However, the former file system was associated with more postoperative pain than the latter. The goal of the present study was to evaluate and compare the clinical efficacy and safety between continuous rotary and reciprocating instrumentation systems. Methods: Fifty patients who fulfilled specific inclusion criteria were assigned into 2 groups according to the root canal instrumentation technique used, ProTaper Gold and WaveOne Gold. Endodontic treatment was performed in a single visit. Patients were instructed to rate the intensity of postoperative pain on Visual Analog Scale and to record the quantity of prescribed analgesic medication taken after 24 hours, 48 hours, 72 hours, and 7 days. She gave a history of the restoration of the teeth 6 months ago that underwent progressive marginal discoloration. On clinical examination, two upper central and two lateral incisor teeth were restored by Glass Ionomer cement and Secondary caries were present beneath restorations. Teeth were vital and did not respond to percussion and mobility test. Radiographic examination revealed that the four maxillary anterior teeth (2 central and 2 laterals) had the existing restoration which showed a radioopaque area in the proximal surface but radiolucent areas were also seen beneath the radioopaque area that extends to the dentin.

A 60 year's old female presented with the complaints of mild pain during mastication on the lower... more A 60 year's old female presented with the complaints of mild pain during mastication on the lower right posterior tooth for 2 months. She gave history of root canal treatment with crown placement on the same tooth 1 year ago. On clinical examination, crown was present on the lower right first molar tooth and there was tenderness on percussion. There was no sign of any swelling and discharge. Dr. Mozammal Hossain (Associate Professor); Preoperative radiograph (Figure 1-A) showed no traces of gutta-percha in the root canals and there was widening of periodontal ligament space surrounding the roots of lower right first molar tooth. Calcification of the root canals in the apical third was also seen. Clinical procedure Dr. Ahmed: The patient was sent to the Department of Prosthodontic for the removal of the crown. After administration of local anesthesia the tooth was isolated with rubber dam. The old restoration was removed with round bur and flushed with normal saline. The access cavity was modified with endo-z bur (Dentsply). Sharp endodontic explorer was used to identify the canal orifices. Three canals were identified; mesiobuccal, mesiolingual and distal canals. He gave the history of trauma on the offending tooth 3 years ago and for this he visited to a local dentist 2 weeks ago but did not get any relief after treatment. On clinical examination, the tooth was non vital, tender to percussion with 2 degree mobility, not associated with any swelling and discharge. Pre-operative radiograph revealed obliteration of pulp chamber, area of rarefaction associated near the apex of upper left lateral incisor tooth, cervical radiolucency indicating the loss of periodontal structures and about 5 mm separated fragment of instrument in the coronal third of the root canal.

A 28 year old male patient came to the outpatient Department complaining of discomfort on his low... more A 28 year old male patient came to the outpatient Department complaining of discomfort on his lower right first molar tooth during mastication for one month. The patient also stated that he had received dental treatment on the lower right side three months back. Clinically, the tooth appeared sound but there was poor coronal filling of the affected tooth and the pocket depth was within normal range (Figure 1A,B). When the pulp vitality test was performed, it responded negatively. However, percussion pain was present. It seems that due to iatrogenic mishaps, chronic periodontitis could be developed in this case. Dr. Mozammal Hossain: Radiograph showed periapical radiolucency <1 mm on the apex of tooth and the floor of the pulp chamber was found perforated. However, there was no vertical or horizontal bony defect around the furcation or around the root apex. There was no sign of periapical lesion (Figure 1C).

The purpose of the present study was to compare the clinical performance of bulk-fill composite r... more The purpose of the present study was to compare the clinical performance of bulk-fill composite resin with that of layered composite resin restorations in occlusal class I cavity of permanent molar teeth. This study includes a total of 104 teeth having class I cavity of 3-4 mm depth in the occlusal surface. They were restored either with layered composite resin or bulk-fill composite resin of 52 teeth in each group. To observe the retention, marginal adaptation and color match, all restorations were assessed by using modified United States Public Health Services (USPHS) or Ryge’s criteria at baseline, 3, 6 and 12 months. The results of 12 months observation showed that retention of 38 layered composite and 48 bulk-fill composite resin restorations revealed alpha rating, which was statistically significant. Thirty teeth having pulpal floor perforation were enrolled in this study, out of which 15 perforations were repaired with mineral trioxide aggregate and the remaining 15 were subjected to Biodentine treatment. Following perforation repair, each tooth was then subjected to root canal therapy. Patients were recalled at 3, 6 and 12 month interval. The assessment of pain, pain on palpation, tenderness on percussion, and radiological evaluation was performed to determine the repair ability of pulpal floor perforation. A 42 year old female presented with the complaints of thermal sensitivity and impaction of food d... more A 42 year old female presented with the complaints of thermal sensitivity and impaction of food during mastication on her lower left posterior region. On clinical examination, carious lesions involving proximal surface was detected at the lower left first molar tooth which extended up to the dentin (Figure 1A). The vitality test showed that the affected tooth was vital and the tooth was also non-tender to percussion. Radiological Findings Dr. Ali. Radiographic examination revealed that the tooth had a radiolucent area in the proximal surface that extended to the dentin. However, there was no involvement of the pulp and periodontal tissues (Figure 1B). Treatment Procedure Dr. Ali: After considering the structural integrity of the tooth, it was decided to restore the tooth by direct nanohybrid resin composite (Ceram X TM) restorative material with an onestep self-etching adhesive system (7 th generation). At first, the carious tissue was removed by carbide bur under constant water cooling. A cavity with a diameter of 3 mm × 4 mm was prepared without bevel and neither lining nor a base material was used. Then, the operative field was carefully isolated with the cotton rolls and a suction device. After removal of caries, a thin metallic matrix band was fixed in the interproximal area and the cavity was cleaned by a thorough full rinsing with water. Application of the adhesive resin was given according to the manufacturer's instructions; after drying the operative field, the bonding agent was applied and light cured for 10 sec. Restoration with nanohybrid composite resin An E-2 shade of nanohybrid composite resin (Ceram X TM) was applied in layers of maximally 2-3 mm width. Every increment was light-cured with a well-controlled LED lightunit for at least 20 sec. The occlusion was adjusted and contour was prepared with finishing diamond burs. The final polishing was performed with the shofu polishing system. The final restoration had excellent marginal integrity and the color was matched with the adjacent tooth (Figure 1C). The patient was instructed to continue the normal oral hygiene.

The teeth were brought by the patient those were healthy and intact. His medical history was non-contributory and he was tetanus immunized. He came 2 hours after the accident with the missing teeth immersed into 0.9% sodium chloride solution. On clinical examination, there were missing of both central incisors. Sockets were full of blood. On palpation, no other tooth was tender.

On clinical examination, carious lesions involving proximal surface was detected at the lower left first molar tooth which extended up to the dentin (Figure 1A). The vitality test showed that the affected tooth was vital and the tooth was also non-tender to percussion. Radiological Findings Dr. Ali. Radiographic examination revealed that the tooth had a radiolucent area in the proximal surface that extended to the dentin. However, there was no involvement of the pulp and periodontal tissues (Figure 1B). Treatment Procedure Dr. Ali: After considering the structural integrity of the tooth, it was decided to restore the tooth by direct nanohybrid resin composite (Ceram X TM) restorative material with an onestep self-etching adhesive system (7 th generation). At first, the carious tissue was removed by carbide bur under constant water cooling. A cavity with a diameter of 3 mm × 4 mm was prepared without bevel and neither lining nor a base material was used. Then, the operative field was carefully isolated with the cotton rolls and a suction device. After removal of caries, a thin metallic matrix band was fixed in the interproximal area and the cavity was cleaned by a thorough full rinsing with water. Application of the adhesive resin was given according to the manufacturer's instructions; after drying the operative field, the bonding agent was applied and light cured for 10 sec. Restoration with nanohybrid composite resin An E-2 shade of nanohybrid composite resin (Ceram X TM) was applied in layers of maximally 2-3 mm width. Every increment was light-cured with a well-controlled LED lightunit for at least 20 sec. The occlusion was adjusted and contour was prepared with finishing diamond burs. The final polishing was performed with the shofu polishing system. The final restoration had excellent marginal integrity and the color was matched with the adjacent tooth (Figure 1C). The patient was instructed to continue the normal oral hygiene.

The teeth were brought by the patient those were healthy and intact. His medical history was non-contributory and he was tetanus immunized. He came 2 hours after the accident with the missing teeth immersed into 0.9% sodium chloride solution. On clinical examination, there were missing of both central incisors. Sockets were full of blood. On palpation, no other tooth was tender.
