This is due to the fact that the desired temperature can vary depending on the condition of the patient and the specific requirements of surgical and treatment procedures. To address this need, our objective is to develop a tool for predicting the electric power needed to maintain a desired temperature in these critical care areas. Previous research has employed artificial learning algorithms and mathematical equations to predict electric power for various types and sizes of buildings, with promising results. Bio-conversion of cellulosic based biomass materials for cellulase production is one among the ma... more Bio-conversion of cellulosic based biomass materials for cellulase production is one among the major increasing demands for various biotechnological applications. Aspergillus niger was cultured in corn cobs based fermentation medium under some pre-optimized growth conditions. After four days of still culture incubation a large magnitude of cellulase (28.3±0.24U/mL) was achieved when the SSF medium containing 10 g corn cobs inoculated with 5 mL of inoculum at 30°C. The effects of different minerals were optimized through Response Surface Methodology (RSM) by adopting a Central Composite Design (CCD). The crude cellulase was purified 5.71 fold with specific activity of 232.5U/mg using ammonium sulfate precipitation, and Sephadex-G-100 gel filtration column chromatography. Enzyme was found to be a monomeric protein as evident by single band corresponding to 43 kDa on SDS-PAGE. The present study demonstrates the application of seismic petrophysics and amplitude versus angle... more The present study demonstrates the application of seismic petrophysics and amplitude versus angle (AVA) forward modeling to identify the reservoir fluids, discriminate their saturation levels and natural gas composition. Two case studies of the Lumshiwal Formation (mainly sandstone) of the Lower Cretaceous age have been studied from the Kohat Sub-basin and the Middle Indus Basin of Pakistan. The conventional angle-dependent reflection amplitudes such as P converted P (RPP) and S (RPS), S converted S (RSS) and P (RSP) and newly developed AVA attributes (ΔRPP, ΔRPS, ΔRSS, and ΔRSP) are analyzed at different gas saturation levels in the reservoir rock. These attributes are generated by taking the differences between the water wet reflection coefficient and the reflection coefficient at unknown gas saturation. Intercept (A) and gradient (B) attributes are also computed and cross-plotted at different gas compositions and gas/water scenarios to define the AVO class of reservoir sands. The numerical simulation reveals that ΔRPP, ΔRPS, ΔRSS, and ΔRSP are good indicators and able to distinguish low and high gas saturation with a high level of confidence as compared to conventional reflection amplitudes such as P-P, P-S, S-S and S-P. In A-B cross-plots, the gas lines move towards the fluid (wet) lines as the proportion of heavier gases increase in the Lumshiwal Sands. Because of the upper contacts with different sedimentary rocks (Shale/Limestone) in both wells, the same reservoir sand exhibits different response similar to AVO classes like the class I and class IV. This study will help to analyze gas sands by using amplitude-based attributes as direct gas indicators in further gas drilling wells in clastic successions.

It provides excellent analgesia during surgery as well as in postoperative period in subumblical surgeries. In this study, we compared the analgesic efficacy and safety of caudal dexmedetomidine added to caudal ropivacaine for postoperative analgesia in children. 60 patients aged 1 to 8 years scheduled for subumblical surgeries were randomly allocated into two groups of 30 patients each. Group RD received 1ml/kg of 0.2% ropivacaine with dexmedetomidine 2ìg/Kg in normal saline 1 ml. Group R received 1ml/kg of 0.2% ropivacaine with normal saline 1ml.Hemodynamic parameters, duration of analgesia, pain scores using observational pain scoring (OPS), requirement of rescue analgesia, and various complications were recorded.The duration of analgesia in group R was 8.4± 1.4 hours while in group RD the duration was 10.4 ± 2 hours. Spreadsheet-based programming has become a worldwide computational tool and a powerful platform f... more Spreadsheet-based programming has become a worldwide computational tool and a powerful platform for executing scientific calculations in different engineering disciplines. The inherent advantage of spreadsheets exists in integrated storage and the ability to manipulate matrices along with wide-ranging mathematical calculations, plotting, and diagrammatic presentation. This paper presents a user-friendly and interactive spreadsheet program “AVA-anisotropy” to characterize the primary P-wave amplitude versus incident angle (AVA) responses in vertically transverse isotropic (VTI), horizontally transverse isotropic (HTI), and orthorhombic media. The set of Rüger’s equations for isotropic, VTI, HTI, and orthorhombic media are solved to describe the seismic energy partitioning at an anisotropic interface. The spreadsheet program has three main sections: basic concepts, data input, and AVA modeling for anisotropic media. The interactive data input and on-screen mathematical and graphic presentation of the spreadsheet enables the students to have an in-depth examination of the effects of changing input data and the resulting model. This greatly helps them in understanding the energy partitioning in anisotropic medium.

Introduction: Caudal block since its first description in 1933 for paediatric urological interven... more Introduction: Caudal block since its first description in 1933 for paediatric urological interventions has evolved to become the most popular regional anesthetic technique for use in children. It provides excellent analgesia during surgery as well as during postoperative period in subumblical surgeries in children. This randomised prospective controlled study was undertaken to find and compare quality and duration of analgesia, motor and sensory block after a single shot caudal block with either Bupivacaine or Ropivacaine.Method: 60 ASA I-II patients in the age range-12 years scheduled for Elective subluminal procedures were enrolled. Standard anaesthetic induction was conducted in all patients. After induction caudal block was performed in the lateral position. Perioperative haemodynamic parameters were recorded. Patients were randomly allocated to one of the two groups of 30 patients each. Group A received 1ml/kg of 0.25% bupivacaine. Group B received 1ml/kg of 0.2% ropivacaine.Results: Post operative pain score was comparable in two groups in the first 4 hours but it was significantly less in ropivacaine group after 4 hour. The mean duration of analgesia in group A was 7.4±1.0 hours while in group B mean duration of analgesia was 7.6±1.3 hours. 19 patients (63.3%) in group A received rescue analgesic as compared to 10 (33.3%) patients in group B during 12 hour study period. Conclusion: Caudal ropivacaine provides effective post-operative analgesia and possessing less motor blockade makes it a suitable agent for day care surgery with increased margin of safety particularly in younger children

Background: Cannulation of central vein has become an invaluable procedure for the management of ... more Background: Cannulation of central vein has become an invaluable procedure for the management of Critical Care patients for a variety of diagnostic and therapeutic reasons. The present study aimed to evaluate whether real time Ultrasound guided technique for central venous Cannulation is superior to the traditionally used landmark based technique. Materials and Methods: The study was performed in an intensive Care unit (ICU). 120 patients requiring Cannulation of Central vein were randomly taken for the study and were divided into two groups: Group A and Group B. Right internal jugular vein (RIJV) was selected for cannulation. In group A, RIJV was cannulated using real time ultrasound guidance while as in group B; RIJV was cannulated using landmark based technique. The parameters observed were; time required for cannulation (access time), number of attempts required for cannulation and complications encountered in both the techniques. Results: Mean number of attempts for successful cannulation were 1.22±0.42 in group A as against 2.27±0.61 for group B. Mean time for successful Cannulation was recorded as 16.6±2.47seconds for group A as against 56.3±7.52 for group B. 1.60% recorded haemothorax, 3.33% pneumothorax, 1.60% haematoma, 3.33% carotid artery puncture and 5% recorded catheter malposition in group B. No such complication was recorded in group A. Conclusion: From the present study it has been observed that the real time ultrasound guided method is superior over the landmark method for the internal jugular vein cannulation in critical care patients. The ultrasonography guided internal jugular vein cannulation minimises the time, number of attempts and complications as compared to the landmark technique.

Samples names were Ginseng, Nan-bao, Choe, Safoof-e-Mughallis (Powder), Imsaki (Jawahar) capsules, Zarjam (capsules), Shahi (capsules), Mumsik sultani (Jawahar) capsules, Laboob-e-Kabeer, Laboob Bard (Jawahar wala), Laboob Khas (Jawahar wala). Results indicate the presence of variable amounts of metals in these samples. Order of concentration of metals in different samples has been found to be as Ca> Mg > K > Na. Shahi capsules exhibited highest amount of sodium (3100±1825 µg/g) and other three minerals were in higher quantities. Choe contained highest amount of calcium (54175±4898 µg/g) and magnesium (53223±3205 µg/g) while Mumsik showed highest quantity of potassium (12070±882 µg/g). Post operative nausea and vomiting (PONV) continue to be frequent occurrences, even when conventi... more Post operative nausea and vomiting (PONV) continue to be frequent occurrences, even when conventional antiemetics are prophylactically used. In a randomized double blind study, 100 female patients scheduled for elective laparoscopic cholecystectomy under general anaesthesia were divided into 2 groups of 50 patients each and received 0.1mg/Kg of Ondansetron (Group X) or 0.04mg/Kg of Granisetron (Group Y) preoperatively. Patients were observed for 24 hours post operatively and interpretation of symptoms of nausea and vomiting was done according to Gan and Alexander scale (0-2). 80% of patients in Group Y and 48 % patients in Group X did not experience PONV; the difference was statistically significant (p<0.001). 2 patients (4%) in Group Y and 15patients (30%) in Group X required rescue antiemetic medication during the 24 hour study period. The difference was found to be highly significant (p<0.001) The difference in the incidence of PONV between the two groups after 6 hours to 24 hours was highly significant (p<0.001). It was concluded that prophylactic administration of Granisetron is more effective than Ondansetron, in reducing in incidence of PONV with prolonged effects.

Objective: To assess the number of TB patients diagnosed in pulmonology department of a teaching hospital and refer them to their nearby treatment centers. Methods: This was a cross sectional study conducted from March 2009 to October 2009 at pulmonology unit of Lady Reading Hospital, Peshawar. All patients diagnosed as active TB patients presented to Pulmonology department were included. Patients' data including age, sex, address, type and site of TB and sputum status were noted. In this study, we compared the effect of preloading with crystalloid and intravenous ephedrine ag... more In this study, we compared the effect of preloading with crystalloid and intravenous ephedrine against the hypotensive effects of propofol and fentanyl induction in ASA I-II patients scheduled for elective surgical procedures. 150 patients aged 18yrs to 60yrs were randomly allocated to one of the three groups of 50 patients each. Group-A (control) did not receive any study medication, group-B received Ringers lactate 20ml/kg over 10-15min and group-C received intravenous ephedrine 0.2mg/kg prior to induction of anesthesia. Anesthesia was induced with propofol 2.5mg/kg, fentanyl 1.5µg/kg and atracurim 0.5mg/kg. Heart rate and blood pressure were recorded before induction and then every min for 5min after induction of anesthesia. After the study period patients were intubated and anesthesia was continued as required. Hypotension was defined as a drop in systolic arterial pressure more than or equal to 20% of baseline. A significant decrease in systolic arterial pressure occurred in both the fluid loaded and the control group. Least decrease in systolic arterial pressure was seen in the ephedrine group. The incidence of hypotension was also lower in ephedrine group when compared with control group. We conclude that crystalloid preloading is not efficacious in preventing hypotension and ephedrine markedly attenuates, but does not fully abolish, the decrease in blood pressure caused by propofol and fentanyl induction.

In this study four inorganic elements i.e. sodium, potassium, calcium and magnesium were estimate... more In this study four inorganic elements i.e. sodium, potassium, calcium and magnesium were estimated in eleven locally available herbal preparations used as sex stimulants using Flame Atomic Absorption Spectrometry and Flame Emission Spectroscopy. Samples names were Ginseng, Nan-bao, Choe, Safoof-e-Mughallis (Powder), Imsaki (Jawahar) capsules, Zarjam (capsules), Shahi (capsules), Mumsik sultani (Jawahar) capsules, Laboob-e-Kabeer, Laboob Bard (Jawahar wala), Laboob Khas (Jawahar wala). Results indicate the presence of variable amounts of metals in these samples. Order of concentration of metals in different samples has been found to be as Ca> Mg > K > Na. Shahi capsules exhibited highest amount of sodium (3100±1825 µg/g) and other three minerals were in higher quantities. Choe contained highest amount of calcium (54175±4898 µg/g) and magnesium (53223±3205 µg/g) while Mumsik showed highest quantity of potassium (12070±882 µg/g). Post operative nausea and vomiting (PONV) continue to be frequent occurrences, even when conventi... more Post operative nausea and vomiting (PONV) continue to be frequent occurrences, even when conventional antiemetics are prophylactically used. In a randomized double blind study, 100 female patients scheduled for elective laparoscopic cholecystectomy under general anaesthesia were divided into 2 groups of 50 patients each and received 0.1mg/Kg of Ondansetron (Group X) or 0.04mg/Kg of Granisetron (Group Y) preoperatively. Patients were observed for 24 hours post operatively and interpretation of symptoms of nausea and vomiting was done according to Gan and Alexander scale (0-2). 80% of patients in Group Y and 48 % patients in Group X did not experience PONV; the difference was statistically significant (p<0.001). 2 patients (4%) in Group Y and 15patients (30%) in Group X required rescue antiemetic medication during the 24 hour study period. The difference was found to be highly significant (p<0.001) The difference in the incidence of PONV between the two groups after 6 hours to 24 hours was highly significant (p<0.001). It was concluded that prophylactic administration of Granisetron is more effective than Ondansetron, in reducing in incidence of PONV with prolonged effects.

Methods: This was a cross sectional study conducted from March 2009 to October 2009 at pulmonology unit of Lady Reading Hospital, Peshawar. All patients diagnosed as active TB patients presented to Pulmonology department were included. Patients' data including age, sex, address, type and site of TB and sputum status were noted. In this study, we compared the effect of preloading with crystalloid and intravenous ephedrine ag... more In this study, we compared the effect of preloading with crystalloid and intravenous ephedrine against the hypotensive effects of propofol and fentanyl induction in ASA I-II patients scheduled for elective surgical procedures. 150 patients aged 18yrs to 60yrs were randomly allocated to one of the three groups of 50 patients each. Group-A (control) did not receive any study medication, group-B received Ringers lactate 20ml/kg over 10-15min and group-C received intravenous ephedrine 0.2mg/kg prior to induction of anesthesia. Anesthesia was induced with propofol 2.5mg/kg, fentanyl 1.5µg/kg and atracurim 0.5mg/kg. Heart rate and blood pressure were recorded before induction and then every min for 5min after induction of anesthesia. After the study period patients were intubated and anesthesia was continued as required. Hypotension was defined as a drop in systolic arterial pressure more than or equal to 20% of baseline. A significant decrease in systolic arterial pressure occurred in both the fluid loaded and the control group. Least decrease in systolic arterial pressure was seen in the ephedrine group. The incidence of hypotension was also lower in ephedrine group when compared with control group. We conclude that crystalloid preloading is not efficacious in preventing hypotension and ephedrine markedly attenuates, but does not fully abolish, the decrease in blood pressure caused by propofol and fentanyl induction.
