W... more Pseudo intestinal obstruction is an unusual initial presentation of congenital hypothyroid-ism. We describe a newborn boy infant who, on the third day of life, developed clinical and radiological evidence of intestinal obstruction. He was initially treated conservatively. Meanwhile, results of neonatal screening suggested congenital hypothyroidism which was confirmed to have permanent primary congenital hypothyroidism, due to thyroid hormone dyshormonogenesis. Replacement therapy with Levothyroxine led to marked improvement.

Systemic basidiobolomycosis is a rare fungal infection caused by Basidiobolus rararum (B. rararum... more Systemic basidiobolomycosis is a rare fungal infection caused by Basidiobolus rararum (B. rararum). The clinical presentation is non-specific and is similar to many gastrointestinal conditions such as Crohn's disease (CD). The most consistent findings of basidiobolomycosis are recurrent abdominal pain, weight loss, fever and peripheral eosinophilia. Most of the patients are diagnosed on surgical resection of the involved region along with compatible histopathological findings like transmural inflammation, granulomas with eosinophilic infiltration (Splendore-Hoeppli phenomenon) and more specifically detection of fungal hyphae on fungal stains. Effective and curative treatment for systemic basidiobolomycosis is available, if diagnosed and managed properly in time. We report here a Saudi boy who had ileo-caecal basidiobolomycosis, but diagnosed after a prolonged course of illness.

The vast majority of gastrointestinal basidiobolomycosis cases were reported from tropical and subtropical regions. We report a Saudi pediatric patient with ileal basidiobolomycosis and initial clinical presentation mimicking acute appendicitis before being misdiagnosed as Crohn's disease. Our case is the first to report effective treatment of pediatric gastrointestinal basidiobolomycosis using voriconazole mono-therapy. In addition, we present extensive review of pediatric gastrointestinal basidiobolomycosis in medical literature.
