In some case reports, changes in PSA levels after acute myocardial infarction (AMI) have been reported. In this study we evaluated variations in PSA levels post-AMI. Twenty-six male patients who had PSA levels within reference limits were included in the study. The diagnosis of AMI was confirmed by clinical findings, ECG (electrocardiogram) and cardiac marker studies. Serum total PSA (tPSA) and free PSA (fPSA) levels were measured at days 0 (day of admission), 1, 2 and 3 after AMI. PSA/albumin ratio was also calculated in order to evaluate the effect of dilution. A statistical analysis of the results of all patients revealed significant decrease in tPSA levels and tPSA/Albumin ratio at day 2 when compared to days 0 and 3, which showed a similar pattern. Methods: Sixty-eight patients who presented with acute anterior MI were enrolled in the study. An electrocardiogram was performed before and at 30, 60, 90 and 120 minutes after the start of FT. PWD was defined as the difference between maximum and minimum P wave duration on standard 12-lead surface electrocardiogram. A multivariate logistic regression model was used to assess whether PWD was predictor of IRA patency and ST-segment resolution (STR) on electrocardiogram. The main purpose of this study was to investigate the role of ghrelin in the pathogenesis of coronary slow flow (CSF) together with endothelial functions. Methods: Twenty-five patients having normal coronary arteries with CSF and 25 controls with normal coronary flow were included into the study. The quantitative measurement of coronary blood flow was performed for each coronary artery using the thrombolysis in myocardial infarction (TIMI) frame count (TFC) method. Ghrelin levels were measured using the enzyme-linked immunosorbent assay method from venous blood samples. Endothelial functions were evaluated from the brachial artery with the flow-mediated dilation (FMD) and nitrate-related dilation methods. Results: There was a significant difference in terms of mean TFC values between the control and CSF groups (p < 0.001 for all coronary arteries). The mean FMD percentage among patients with CSF was lower than that of the control group (5.9 ± 0.8 vs. 10.7% ± 1.1%; p < 0.001). A moderate negative correlation was observed between the FMD percentages and the TFCs. There was no relationship between the TFC and ghrelin levels. Conclusion: Plasma ghrelin levels seem to be uninfluential while impaired endothelial functions play an important role in the etiopathogenesis of CSF.

The study population included 25 patients aged 56 +/- 18 years with non-dipper hypertension and 25 age- and sex-matched patients with dipper hypertension. They underwent conventional Doppler echocardiography and color tissue Doppler from apical 4- and 2-chamber views. In non-dipper patients, diastolic left ventricular function was reduced significantly. Twenty-four patients (mean age: 55+/-8 years) with heart failure were enrolled to the study. All were given metoprolol succinate with titrated target dose of 50 mg/d controlled release tablets for 1 month. Transmitral flow and pulmonary venous flow, systolic, diastolic and atrial reversal flow velocities were measured, and compared with the pretreatment values. Heart rate significantly decreased. No significant change was observed in ejection fraction. Lymphomatoid granulomatosis is a rare, diffuse, large B-cell lymphoma that is positive for Epstei... more Lymphomatoid granulomatosis is a rare, diffuse, large B-cell lymphoma that is positive for Epstein-Barr virus. A multiorgan process, it manifests itself chiefly in the lungs but can also affect the skin, nervous system, and kidneys. Cardiac involvement and pericardial effusion are very unusual. We report the case of a 62-year-old man with lymphomatoid granulomatosis involving the heart and lungs. Diagnosis was confirmed with wedge biopsy at pericardiotomy, and the patient was treated with cyclophosphamide, prednisolone, and vincristine. Although the patient was still symptomatic at 6-month follow-up, he was in partial remission with improved functional capacity.

Introduction: The aim of this study was to evaluate the relationship between the extend and sever... more Introduction: The aim of this study was to evaluate the relationship between the extend and severity of coronary artery disease (CAD) determined by the Gensini score and complete blood count parameters (white blood cell, hemoglobin, platelet, mean platelet volume, lymphocyte, neutrophil). Patients and Methods: Ninety patients with CAD underwent coronary angiography (40 females, mean age 61 ± 1.2 years) were included in this study. Patients with acute coronary syndrome and prior cardiovascular disease excluded from the study. The association between the extent and severity of CAD, which were assessed by the Gensini score, and complete blood count parameters was analyzed by a correlation analysis. Results: Coronary angiography revealed, 6 (6.7%) patients had three, 16 (17.8%) patients had two, and 24 (26.7%) patients had single-vessel disease; 44 (48.9%) patients had non-critical stenosis. The mean Gensini score was 19.1 ± 2.1. We found a relationship between white blood cell and neutrophil counts and the Gensini score. There was no relationship between Gensini score and the mean platelet volume and other parameters. Conclusion: The present study supports the hypothesis that inflammation is one of the main component in the pathogenesis of CAD.

We evaluated the results of the Turkish arm of the GOOD survey which investigated the cardiometab... more We evaluated the results of the Turkish arm of the GOOD survey which investigated the cardiometabolic risk profile and the control of blood pressure (BP) of adult hypertensive outpatients in 12 countries across Europe. A total of 218 hypertensive patients (139 females, 79 males; mean age 57.2±10.9 years) from Turkey were included in this pan-European survey. Blood pressure control (defined as BP <140/90 mmHg for nondiabetics and <130/80 mmHg for diabetics) and cardiometabolic risk factors such as diabetes mellitus, metabolic syndrome, obesity, sedentary lifestyle, and atherogenic dyslipidemia were evaluated in accordance with the 2003 ESH/ESC guidelines on management of hypertension. Control of BP was achieved in only 21.6% of the patients diagnosed with hypertension for a mean duration of 7.7±5.4 years. Inflammation is the most important mechanism of plaque disruption playing an essential role in ac... more Inflammation is the most important mechanism of plaque disruption playing an essential role in acute coronary syndromes. It is controversial whether the inflammatory mediators are the cause or the result in the development of plaque rupture. Stimulation of interleukins increases adhesion molecules, fibrinogen and plasminogen activator inhibitors, which cause the activation of inflammation and thrombosis. However, the importance of interleukins in acute coronary syndromes has not been clearly defined. We did not find any article concerning relations between the levels of serum interleukin (IL)-1b, IL-2, IL-8 and tumor necrosis factor (TNF)-a in patients with unstable angina pectoris (UAP). So the aim of this study was to determine the levels of serum IL-1b, IL-2, IL-8 and TNF-a during the early stage of UAP. Methods and results: Thirty-seven patients with UAP (12 females and 25 males; mean age, 57.59/9.7 years) within 6 h of admission and 20 healthy volunteers (eight females and 12 males; mean age, 51.39/6.3 years) were included in the study. IL-1b, IL-2, IL-8 and TNF-a levels were measured using the enzyme-linked immunosorbent assay method. Patients with acute or chronic inflammation, renal failure or chronic heart failure were excluded from the study. The age, gender and risk factors of the study and control groups were similar. The levels of IL-1b, IL-8 and TNF-a were significantly increased (p B/0.0001, p B/0.001 and p B/0.016, respectively) in patients with UAP. There was no difference of IL-2 levels between the UAP group and controls. Conclusion: We detected high levels of IL-1b, IL-8 and TNF-a in patients with UAP during early phase. We suggest that proinflammatory cytokines (e.g. IL-1b, IL-8, TNF-a) may play an important role in the development of atherosclerosis and its complications.

Background: We aimed here to investigate hydration status by echocardiography in end stage renal ... more Background: We aimed here to investigate hydration status by echocardiography in end stage renal disease (ESRD) patients. Methods: 25 ESRD patients [15 males; mean age: 54.0±16.6 years; 13 hemodialysis; 12 peritoneal dialysis] were considered eligible for this study. We also examined 29 healthy volunteers as a control group (17 males; mean age: 46.5±12.8 years). Body composition analysis using the bioimpedance spectroscopy technique was performed for volume overload diagnosis. The ratio of extracellular water (ECW) to height was used as volume indices. The aortic elastic parameters were calculated by echocardiography. A correlation analyses was performed between the ratio of ECW to height indicating the volume overload and the aortic elastic parameters e.g. Aortic strain (AS), Aortic distensibility (AD) and Aortic stiffness index (ASI). Results: The ratio of ECW to height that indicates volume overload in ESRD patients was considerably higher than that in the control group (10.25±1.98 L/m vs 8.66±1.22 L/m, p=0.001). There was a negative correlation between the ratio of ECW to height and AS and AD and a positive correlation between the ratio of ECW to height and ASI. Conclusion: Given the importance of the diagnosis and follow up of volume overload, the results show that aortic elasticity measurements, being easy to perform and replicate, can be used for this purpose.

Coronary angiography revealed massive intracoronary thrombus, which completely occluded the distal part of the left circumflex coronary artery. The thrombotic segment and the rest of the coronary tree were free of atherosclerosis. The patient was treated with intravenous tirofiban, a glycoprotein IIb/IIIa inhibitor. A control angiography was performed one week later and showed total dissolution of the thrombus with tirofiban therapy.

Twenty-four patients (mean age: 55+/-8 years) with heart failure were enrolled to the study. All were given metoprolol succinate with titrated target dose of 50 mg/d controlled release tablets for 1 month. Transmitral flow and pulmonary venous flow, systolic, diastolic and atrial reversal flow velocities were measured, and compared with the pretreatment values. Heart rate significantly decreased. No significant change was observed in ejection fraction. When the post treatment values were compared with the pretreatment values, it was detected that isovolumetric relaxation time significantly decreased (p<0.05), mitral E wave and E/A ratio were significantly increased (p<0.05 and p<0.05, respectively) after the treatment. A multiorgan process, it manifests itself chiefly in the lungs but can also affect the skin, nervous system, and kidneys. Cardiac involvement and pericardial effusion are very unusual. We report the case of a 62-year-old man with lymphomatoid granulomatosis involving the heart and lungs. Diagnosis was confirmed with wedge biopsy at pericardiotomy, and the patient was treated with cyclophosphamide, prednisolone, and vincristine. Although the patient was still symptomatic at 6-month follow-up, he was in partial remission with improved functional capacity.

Introduction: The aim of this study was to evaluate the relationship between the extend and sever... more Introduction: The aim of this study was to evaluate the relationship between the extend and severity of coronary artery disease (CAD) determined by the Gensini score and complete blood count parameters (white blood cell, hemoglobin, platelet, mean platelet volume, lymphocyte, neutrophil). Patients and Methods: Ninety patients with CAD underwent coronary angiography (40 females, mean age 61 ± 1.2 years) were included in this study. Patients with acute coronary syndrome and prior cardiovascular disease excluded from the study. The association between the extent and severity of CAD, which were assessed by the Gensini score, and complete blood count parameters was analyzed by a correlation analysis. Results: Coronary angiography revealed, 6 (6.7%) patients had three, 16 (17.8%) patients had two, and 24 (26.7%) patients had single-vessel disease; 44 (48.9%) patients had non-critical stenosis. The mean Gensini score was 19.1 ± 2.1. We found a relationship between white blood cell and neutrophil counts and the Gensini score. There was no relationship between Gensini score and the mean platelet volume and other parameters. Conclusion: The present study supports the hypothesis that inflammation is one of the main component in the pathogenesis of CAD.

A total of 218 hypertensive patients (139 females, 79 males; mean age 57.2±10.9 years) from Turkey were included in this pan-European survey. Blood pressure control (defined as BP <140/90 mmHg for nondiabetics and <130/80 mmHg for diabetics) and cardiometabolic risk factors such as diabetes mellitus, metabolic syndrome, obesity, sedentary lifestyle, and atherogenic dyslipidemia were evaluated in accordance with the 2003 ESH/ESC guidelines on management of hypertension. Control of BP was achieved in only 21.6% of the patients diagnosed with hypertension for a mean duration of 7.7±5.4 years. It is controversial whether the inflammatory mediators are the cause or the result in the development of plaque rupture. Stimulation of interleukins increases adhesion molecules, fibrinogen and plasminogen activator inhibitors, which cause the activation of inflammation and thrombosis. However, the importance of interleukins in acute coronary syndromes has not been clearly defined. We did not find any article concerning relations between the levels of serum interleukin (IL)-1b, IL-2, IL-8 and tumor necrosis factor (TNF)-a in patients with unstable angina pectoris (UAP). So the aim of this study was to determine the levels of serum IL-1b, IL-2, IL-8 and TNF-a during the early stage of UAP. Methods and results: Thirty-seven patients with UAP (12 females and 25 males; mean age, 57.59/9.7 years) within 6 h of admission and 20 healthy volunteers (eight females and 12 males; mean age, 51.39/6.3 years) were included in the study. IL-1b, IL-2, IL-8 and TNF-a levels were measured using the enzyme-linked immunosorbent assay method. Patients with acute or chronic inflammation, renal failure or chronic heart failure were excluded from the study. The age, gender and risk factors of the study and control groups were similar. The levels of IL-1b, IL-8 and TNF-a were significantly increased (p B/0.0001, p B/0.001 and p B/0.016, respectively) in patients with UAP. There was no difference of IL-2 levels between the UAP group and controls. Conclusion: We detected high levels of IL-1b, IL-8 and TNF-a in patients with UAP during early phase. We suggest that proinflammatory cytokines (e.g. IL-1b, IL-8, TNF-a) may play an important role in the development of atherosclerosis and its complications.

Background: We aimed here to investigate hydration status by echocardiography in end stage renal ... more Background: We aimed here to investigate hydration status by echocardiography in end stage renal disease (ESRD) patients. Methods: 25 ESRD patients [15 males; mean age: 54.0±16.6 years; 13 hemodialysis; 12 peritoneal dialysis] were considered eligible for this study. We also examined 29 healthy volunteers as a control group (17 males; mean age: 46.5±12.8 years). Body composition analysis using the bioimpedance spectroscopy technique was performed for volume overload diagnosis. The ratio of extracellular water (ECW) to height was used as volume indices. The aortic elastic parameters were calculated by echocardiography. A correlation analyses was performed between the ratio of ECW to height indicating the volume overload and the aortic elastic parameters e.g. Aortic strain (AS), Aortic distensibility (AD) and Aortic stiffness index (ASI). Results: The ratio of ECW to height that indicates volume overload in ESRD patients was considerably higher than that in the control group (10.25±1.98 L/m vs 8.66±1.22 L/m, p=0.001). There was a negative correlation between the ratio of ECW to height and AS and AD and a positive correlation between the ratio of ECW to height and ASI. Conclusion: Given the importance of the diagnosis and follow up of volume overload, the results show that aortic elasticity measurements, being easy to perform and replicate, can be used for this purpose.

Coronary angiography revealed massive intracoronary thrombus, which completely occluded the distal part of the left circumflex coronary artery. The thrombotic segment and the rest of the coronary tree were free of atherosclerosis. The patient was treated with intravenous tirofiban, a glycoprotein IIb/IIIa inhibitor. A control angiography was performed one week later and showed total dissolution of the thrombus with tirofiban therapy.
