Most of the surveyed families (92.4%) consider mosquitoes as a nuisance and 83.8% say to do something about it. Among the latter, 43.5% spend money for their protection: incense coils (85.6%), insecticide sprays (55.5%), bednets (38.6%). In May 1990, families have spent a median sum of US$ 5.00, which was at that time about the price of an impregnated bednet. 89.5% of the surveyed families said they would agree to buy an impregnated bednet if the price was acceptable (median sum of US$ 4.00 is judged as acceptable). Impregnated bednets being an efficient method of malaria control, financially accessible and acceptable, their utilization should be further encouraged by the national programme of malaria control and primary health care.

However, a partial coverage, as seen in Belgium and France, only slows the spread of the wild virus, thus increasing the age at infection and the risk of complications. This is to be added to the fact that there are 5% primary vaccine failure (no antibody production) and 5% secondary vaccine failure (loss of antibodies over time). When introducing first immunization at 15 months of age it is thus very important to increase quickly the immunization coverage by immunization of all non-immune children entering school and by re-immunization of all teenagers.

Inaccurate perceptions of patients and those around them may lead to attitudes that delay seeking care. This study analyzes perceptions of danger and related delays to seek care in Kinshasa. Methods: Sixty deceased women who died between March and April 2004 were taken away from two Kinshasa mortuaries. History of disease and deaths were reconstructed through medical records and semi-structured interviews of family members and leaders. The Qualitative Software Research was used to conduct a qualitative analysis. Results: Perceived health warning signs had five manifestations: Specific clinical health warning signs, aggravation of non-specific signs, persistence of signs, indirect danger signs and superstitious signs. The incorrect perception of the signs was an important cause of late awareness of the danger and delayed decision to seek care. Conclusion: Misunderstandings of signs often delayed awareness of danger as well as decisions to seek appropriate care. Educational programs teaching health warning signs should be designed to promote the timely use of facilities.

In 26 cases the aortic juxtavalvular region (bulbus) was dilated above 2 SD compared to a control group. All patients with aortic regurgitation of with cusp prolapse belonged to this group of 26 cases. Aortic root dilatation seems to have a specificity of 89 p. 100, a sensitivity of 75 p. 100 and a negative predictive value of 98 p. 100 in relation to the occurrence of aortic insufficiency. The finding of an aortic root dilatation associated with a ventricular septal defect should incite to a closer supervision of non-operated patients should be used as an argument when discussing the surgical closure of the ventricular septal defect.

In the neonate group the left ventricular aortic gradient dropped from 49 +/- 20 mmHg to 15 +/- 16 mmHg. This improvement persisted in 57 p. cent of the cases after 2 years. In the older group (mean age 6 years) the gradient was reduced from 73 +/- 22 mmHg to 30 +/- 17 mmHg with maintained results in 78 p. cent after 2 years. Aortic incompetence occurred in the majority of cases but did not seem to be more important than after surgical commissurotomy. In critical stenosis, balloon aortic valvuloplasty seems to be a valuable alternative to surgery as a palliative measure before prosthetic valve replacement.
