in Negeri Sembilan district areas. Basically, the agar diffusion method uses six media which have been separately cultured with different types of bacteria that are relatively susceptible and resistant towards specific antimicrobial at certain pH level. Forty one (41) kidney samples and another fourteen (14) samples were obtained from non-abattoir and approved (Senawang, Negeri Sembilan Abattoir) slaughter houses respectively. 14 (34 %) of the 41 kidney samples from non-abattoir slaughter houses and 1 (7%) of the 14 samples from non-abattoir slaughter houses were positive for antibiotic residues. The result of the study revealed significant association (p<0.05) between types of slaughter house positive for antibiotic residues using chi-square test and confirmed using Binomial logistic Regression. The test divulged seven times possibility of the samples taken from non-abattoir slaughter house to test positive. However, there were no significant differences between all groups of antibiotics from the positive samples. From this study, it is imperative to include more study areas and sample size to conclude the safety of the meat for consumers in Negeri Sembilan.

This case was carried out to compare the healing process and time for the treatment of mange in g... more This case was carried out to compare the healing process and time for the treatment of mange in goats using a combination of Ivermectin with dexamethasone to the earlier Ivermectin with antihistaminics approach. A 3 ½ years old female Boer cross goat was presented with a complaint of crusty lesions on different parts of the body during a routine farm visit in Selangor, Malaysia. Microscopic examinations of the skin scrapes taken revealed Sarcoptes and Psoroptes sp. of mites, while hematological analysis showed slight leucocytosis, neutrophilia with left shift, monocytosis, hyperproteinemia and hyperglobulinemia. Ivermectin injection at 0.2 mg/kg subcutaneously was administered once every 2 weeks in 4 divided doses, while Dexamethasone injection at 1 mg/10 kg and multivitamin at 10 mg/kg IM for 5 days repeated at alternate weeks. Significant improvement was recorded 6 weeks after commencement of treatment and full recovery at 10 weeks. The current drug regimen proved more effective when compared with previous cases that were treated with a combination of ivermectin and antihistaminics.

The effluents from tanning industries in and around Palar river basin are the major cause of Cr (... more The effluents from tanning industries in and around Palar river basin are the major cause of Cr (III) pollution. Forty-five chromium (III) tolerant bacterial strains were isolated from the Palar river basin. Bacillus subtilis VITSCCr01 showed tolerance up to 1500 mg/l and its Cr (III) bioremoval capacity was 64%. Increasing the concentration of Cr (III) increased exopolysaccharide (EPS) production by the bacteria. FT-IR spectral studies confirmed the presence of polysaccharides in the Cr (III) treated bacteria. Adaptation of Bacillus subtilis VITSCCr01 with higher Cr (III) concentration improved the bioremoval capacity to 85%. SEM-EDX showed that the adapted bacteria accumulated high concentration of chromium. Bacillus subtilis VITSCCr01 could be used as a tool for in situ removal of Cr (III) especially in the tannery polluted environment.

Ivermectin injection at 0.2 mg/kg subcutaneously was administered once every 2 weeks in 4 divided doses, while Dexamethasone injection at 1 mg/10 kg and multivitamin at 10 mg/kg IM for 5 days repeated at alternate weeks. Significant improvement was recorded 6 weeks after commencement of treatment and full recovery at 10 weeks. The current drug regimen proved more effective when compared with previous cases that were treated with a combination of ivermectin and antihistaminics.

Forty-five chromium (III) tolerant bacterial strains were isolated from the Palar river basin. Bacillus subtilis VITSCCr01 showed tolerance up to 1500 mg/l and its Cr (III) bioremoval capacity was 64%. Increasing the concentration of Cr (III) increased exopolysaccharide (EPS) production by the bacteria. FT-IR spectral studies confirmed the presence of polysaccharides in the Cr (III) treated bacteria. Adaptation of Bacillus subtilis VITSCCr01 with higher Cr (III) concentration improved the bioremoval capacity to 85%. SEM-EDX showed that the adapted bacteria accumulated high concentration of chromium. Bacillus subtilis VITSCCr01 could be used as a tool for in situ removal of Cr (III) especially in the tannery polluted environment.
