Gallbladder extracorporeal lithotripsy was performed on 60 patients before laparoscopic cholecyst... more Gallbladder extracorporeal lithotripsy was performed on 60 patients before laparoscopic cholecystectomy. Of the 44 cases with solitary stones (range, 17-45 mm; mean +/- SEM, 26.9 +/- 0.1 mm), satisfactory fragmentation was obtained in 77.2%. Of the 16 cases with multiple stones (range, 11-25 mm; mean +/- SEM, 14.9 +/- 0.7 mm), satisfactory fragmentation was obtained in 18.75%. Minimal adverse effects were observed both clinically and macroscopically during surgery. Upon histologic investigation, only two small gallbladder lesions could be attributed to extracorporeal lithotripsy. No changes in blood chemistry tests were recorded. When carried out with high performance equipment, extracorporeal lithotripsy appears to be an interesting procedure that permits an appreciable reduction in the number of parietal wall incisions that need to be widened, therefore simplifying laparoscopic cholecystectomy when dealing with large stones.

Water-soluble contrast swallow (CS) is usually performed before refeeding for anastomosis assessm... more Water-soluble contrast swallow (CS) is usually performed before refeeding for anastomosis assessment after esophagectomy with intrathoracic anastomosis but the sensitivity of CS is low. Another diagnostic approach is based on analysis of computed tomography (CT) scan with oral contrast and of CT mediastinal air images. We undertook to compare them prospectively. Ninety-seven patients with an esophageal carcinoma operated by intrathoracic anastomosis were included prospectively in a study based on a CT scan at postoperative day 3 (without oral and intravenous contrast) and CT scan and CS at day 7. CT scan analysis consisted of assessing contrast and air leakage. In case of doubt, an endoscopy was done. A diagnosis of anastomotic leak was made in 13 patients (13.4%), in 2 cases before day 7 and in 3 beyond day 7. At day 3, 94 CT scans were performed, but the diagnostic value was poor. In 95 patients with both CS and CT scan at day 7, CS disclosed a leak in 5 of 11, and CT scan was abnormal in 8 of 11. Leakage of contrast and/or presence of mediastinal gas had the best negative predictive value (95.8%). Endoscopy was done in 16 patients with only mediastinal gas at day 7 CT scan. It disclosed a normal anastomosis in 11, fibrin deposits in 4, and a leak in 1. In comparison with CS only, CT at day 7 improves the sensitivity and negative predictive value for diagnosing an anastomotic leak. In case of doubt endoscopy is advisable. This approach provides an accurate assessment of the anastomosis before refeeding.

As with MRI, multidetector CT has now become an all in one examination using 2 D and 3 D multiplanar reformations with the ability to perform with high accuracy: a) vascular assessment using MIP reconstruction and b) biliary and pancreatic ducts assessment using minIP reconstruction. This review compares and illustrates the respective advantages of MRI and multidetector CT in the assessment of pancreatic diseases.

Two cases of recurrent macroscopic hematuria in which the diagnosis of left renal vein varices ha... more Two cases of recurrent macroscopic hematuria in which the diagnosis of left renal vein varices has suggested on CT are described. Bloody efflux was seen from the left ureteric orifice. On CT scans, tubulated contrast-enhanced densities in left perineal fat were seen. Selective renal angiography was normal. Selective left renal phlebography demonstrated intra and perirenal varices. In the two cases, embolization with metallic coli was successfully performed during left renal phlebography, to stop renal varices flux. Diagnostic and therapeutic modalities of renal varices are discussed with predominant place for CT and phlebography.

Décrire les différentes formes radio-anatomiques des TIPMP fistulisées à propos de 3 observations... more Décrire les différentes formes radio-anatomiques des TIPMP fistulisées à propos de 3 observations colligées. Revue de la littérature.Analyse rétrospective d’une série de 3 patients (2 hommes et 1 femme ; entre 65 et 81 ans), porteurs d’une TIPMP prouvée histologiquement, et explorés par scanner multi-coupe. Un cas a été corrélé à l’IRM. Une corrélation anatomo-pathologique était obtenue après résection chirurgicale dans 2 cas.Dans tous les cas, il y avait une atteinte segmentaire ou diffuse du canal pancréatique principal qui était dilaté, associée ou non à une atteinte des canaux secondaires au niveau de la tête du pancréas. La TIPMP était dégénérée dans 1 cas, il y avait des lésions de dysplasie dans 3 cas. L’imagerie montrait une fistule pancréatico-biliaire dans 1 cas, une fistule pancréatico-duodénale dans 3 cas, une fistule pancréatico-gastrique dans 3 cas, une fistule pancréatico-mésentérique dans 1 cas. Deux patients ont bénéficié d’une pancréa-tectomie réglée qui a confirmé les données de l’imagerie.L’apparition de fistules est un complication rare dans l’évolution des TIPMP. La constitution de ces fistules peut être liée à un envahissement tumoral ou d’origine mécanique. L’IRM et la TDM permettent de préciser le siège et l’extension de ces fistules et de guider la prise en charge chirurgicale.
