Amaç: Sitotoksik kemoterapi, küçük hücreli dýþý akciðer kanseri (KHDAK) ileri evre tedavisinde ar... more Amaç: Sitotoksik kemoterapi, küçük hücreli dýþý akciðer kanseri (KHDAK) ileri evre tedavisinde artan bir öneme sahiptir. Ýleri evre hastalýkta kemoterapinin saðkalým süresinin etkisini araþtýran pek çok çalýþma yayýnlanmýþtýr. Gereç ve Yöntem: Biz çalýþmamýzda CEP (Siklofosfamid, Etoposid, Sisplatin) tedavisi alan ve çeþitli nedenler ile kemoterapi almayan KHDAK hastalarýnýn saðkalým sürelerini karþýlaþtýrmayý amaçladýk. CEP tedavisinde oluþan toksisiteyi deðerlendirdik. CEP tedavisi, evre IIIB ve evre IV KHDAK olan 37 hastaya (3 bayan, 34 erkek) verildi. Kemoterapi almayan evre IIIB ve evre IV KHDAK olan 13 hasta (1 bayan, 12 erkek) kontrol grubunu oluþturdu. CEP grubunda hastalara 21 gün arayla siklofosfamid 1 g/m2, cisplatin 50mg/m2 ve etoposid 80mg/m2 kombine kemoterapisi uygulandý. Bulgular: Tam yanýt hiçbir olguda alýnamadý, kýsmi yanýt sekiz olguda (% 21.6), stabil yanýt 13 olguda (%35.1) ve progresyon 16 olguda (%43.2) saptandý. CEP tedavisinde medyan saðkalým süresi 5.7±4.2 ay ve medyan yanýt süresi 2.97±3.1 ay saptandý. CEP tedavisine baðlý bulantý-kusma 17 hastada (% 45.9), anemi 10 hastada (% 27) ve nötropeni 1 hastada (% 2.7) geliþti. Kemoterapi almayan kontrol grubunda ise medyan saðkalým süresi 4.5±3.2 ay olarak saptandý. Her iki grubun medyan saðkalým süreleri karþýlaþtýrýldýðýnda istatistik olarak anlamlý bir fark olmadýðý görüldü (p>0.05). Her iki grupta bir yýllýk saðkalým oraný sýrasýyla n:37 (%8.9) ve n:13 (%7.6) idi. Sonuç: Sonuç olarak çalýþmamýzda elde edilen verilere göre yanýt oranlarýnýn yapýlan çalýþmalardan farklý olmadýðý, fakat saðkalým süresi ile bir yýllýk saðkalým oranlarýnýn çok düþük olduðu ve yan etki profilinin ise tolere edilebilir düzeyde bulunduðu gözlendi.

Cadmium accumulation in the lens has been implicated in cataractogenesis of chronic smokers. This... more Cadmium accumulation in the lens has been implicated in cataractogenesis of chronic smokers. This study was planned to evaluate whether or not in vivo cigarette smoke exposure causes cadmium accumulation in rat lens, and possible protective effect and mechanism of alpha-tocopherol (vitamin E) treatment on cataractogenesis. 28 male Wistar rats were randomly divided into four equal groups. Group 3 and 4 rats were exposed to cigarette smoke over ninety consecutive days, and Group 1 and 2 rats were treated in a similar fashion but exposed only to room air. Additionally, vitamin E was given to Group 2 and 4 rats. Significantly higher iron levels were observed in the lenses of Group 3 rats compared to other groups. With respect to cadmium, Group 3 and 4 rats had significantly higher levels compared to Group 1 and 2 rats. Although vitamin E treatment prevented iron accumulation in Group 4 rats, it had no effect on cadmium concentrations. Distinct histopathological changes observed in Group 3 rats were not present in Group 4 rats. Our study demonstrates that in vivo cigarette smoke exposure causes accumulation of cadmium in rat lens and IM vitamin E treatment does not affect this accumulation. The protective effect of vitamin E treatment on smoke exposed rat lens seems to be mediated by blockage of iron accumulation in the lens.

Mutations in HPGD have recently been reported to cause primary hypertrophic osteoarthropathy (PHO... more Mutations in HPGD have recently been reported to cause primary hypertrophic osteoarthropathy (PHO), a rare genetic disease characterized by digital clubbing, pachydermia, and periostosis. We screened HPGD mutations in six patients from three unrelated Turkish families with PHO, in which we showed one previously reported, p.A140P, and one novel, p.M1L, homozygous mutations. Both mutations co-segregated with the phenotype in all three families and were absent in 100 Turkish controls. These results confirm the presence of biallelic HPGD mutations in patients with PHO in an independent series from a different population.
