STUDY DESIGN The rats were allotted randomly into 3 experimental groups: A (control), B (diabetic untreated), and C (diabetic treated with low-intensity exercise); each group contained 8 animals. Groups B and C received STZ. Diabetes was induced in 2 groups by a single intraperitoneal injection of STZ (40 mg/kg, freshly dissolved in 0.1 M citrate buffer, pH 4.2). Two days after STZ treatment, diabetes in 2 experimental groups was confirmed by measuring blood glucose levels. Rats with blood glucose levels ≥ 250 mg/dL were considered to be diabetic. Animals in the exercise group were made to run the treadmill once a day for 4 consecutive weeks. Exercise started 3 days prior to STZ administration. Background: Femoral shaft fractures are an important cause of mortality and morbidity in the lowe... more Background: Femoral shaft fractures are an important cause of mortality and morbidity in the lower extremity injuries.Methods: 21 femoral body fractures of 19 adult patients were treated with the unreamed intramedullary nailing. Of 19 patients 14 were males and 5 were females.Results: All fractures healed. Deep infection was not encountered. According to the Thoresen criteria, the rate of excellent-good results was 78%. In two patients, who had also cranial trauma, development of excessive callus was observed. None of the patients required a secondary operation. Conclusions: Unreamed intramedullary nailing, if it can be implemented with closed technique and with double lock screw at the distal side, provides satisfactory results in the femoral shaft fractures.

A 17 year-old male patient who had a history of Nuss procedure 8 months ago with a diagnosis of p... more A 17 year-old male patient who had a history of Nuss procedure 8 months ago with a diagnosis of pectus excavatum, the bar had been removed with left anterior thoracotomy due to complications such as bar dislocation, hemothorax and infection. Minimally invasive reoperation was performed on the patient who had an asymmetrical collapse distinctive on the right side. Classical Nuss operation was not possible due to strong adhesions that developed after past procedures. At this point, instead of open surgery, we entered the thoracic cavity with two new incisions performed on the two peak points of the collapse. A tunnel was formed beneath the sternum with obtuse and sharp opposing dissections through the newly formed holes. Subsequently, subcutaneous tunnels were opened up to the new entry holes through the initial incisions that were formed left and right previously. The bar was swiped under the sternum and rotated by using the subsequent holes as entry and exit holes for the bar. It was immobilized with a stabilizer. No complications occurred in the postoperative period. Pleural adhesions are accepted as one of the most important contraindications of thoracoscopic surgery. Nevertheless, with the help of two incisions in addition to the main procedure we present our case of re-operated pectus excavatum with a minimally invasive procedure and a successful outcome.

Hereditary exostoses (HME) is an autosomal dominant disorder with variable penetrance characterized by multiple osteochondromas near joints. It is one of the most commonskeletal dysplasias with a frequency of about 1.18%. Objective: In this study, the results of open release surgeries performed for the stiff elbows wh... more Objective: In this study, the results of open release surgeries performed for the stiff elbows which were caused by intrinsic, extrinsic or by combination of both factors were evaluated. Methods: Twenty-three elbows of 22 patients who could not perform functional elbow motions were surgically treated between January 2005 and December 2012. The mean age was 30.6±11.4 years (16 to 67 years), and the average follow-up period was 81 months (58 months to 12 years). Elbow arc of motions was recorded pre-and postoperatively. Patients were evaluated clinically by using quick Disabilities of the Arm, Shoulder and Hand (DASH) self-report questionnaire, and MAYO elbow performance scores. Results: The mean preoperative elbow arc of motion was 42 degrees (0-90), and it became 109 degrees (70-140) postoperatively after at least 58-month follow-up. The average increase was 67.8±25 degrees (30-125). The mean Quick DASH score was 18.2±12.7 (6.8 to 63.6). According to MAYO elbow rating system, one patient who had experienced infection following the initial fracture treatment had poor result, three patients had fair results, 17 patients had good results, and one patient who had bilateral elbow stiffness had excellent results. Conclusion: Although good results can be achieved by open release of the stiff elbows, one must keep in mind that preventing the stiffness rather than solving this problem would bring better results. As 15 patients with stiff elbows in this study had a previous surgery performed for an elbow region fracture, stabile fixation, meticulous hemostasis, shorter immobilization period and early rehabilitation can be considered as the most important steps for avoiding elbow stiffness where surgery is indicated.

Obezitenin giderek artması ve toplumda ortalama yaşam beklentisinin uzamasına bağlı olarak osteoa... more Obezitenin giderek artması ve toplumda ortalama yaşam beklentisinin uzamasına bağlı olarak osteoartrit görülme sıklığı giderek artmaktadır. Son çalışmalarda, 60 yaş üzeri toplumda insanların %13'ünde semptomatik osteoartrit (OA) görüldüğü bildirilmiştir. Son 20 yılda bu hastalarda ağrıyı azaltmak ve fonksiyonları koruyabilmek amacıyla minimail invaziv yöntemler kullanılarak uygulanabilen birçok ilaç geliştirilmiştir. Biz bu çalışmamızda, Kellgren-Lawrence evrelemesine göre evre 2 ve 3 gonartrozu olan hastalarda trombositten zengin plazma (PRP) enjeksiyonunun ağrı ve diz fonksiyonları üzerine etkilerini incelemeyi amaçladık. Gereç ve Yöntem: Çalışma için PRP enjeksiyonu yapılan 73 hastanın medikal kayıtları retrospektif olarak incelendi. Kayıtları eksik olan veya takiplere gelmeyen 29 hasta çalışmaya dahil edilmedi. Çalışmaya dahil edilme kriterleri; Kellgren-Lawrence evre 2 veya 3 gonartrozu olmak, 50 yaşından büyük olmak, lomber herniasyon kaynaklı ağrısı olmamak ve medikal kayıtlarında değerlendirme formlarının tam olarak bulunmasıydı. Bulgular: Değerlendirme sonrasında hastaların 37'sinde (%84) VAS skorlarında iyileşme saptandı (p<0,001). Hastaların 5'i (%11) ağrılarında değişiklik olmadığını tariflerken 2'si (%5) ağrılarının dahada ilerlediğini bildirdi. KSS sonuçları değerlendirildiğinde hastaların 40'ında (%90) iyileşme saptandı (p<0,001). Hastaların 4'ünde (%10) ise değşiklik saptanmadı. Hastaların hiçbirinde KSS skorlarında kötüleşme saptanmadı. Sonuç: Sonuç olarak, PRP enejksiyonu ile tedavi ucuz, kolay ulaşılabilir ve ağrıyı azaltan bir tedavi yöntemidir. PRP enjeksiyonu ile diz fonksiyonları orta-ileri evre gonartrozu olan hastalarda korunabilir. Kullanılacak enjeksiyon sayısı, sıklığı ve zamanı konusunda net sonuca varmak, kullanılacak trombosit konsantrasyon miktarının net olarak belirlemek adına ilerde daha fazla sayıda katılımcının birlikte çalıştığı, randomize ve karşılaştırmalı çok merkezli çalışmalar yapılmasını önermekteyiz.

This study aims to evaluate changes in surgical strategy and orthopedic epidemiology, and to compare the frequency of surgeries before and during the COVID-19 pandemic. Methods. For periods from April 1 to May 31 in years 2019 and 2020, retrospective data on patient demographics and types of orthopedic surgical procedures were obtained from hospital databases in a tertiary referral hospital. Results. During the COVID-19 pandemic, the most common orthopedic surgical procedures performed were trauma surgery (n = 81), while other procedures were referred to oncology (n = 19), biopsy (n = 11), debridement (n = 10), amputation (n = 6), surgery of dysplastic hip (n = 5) and knee ligament repair (n = 1). In this study, we aimed to present our treatment results of two patients with total and proximal femoral mega prosthesis respectively. Case Series: With the diagnosis of periprosthetic infection, in both cases, prosthesis were disassembled and sterilized in autoclave. Prosthesis were reimplanted after sterilization and Vacuum Assisted Closure system with instillation. Aim: Mathematical relationships between bones in close proximity are useful for determining the p... more Aim: Mathematical relationships between bones in close proximity are useful for determining the proper length of comminuted or severely displaced bones during surgical reconstruction. This study examined the relationship between the lengths of the clavicle and ulna to develop a length propotion formula that can facilitate obtaining the proper length of the clavicle during surgery for displaced and comminuted fractures. Methods: The study enrolled 130 individuals (76 males, 54 females) who were healty for their upper extremities and was seen in our orthopedics outpatient clinic during April 2019 to June 2019. The right and left clavicles and ulnas of each individual were measured. All measurements were peformed by two of the authors in an blinded fashion. Results. Mean overall age was 32.2 years. There were significant correlations between the clavicle and ipsilateral ulnar lengths (p=0.001). However, there was a length difference between the right and left clavicles. The left clavicle was 9.00 ± 2.16 mm longer than the right in males and 7.13 ± 2.03 mm longer in females (p=0.001). There was no significant length difference between the right and left ulnas in either sex (0.84 ± 1.033 mm in males and 0.52 ± 0.818 mm in females). Conclusion. There is a significant ratio between the clavicle and ulna lengths on both the right and left sides in both sexes, but it isn't the same for males and females. We propose that these length proportion formula can be used for determining the clavicle length for the surgical treatment of comminuted fractures with plates and particularly with nails.

The differential diagnosis includes benign and malignant lesions. Case presentation: A 23-year-old European woman presented at an outpatient clinic with a 9-month history of a painless swelling on the right posterolateral side of her chest wall. The case reported here is of a very rarely seen parosteal osteosarcoma of the rib that was treated with wide resection and chest wall reconstruction. There was no evidence of local recurrence or distal metastasis after a 1-year follow-up. Conclusion: Parosteal osteosarcoma is a locally aggressive malignant tumor, and resection with a wide margin is the most appropriate treatment. Correct diagnosis of parosteal osteosarcoma is challenging for an orthopedic surgeon. Although rare, in the differential diagnosis of lesions located on the ribs, parosteal osteosarcoma should be considered and a systematic diagnostic approach should be taken.

Trevor's disease, also known as dyplasia epiphysealis hemimelica, is characterised by osteoch... more Trevor's disease, also known as dyplasia epiphysealis hemimelica, is characterised by osteochondromas arising from epiphyses. The disease typically affects one side of an epiphysis (usually the medial side). A case in whom both the medial and lateral sides of the epiphysis were involved is described. Thus, the use of the descriptor "hemimelica" is not always appropriate. Although rare, Trevor's disease should be kept in mind during the differential diagnosis of patients with ankle pain and a mass. Other possible joints should be examined to explore multiple involvement. Surgical treatment, only on lesions causing impingements, may improve functional status and reduce pain.

Preoperatif ve postoperatif düşük hemoglobin değerlerinin kalça cerrahisi sonrası morbidite, mort... more Preoperatif ve postoperatif düşük hemoglobin değerlerinin kalça cerrahisi sonrası morbidite, mortalite, erken fonksiyonel sonuçlara ve yürüme mesafesine olan etkisine dair farklı sonuçlar bildirilmiştir. Biz bu çalışmada ameliyat öncesi aneminin kalça kırıklı hastalarda cerrahi sonrası mortalite ve hastanede kalış süresi üzerinde etkisi olup olmadığını inceledik. Çalışmaya kliniğimizde parsiyel protez yapılarak tedavi edilen 135 kalça dahil edildi. Hasta dosyaları retrospektif olarak incelendi ve hastalara ameliyat öncesi anemisi olanlar ve olmayanlar olarak iki gruba ayrıldı. Her iki hasta grubu ortalama yaş, morbidite dağılımı, preoperatif ve postoperatif hemoglobin değerleri, cerrahi öncesi ve sonrası hastane yatış süreleri ve transfüzyon gerekliliği açısından istatistiksel olarak değerlendirildi. Yaptığımız çalışmada anemisi olan hastalarda preoperatif dönemde yatış süresinin anemisi olmayan gruba göre anlamlı derecede fazla olduğunu, ancak iki grup arasında postoperatif yatış süresi arasında anlamlı fark olmadığını saptadık.

After surgical intervention, the patient’s complaints completely disappeared in the post-operative period. This should come to mind in the differential diagnosis of knee pain.

Osteochondromas are the most common benign bone tumors comprising 9% of all bone tumors and 35% of benign bone tumors Porter (2000) [2]. Hereditary exostoses (HME) is an autosomal dominant disorder with variable penetrance characterized by multiple osteochondromas near joints. It is one of the most commonskeletal dysplasias with a frequency of about 1.18%. In this study, we aimed to present a case with a drop foot resulting from osteochondromas of proximal tibia and fibula and help to guide the clinicians in differential diagnosis according to SCARE criteria Agha (2016) [3].

Objective: In this study, the results of open release surgeries performed for the stiff elbows wh... more Objective: In this study, the results of open release surgeries performed for the stiff elbows which were caused by intrinsic, extrinsic or by combination of both factors were evaluated. Methods: Twenty-three elbows of 22 patients who could not perform functional elbow motions were surgically treated between January 2005 and December 2012. The mean age was 30.6±11.4 years (16 to 67 years), and the average follow-up period was 81 months (58 months to 12 years). Elbow arc of motions was recorded pre-and postoperatively. Patients were evaluated clinically by using quick Disabilities of the Arm, Shoulder and Hand (DASH) self-report questionnaire, and MAYO elbow performance scores. Results: The mean preoperative elbow arc of motion was 42 degrees (0-90), and it became 109 degrees (70-140) postoperatively after at least 58-month follow-up. The average increase was 67.8±25 degrees (30-125). The mean Quick DASH score was 18.2±12.7 (6.8 to 63.6). According to MAYO elbow rating system, one patient who had experienced infection following the initial fracture treatment had poor result, three patients had fair results, 17 patients had good results, and one patient who had bilateral elbow stiffness had excellent results. Conclusion: Although good results can be achieved by open release of the stiff elbows, one must keep in mind that preventing the stiffness rather than solving this problem would bring better results. As 15 patients with stiff elbows in this study had a previous surgery performed for an elbow region fracture, stabile fixation, meticulous hemostasis, shorter immobilization period and early rehabilitation can be considered as the most important steps for avoiding elbow stiffness where surgery is indicated.

Obezitenin giderek artması ve toplumda ortalama yaşam beklentisinin uzamasına bağlı olarak osteoa... more Obezitenin giderek artması ve toplumda ortalama yaşam beklentisinin uzamasına bağlı olarak osteoartrit görülme sıklığı giderek artmaktadır. Son çalışmalarda, 60 yaş üzeri toplumda insanların %13'ünde semptomatik osteoartrit (OA) görüldüğü bildirilmiştir. Son 20 yılda bu hastalarda ağrıyı azaltmak ve fonksiyonları koruyabilmek amacıyla minimail invaziv yöntemler kullanılarak uygulanabilen birçok ilaç geliştirilmiştir. Biz bu çalışmamızda, Kellgren-Lawrence evrelemesine göre evre 2 ve 3 gonartrozu olan hastalarda trombositten zengin plazma (PRP) enjeksiyonunun ağrı ve diz fonksiyonları üzerine etkilerini incelemeyi amaçladık. Gereç ve Yöntem: Çalışma için PRP enjeksiyonu yapılan 73 hastanın medikal kayıtları retrospektif olarak incelendi. Kayıtları eksik olan veya takiplere gelmeyen 29 hasta çalışmaya dahil edilmedi. Çalışmaya dahil edilme kriterleri; Kellgren-Lawrence evre 2 veya 3 gonartrozu olmak, 50 yaşından büyük olmak, lomber herniasyon kaynaklı ağrısı olmamak ve medikal kayıtlarında değerlendirme formlarının tam olarak bulunmasıydı. Bulgular: Değerlendirme sonrasında hastaların 37'sinde (%84) VAS skorlarında iyileşme saptandı (p<0,001). Hastaların 5'i (%11) ağrılarında değişiklik olmadığını tariflerken 2'si (%5) ağrılarının dahada ilerlediğini bildirdi. KSS sonuçları değerlendirildiğinde hastaların 40'ında (%90) iyileşme saptandı (p<0,001). Hastaların 4'ünde (%10) ise değşiklik saptanmadı. Hastaların hiçbirinde KSS skorlarında kötüleşme saptanmadı. Sonuç: Sonuç olarak, PRP enejksiyonu ile tedavi ucuz, kolay ulaşılabilir ve ağrıyı azaltan bir tedavi yöntemidir. PRP enjeksiyonu ile diz fonksiyonları orta-ileri evre gonartrozu olan hastalarda korunabilir. Kullanılacak enjeksiyon sayısı, sıklığı ve zamanı konusunda net sonuca varmak, kullanılacak trombosit konsantrasyon miktarının net olarak belirlemek adına ilerde daha fazla sayıda katılımcının birlikte çalıştığı, randomize ve karşılaştırmalı çok merkezli çalışmalar yapılmasını önermekteyiz.

This study aims to evaluate changes in surgical strategy and orthopedic epidemiology, and to compare the frequency of surgeries before and during the COVID-19 pandemic. Methods. For periods from April 1 to May 31 in years 2019 and 2020, retrospective data on patient demographics and types of orthopedic surgical procedures were obtained from hospital databases in a tertiary referral hospital. Results. During the COVID-19 pandemic, the most common orthopedic surgical procedures performed were trauma surgery (n = 81), while other procedures were referred to oncology (n = 19), biopsy (n = 11), debridement (n = 10), amputation (n = 6), surgery of dysplastic hip (n = 5) and knee ligament repair (n = 1). Aim: Mathematical relationships between bones in close proximity are useful for determining the p... more Aim: Mathematical relationships between bones in close proximity are useful for determining the proper length of comminuted or severely displaced bones during surgical reconstruction. This study examined the relationship between the lengths of the clavicle and ulna to develop a length propotion formula that can facilitate obtaining the proper length of the clavicle during surgery for displaced and comminuted fractures. Methods: The study enrolled 130 individuals (76 males, 54 females) who were healty for their upper extremities and was seen in our orthopedics outpatient clinic during April 2019 to June 2019. The right and left clavicles and ulnas of each individual were measured. All measurements were peformed by two of the authors in an blinded fashion. Results. Mean overall age was 32.2 years. There were significant correlations between the clavicle and ipsilateral ulnar lengths (p=0.001). However, there was a length difference between the right and left clavicles. The left clavicle was 9.00 ± 2.16 mm longer than the right in males and 7.13 ± 2.03 mm longer in females (p=0.001). There was no significant length difference between the right and left ulnas in either sex (0.84 ± 1.033 mm in males and 0.52 ± 0.818 mm in females). Conclusion. There is a significant ratio between the clavicle and ulna lengths on both the right and left sides in both sexes, but it isn't the same for males and females. We propose that these length proportion formula can be used for determining the clavicle length for the surgical treatment of comminuted fractures with plates and particularly with nails.

The differential diagnosis includes benign and malignant lesions. Case presentation: A 23-year-old European woman presented at an outpatient clinic with a 9-month history of a painless swelling on the right posterolateral side of her chest wall. The case reported here is of a very rarely seen parosteal osteosarcoma of the rib that was treated with wide resection and chest wall reconstruction. There was no evidence of local recurrence or distal metastasis after a 1-year follow-up. Conclusion: Parosteal osteosarcoma is a locally aggressive malignant tumor, and resection with a wide margin is the most appropriate treatment. Correct diagnosis of parosteal osteosarcoma is challenging for an orthopedic surgeon. Although rare, in the differential diagnosis of lesions located on the ribs, parosteal osteosarcoma should be considered and a systematic diagnostic approach should be taken.

The disease typically affects one side of an epiphysis (usually the medial side). A case in whom both the medial and lateral sides of the epiphysis were involved is described. Thus, the use of the descriptor "hemimelica" is not always appropriate. Although rare, Trevor's disease should be kept in mind during the differential diagnosis of patients with ankle pain and a mass. Other possible joints should be examined to explore multiple involvement. Surgical treatment, only on lesions causing impingements, may improve functional status and reduce pain.

Biz bu çalışmada ameliyat öncesi aneminin kalça kırıklı hastalarda cerrahi sonrası mortalite ve hastanede kalış süresi üzerinde etkisi olup olmadığını inceledik. Çalışmaya kliniğimizde parsiyel protez yapılarak tedavi edilen 135 kalça dahil edildi. Hasta dosyaları retrospektif olarak incelendi ve hastalara ameliyat öncesi anemisi olanlar ve olmayanlar olarak iki gruba ayrıldı. Her iki hasta grubu ortalama yaş, morbidite dağılımı, preoperatif ve postoperatif hemoglobin değerleri, cerrahi öncesi ve sonrası hastane yatış süreleri ve transfüzyon gerekliliği açısından istatistiksel olarak değerlendirildi. Yaptığımız çalışmada anemisi olan hastalarda preoperatif dönemde yatış süresinin anemisi olmayan gruba göre anlamlı derecede fazla olduğunu, ancak iki grup arasında postoperatif yatış süresi arasında anlamlı fark olmadığını saptadık.
